The clinical arm of the study confirmed our biomechanic findings by showing the loss of pronation function. This loss may be an important factor when planning reconstruction …

Juvenile bunions have different etiologies and require specific operative approaches. Any operative procedure to correct a juvenile hallux valgus deformity should correct all the components of the deformity, i.e. pronation of the hallux, the increased hallux valgus angle, the enlarged medial eminence, the increased intermetatarsal angle, and hypermobility or obliquity of the first metatarso-cuneiform joint with the intention of decreasing the rate of recurrence.

Each of six below-the-knee amputation specimens were transfixed to a wooden block and mounted to a jig on an amputee testing device preloaded with 5 N applied to the proximal phalanx and displaced at a constant rate of 2 mm/min. Load displacement curves were generated for the intact joint and after sequential incisions of the lateral capsule, the adductor hallucis, and the lateral slip of the flexor hallucis brevis tendon, which caused varus dislocation of the hallux. An extensor hallucis brevis tenodesis was performed after the varus dislocation. Division of the lateral capsule, the adductor,

Seventy three Camitz-Littler operations combining carpal tunnel release and palmaris longus oppenens transfer provided 90.7% good long term results. A technical modification can provide pronation.

The management of the upper extremity in traumatic tetraplegia is complex and extremely important for the rehabilitation of the patient. The evolution of present management is reviewed. The evaluation and classification of the tetraplegic patient is discussed with general recommendations for treatment.

Varus and valgus foot, often associated with equinus, are common problems in cerebral palsy. These deformities can interfere with standing or walking, aggravate shoe wear, and lead to painful calluses on pressure areas of the foot. The deformities are caused by a basic muscle imbalance. If the posterior or anterior tibial muscles are overactive, the foot will swing into varus. If the peroneal muscles predominate, the foot assumes a valgus position. However, the alignment of the hip and knee play an important role in the production of deformity. Correction of the deformity must be preceded by a

This work describes a split gastrocnemius musculotendinous surgical transfer procedure that may be used to correct an equinosupinatus deformity. The procedure was used to realign the foot of a patient whose deformity was iatrogenically induced by an improperly placed gluteal injection. The injection caused symptoms consistent with a sciatic nerve lesion, with resulting denervation, multiple leg atrophy, ensuing muscle imbalance, and equinosupinatus deformity. Correction was obtained by passing the medial portion of the gastrocnemius tendon and muscle deep to the lateral portion, around the lat

BACKGROUND: Reverse shoulder arthroplasty (RSA) for irreparable massive rotator cuff tear (mRCT) and cuff tear arthropathy (CTA) demonstrates satisfactory clinical outcomes. However, many studies have reported no significant improvements in internal and external rotation. To our knowledge, there have been no studies on new attempts to restore active internal rotation following RSA. The purpose of this study was to compare RSA alone and RSA with anterior latissimus dorsi and teres major (aLDTM) tendon transfer in patients with CTA and mRCT with combined loss of active elevation and internal rot…

OBJECTIVE: To study the reconstructional method and effect of shoulder joint function in the older obstetrical palsy with medial rotation contracture deformity.
METHODS: From April 1996 to July 1999, 7 patients of older obstetrical palsy were adopted in this study. Among them, there were 5 males and 2 females, aged from 13 to 21 years old. No previous operation history and the main deformity was medial rotation contracture of shoulder. During operation, these patients were treated with "Z"-shape elongation of the tendon of subscapular muscle, transfer of the tendons of latissimus dorsi and te

BACKGROUND: Brachial plexus birth palsy is frequently associated with internal rotation contractures of the shoulder as a result of muscle imbalance. The purpose of this study is to assess the effect of botulinum toxin A (BTX-A) injection in the subscapular (SC) muscle on external rotation and the need for tendon transfer for external rotation of the shoulder.
METHODS: A prospective comparative study was performed including 15 consecutive patients treated with BTX-A and a historic control group of 67 patients with mean age 30 months (SD 10). The BTX-A injection (2 IU/kg body weight) was perfo…

BACKGROUND: Chronic tears of the Achilles tendon with a tendon gap exceeding 6 cm are a surgical challenge. The purpose of this study is to report the long-term results of reconstruction of such chronic Achilles tendon ruptures with use of a free autologous gracilis tendon graft.
METHODS: Twenty-one patients underwent reconstruction of a chronic rupture of the Achilles tendon. Fifteen patients were available for clinical and functional assessment on the basis of anthropometric measurements, isometric strength testing, and the Achilles Tendon Total Rupture Score after a mean duration of follow

Anterior cruciate ligament injuries in the pediatric and adolescent population are becoming more common with the trends of increased participation in competitive athletics in younger children. The natural history of this injury is similar to that of the adult and results in frequent reinjury, instability, and debilitation. Delayed reconstruction has proven to be a clinically effective treatment method but requires a prolonged restriction of competitive athletics. Reconstruction in the acute and subacute period has been shown by many authors to be a successful, reproducible treatment regimen. N

Transposition of part of the pes anserinus was used to treat 96 knees in 78 patients with recurrent, habitual or permanent dislocations of the patella. The knees with habitual (48) and permanent (27) dislocations also had extensive release of superolateral contracture. The patients were reviewed at an average follow-up of 9 years 8 months. The medial stability of the patella had been improved in all the cases. There were no recurrences in 21 knees with recurrent dislocations. There were four recurrences (5.3%) in knees with habitual and permanent dislocations due to inadequate release of the s

A modified Bristow-Helfet-May procedure was performed for recurrent dislocation or subluxation of the glenohumeral joint in 207 patients (212 shoulders), whose average age at the time of surgery was 20.3 years (range, fourteen to forty-seven years). The procedure was modified by directing the coracoid segment and conjoined tendon over the superior border rather than through the substance of the subscapularis tendon and muscle. The indications were either documented recurrent anterior dislocation of the glenohumeral joint or subluxation with instability as demonstrated on examination with the p

This article reviews the indications and the operative technique for the medial calcaneal slide osteotomy for the treatment of posterior tibial insufficiency. Patient selection, expected results, and complications of this technique are discussed. When used in combination with flexor digitorum longus transfer, the medial calcaneal slide osteotomy is an effective method of treatment for the adult acquired flatfoot associated with insufficiency of the posterior tibial tendon.

Lateral ankle instability is frequently encountered when performing total ankle replacement and remains a challenge. In the present techniques report, I have described a modification of the Evans peroneus brevis tendon lateral ankle stabilization harvested through limited incisions using simple topographic anatomic landmarks. The harvested peroneus brevis is then transferred either to the anterior distal tibia concomitantly with total ankle replacement or through the tibia when performed after total ankle replacement and secured with plate and screw fixation. This modified Evans peroneus brevi
