Complete change of seizure and spike lateralization in temporal lobe epilepsy at two separate monitorings.

OBJECTIVE: To report complete change of seizure and spike lateralization over time in bilateral temporal lobe epilepsies (TLE). METHODS: Repetition of video-EEG monitorings in 115 patients; 2 cases are reported in detail; 113 other severe partial epilepsies were included to estimate retrospectively the frequency of the reported phenomenon. RESULTS: In 2 cases, two video-EEG monitorings, separated by several months, revealed the first time one unilateral TL (temporal lobe) seizure and spike focus and the second time a distinct seizure and spike focus located in the opposite TL. The second mon…

Hippocampal dysgenesis associated with temporal lobe hypoplasia and arachnoid cyst of the middle cranial fossa.

We describe two children with left hippocampal dysgenesis in association with temporal lobe hypoplasia and arachnoid cyst of the middle cranial fossa. The hippocampus showed an abnormal globular shape and blurred internal structure in both patients. One of the patients had juvenile myoclonic epilepsy without evidence of seizure onset in the abnormal temporal region. The other patient did not have epilepsy; however, he showed developmental language disorder with a probable relationship to the left temporal abnormalities.

Electrophysiological, neurochemical and regional effects of levetiracetam in the rat pilocarpine model of temporal lobe epilepsy.

This study compared levetiracetam (Keppra) with reference antiepileptic drugs (AEDs) in the rat pilocarpine model of temporal lobe epilepsy. Electroencephalogram (EEG) recordings showed that i.p. administration of valproate (300 mg/kg), phenobarbital (5 mg/kg) and clonazepam (0.5 mg/kg) all significantly delayed the appearance of the first epileptic spike discharge in hippocampus as well as synchronous epileptiform activity in hippocampus and cortex. In contrast, i.p. administration of levetiracetam (17 mg/kg) only significantly delayed the appearance of the latter. This was corroborated by fi…

Arachnoid cyst of the frontal part of the temporal lobe producing exopthtalmos CT and MRI evaluation.

Twenty-three patients with an arachnoid cyst in the frontal part of the temporal bone, which produced exopthtalmos, were evaluated with CT and MRI. The majority of the patients had the peak of the bowing of the bony orbit wall in the posterior third (20/23) and only three in the middle. Arachnoid cyst are classified as small (13/23), medium (5/23) or large (5/20) size. Exopthtalmos are also classified as mild, moderate or large. CT and MRI clearly revealed the exopthtalmos, the bone remodelling process and the relationship of the orbit muscles, optic nerve and posterior bony wall of the orbit.

Spatial working memory in Asperger's syndrome and in patients with focal frontal and temporal lobe lesions.

Spatial working memory (SWM) was investigated in 15 patients with Asperger's syndrome (AS) comparing their performance to 18 age- and IQ-matched control subjects. An additional comparison was made with 20 unilateral frontal excision patients [9 right (RFL); 11 left (LFL)] and with 38 unilateral temporal lobectomy patients [18 right (RTL); 18 left (LTL)], the frontal and temporal lobe patients having separate matched control groups. SWM was tested using the Executive Golf Task, a test that also measures spatial strategy formation. The AS group showed a substantial deficit on SWM, but no impairm…

The clinical-pathogenic mechanisms of hippocampal neuron loss and surgical outcomes in temporal lobe epilepsy.

A retrospective study was carried out to determine whether a prior cerebral injury or medical illness was associated with hippocampal sclerosis in intractable, surgically treated temporal lobe epilepsy (TLE), or whether there was evidence for progressive hippocampal neuron damage from repeated seizures. Temporal lobe epilepsy patients (n = 162) from one epilepsy centre were retrospectively and blindly catalogued into groups based on the presence or absence of an initial precipitating injury (IPI) and whether, when an IPI was present, it had involved seizures (independent variables). Patients w…

Localizing value of seizure manifestations of temporal lobe epilepsies and the consequence of analyzing their sequential appearance.

We investigated the localizing and lateralizing value of principal seizure manifestations in temporal lobe epilepsies (signal symptoms, oroalimentary automatisms, somatomotor manifestations, unilateral dystonic posturing, ictal speech, motionless stare) of 223 complex partial seizures in 50 patients. All the patients had invasive long-term monitoring with the combined implantation of intracerebral electrodes in and subdural electrodes on the bilateral temporal lobes. Postoperative freedom from seizures was ascertained for longer than one year. We found that 35 patients had amygdalohippocampal …

[Alexia with agraphia produced by localized infarction in the inferior posterior region of the left temporal lobe].

There have been several reports on alexia with agraphia due to hemorrhage or trauma in the inferior posterior region of the left temporal lobe since Yamadori (1982) first reported a case of cerebral hemorrhage. We presented the first case of alexia with agraphia due to a circumscribed infarct in the inferior posterior region of the left temporal lobe. A 65-year-old right-handed man had an acute onset of inability to read Japanese "kana" letters and to write any letters. He had no difficulty in speaking or understanding. Neurological examination on admission was normal except for homonymous qua…

An autopsy case of atypical senile dementia with atrophy of the temporal lobes--a clinical and histopathological report.

A man aged 70, descendant of an apparently healthy family, showed disorientation, delusional ideas and rages at 66. Later there was slowly advancing deterioration with muteness, disorientation and dysphagia. He died of cardiac failure. There was the diffuse atrophy of the cerebrum, which was remarkably accentuated on both temporal lobes (poles, T2 and T3), where the loss of nerve cells and proliferation of astrocytes were found in the cortex and pallor and conspicuous fibrillary gliosis were noted in the white matter. These findings fundamentally suggest Pick's disease. On the one hand, numero…

Contralateral piriform cortex alterations after surgery for temporal lobe epilepsy: A pilot MRI volumetry study.

