Extensor tendon injuries are traditionally splinted with no motion for 3 to 4 weeks after repair. This may result in limitation of flexion because of extensor tenodesis at the site of repair. To prevent this, we used a dynamic splinting program opposite to the one that is used for flexor tendon repair, with an outrigger splint holding the fingers in extension and allowing full active flexion. Fifty-two patients who had extensor tendon repairs in the area from the wrist to the middle of the proximal phalanx were treated. Motion was begun 2 to 5 days after repair and was continued for approximat

Posterior skin loss of the distal lower leg enhances the risk of exposure of the Achilles tendon. Most commonly, these wounds are a sequela to peripheral vascular insufficiency or else posttraumatic in origin. As a consequence, local flaps or skin grafts frequently are inadequate options for achieving coverage. Free-tissue transfers have proven to be a reasonable alternative in these situations for preservation of tendon function or even limb salvage. In this series of 12 patients, small defects were best covered with fasciocutaneous flaps, whereas the larger and usually chronic, concomitantly

Twenty-eight posterior tibial tendon transfers through the interosseous membrane were performed to correct spastic equinovarus. All patients improved their gait, and 82% discarded their brace. Foot striking at the beginning of the stance phase was good or satisfactory in 82%; 68% had a neutral position of the heel. The overall score was good in 68%, satisfactory in 21%, and poor in 11%. The main requirements for obtaining good results with this type of transfer are: no fixed varus deformity preoperatively, age at operation between 5 and 10 years, and reinsertion near the midline of the foot.

One of the principal drawbacks to clinical implementation of magnetic resonance imaging is comparatively long imaging times involved. A case is reported here where the most useful image sequence for the diagnosis of Achilles tendinitis was also the shortest--a sagittal, low flip, angle-scan.

Criteria for evaluation of results after extensor tendon lesions are presented, which are easy to apply and based on angular measurements of active joint mobility. The scheme is analogous to the evaluation method for flexor tendon injuries published by Buck-Gramcko et al. in 1976 which is now widely used. A modification was necessary to treat the extension deficits more severely. A comparison is made to the evaluation method proposed by the Group of Erlangen, and both schemes are tested on 68 patients with extensor tendon injuries of different zones, various fingers and combinations. Lesions o

Rotator cuff injuries, especially in athletes, can be very disabling. A case of an isolated rupture of the subscapularis tendon in an arm wrestler is reported. Preoperative arthrogram and CT scan with intraoperative pictures are used to illustrate the pathology. Recommendation for treatment is surgical repair.

Twenty-three shoulders on which a Bristow-Latarjet operation had been performed on account of anterior glenohumeral instability were followed up for an average of 24 months (range 15-36). The series consisted of 12 recurrent dislocations, 5 chronic subluxations + 3 with occasional dislocations, and 3 anterior + inferior (multidirectional) subluxations. Four shoulders were classified as poor at follow-up. The reason for failure was excessively proximal attachment of the coracoid transplant in one case, while an objective diagnosis of multidirectional instability by reference to the autotraction

Extensor tendon involvement in three Smith's and two Galeazzi's fractures are reported. In two cases of previous Smith's fracture, the extensor indicis proprius tendon had been entrapped and ruptured in one case, and in the other the extensor digitorum communis tendon of the index was pierced by the fragment. Both showed a severe tenodesis effect on the index finger and thumb. In a new case of Smith's fracture, long extensor tendons of the fingers and thumb had been trapped beneath a dorsally displaced third fragment making closed reduction impossible. In the two cases of Galeazzi's fracture; …

The great upsurge in popularity of running activities has increased the number of athletes presenting with pathology of the Achilles tendon. A clinical and ultrasonic study was performed on 47 middle and long distance runners referred to the Authors with such problems. The results of this study can be grouped as follows: 1. paratendonitis: enlargement of the antero-posterior diameter of the tendon, and hyperechogenicity of Kager's triangle; 2. tendonitis (with or without paratendonitis): thickening of the tendon, with the presence of degenerative nodules; 3. enthesopathy: thickening of the dis

Lesions of the biceps tendon have been studied with plain radiographs and arthrograms of the intertubercular groove. Sometimes, however, especially when the rotator cuff has a full-thickness tear anteriorly, the sheath of the biceps tendon fails to fill with contrast medium, thus preventing diagnosis of dislocation of the tendon. To solve this problem the authors have tried sonography of the biceps tendon. The suitability of sonography for examining the intertubercular groove and the biceps tendon was assessed in ten volunteers. Thirty patients with chronic shoulder pain were subsequently stud

The author presents his experience with an established technique of flexor tendon lengthening by tenotomy at the musculotendinous junction. This technique can be used for digital stiffness of forearm origin when active extension is possible on flexion of the wrist. There must not be adherence in the carpal tunnel or in the digital sheath, and active flexion must be preserved. It can also be used for digital stiffness in addition to other techniques. This kind of lengthening has some advantages: there are no sutures in the tendon itself, and it allows early reeducation in association with dynam

In this article we have attempted to analyze the results of Zone II flexor tendon repair, staged flexor tendon reconstruction, and tenolysis based on cases taken from a single hand surgical practice. Formulas have been offered to determine the percentage return of motion at the PIP and DIP joints utilizing the total active and passive motion measurements. Variation in the formulas for each procedure are necessitated by the preoperative active and passive motion. A common classification system based on the percentage return following each procedure has also been utilized. By employing these ass

An end-result study was made of 16 cases of avulsion of the insertion of the biceps brachii tendon. The follow-up period spanned one to 25 years post-treatment (average, eight years). Fourteen of the 16 cases included in the study were acute cases with primary repair and reinsertion of the biceps tendon into the area of the radial tuberosity. One case, with the loss of function of the biceps muscle secondary to temporary paralysis of the musculocutaneous nerve, was explored. The tendon was found intact and did not require repair. The sixteenth case was included for comparison of a patient who

We have treated 73 patients with 107 injuries of the extensor tendon. We started with an elastic dynamic splinting means = 9 (2-27) days after operation. The duration of functional treatment was means = 40 (13-103) days. Infections were only seen in severe cases (12.3%), and there was no rupture of the sutured tendon. Good or excellent functions were found in 92% of the patients. To sum up, primary extensor tendon repair will provide superior functional results at all levels of injury by postoperative dynamic splinting.

Good functional results can be obtained with early conservative treatment of lacerations or ruptures of the terminal portions of the finger extensor tendons. If the injury is not treated or followed closely, the typical deformity can be seen after several months. If the appearance and the functional disturbance are severe, operative reconstruction is indicated. In these cases, the ruptured or avulsed tendon generally heals spontaneously in three months in a lengthened position. This observation has been the impetus to the investigation of a very simple operative procedure. Observations on the …

Eleven posterior tibial tendon transfers were performed in eight patients with spastic equinovarus with an average follow-up of 3 years (range 1 1/2-6 1/2 years). The main goals--to eliminate the need for braces and to improve gait--were achieved in all patients. An adequate operation technique kept the well-known postoperative complications, e.g., valgus and calcaneal deformity, to a minimum.