PURPOSE: Temporal lobe epilepsy (TLE) is increasingly conceptualized as a network disorder, with the piriform cortex as an implicated region. We characterized postoperative contralateral piriform cortex-cortical amygdala (PCA) structural and perfusion-related changes in a small seizure-free TLE cohort. METHODS: Nine patients with drug-resistant TLE who underwent epilepsy surgery and achieved postoperative seizure freedom were included in this single-center pre-post pilot study. Bilateral PCA was manually delineated on 3D T1-weighted MRI using ITK-SNAP, and volumes were normalized to total int…

White matter-guided transtemporal corridors: An anatomical blueprint for safe access through the temporal lobe.

BACKGROUND: Transtemporal approaches provide access to deep temporal and intraventricular lesions but may traverse critical white matter pathways, making tract-aware planning important for minimizing postoperative deficits. Therefore, the microsurgical anatomy of commonly used transtemporal corridors, the fibers encountered along each approach, and their tract-specific risk profiles should be defined to guide corridor selection. METHODS: Three formalin-fixed cadaveric brain specimens (six hemispheres) were dissected, proceeding stepwise from the cortical surface to the ventricular system. RE

Antecedents and Potential Risk Factors for Temporal Lobe Epilepsy: Data From a Canadian Epilepsy Clinic.

BACKGROUND: Despite extensive research on temporal lobe epilepsy (TLE), most work focuses on treatment outcomes and their determinants. Detailed evaluation of causal factors has not been adequately addressed in most published studies. This study aimed to explore potential etiologic risk factors for TLE through a chart review of prospectively collected data from a Canadian epilepsy clinic. METHODS: Consecutive patients diagnosed with TLE at a Canadian epilepsy clinic were included. Consecutive patients with genetic generalized epilepsy (GGE) served as a comparison group. The study database com…

Disrupted modular and hub topology in right temporal lobe epilepsy: a multimodal MRI network analysis.

Right temporal lobe epilepsy (rTLE) is associated with disruptions in functional brain networks and structural connectivity, yet underlying mechanisms remain unclear. This study investigated the alterations in modular interactions, connector hub (CH) topology, and related structural changes in rTLE patients. It included 30 rTLE patients and 30 matched healthy controls (HCs), all of whom underwent resting-state functional MRI (rs-fMRI), diffusion-weighted imaging (DWI), and volumetric MRI (vMRI). Functional networks were analyzed by assessing modular interactions, functional connectivity (FC), …

EEG microstate-derived dynamic network biomarkers for lateralization and structural etiology in temporal lobe epilepsy.

OBJECTIVE: Temporal lobe epilepsy (TLE) is the most common focal epilepsy but remains highly heterogeneous across hemispheric and structural etiology. This study aimed to characterize microstate-based network dynamics in TLE and evaluate their diagnostic value for seizure lateralization and structural etiology using machine learning. METHODS: Resting-state electroencephalography (EEG) recordings from 150 patients with unilateral TLE (71 right, 79 left) and 65 healthy controls (HCs) were analyzed. EEG signals were segmented into canonical microstates (A, B, C, D), and microstate-specific spati…

Temporal lobe disconnection in drug-resistant epilepsy associated with hippocampal sclerosis: how I do it.

BACKGROUND: Epilepsy surgery is the treatment of choice for drug-resistant epilepsy associated with hippocampal sclerosis. Patients over 50 years of age have an increased risk of postoperative complications after epilepsy surgery. METHODS: The authors present an operative video demonstrating right temporal lobe disconnection in a case of hippocampal sclerosis, along with an anatomical introduction and a section providing strategies to minimize complications. RESULTS: In selected cases, temporal lobe disconnection represents a viable alternative to resective procedures.

Does classical semiology ("5As") predict post-operative seizure outcome in patients with temporal lobe epilepsy?

OBJECTIVE: Temporal lobe epilepsy (TLE) accounts for 50%-70% of cases referred for epilepsy surgery. This study was designed to evaluate the distribution of the five hallmark seizure features-antecedent, aura, arrest, automatisms, and amnesia-considered collectively as the "5As," in individuals with temporal lobe epilepsy. We also analyzed the prognostic significance of 5As, if any, in predicting surgical outcome in drug-resistant TLE patients. METHODS: We included 684 patients who underwent clinical evaluation, neuropsychological assessment, high-resolution MRI, and VEEG monitoring followed …

Cortical electrical stimulation modulates hippocampal electrophysiological activity in mouse brain slices of temporal lobe epilepsy.

To investigate structural and functional alterations in the prefrontal cortex-hippocampus (PFC-HPC) circuit in temporal lobe epilepsy (TLE) and the regulatory effects of cortical electrical stimulation on hippocampal activity. A kainic acid (KA)-induced TLE mouse model was used. Diffusion tensor imaging (DTI) analyzed PFC-HPC structural connectivity. Optimized PFC-HPC brain slices were prepared for electrophysiological recording via high-density microelectrode arrays (HD-MEAs), and cortical electrical stimulation at different intensities was applied. DTI revealed abnormal diffusion metrics fra…

Distinct Theta-gamma network reorganization differentiates hippocampal sclerosis and MRI-negative mesial temporal lobe epilepsy.

INTRODUCTION: Temporal lobe epilepsy (TLE), particularly mesial temporal lobe epilepsy (MTLE), often presents with visual working memory (VWM) impairments, with potential heterogeneity between hippocampal sclerosis (HS) and MRI-negative subtypes. However, task-related electrophysiological (EEG) evidence regarding brain network alterations during VWM processing in MTLE remains limited. This study aims to identify shared brain network alterations and their behavioral correlates in patients with MTLE and to further characterize subtype-specific differences between HS-MTLE and MRI negative-MTLE. …
