Congenital tracheal stenosis is a serious disease associated with high operative mortality and morbidity, especially in the newborn period. Its surgical management involves either sliding tracheoplasty, which is associated with extensive dissection and recurrent stenosis, or with pericardial patch tracheoplasty that fails to address recoiling of the stenotic complete ring and collapse of the patch into the tracheal lumen. We report a modification of the latter technique, in which a half ring external stenting device is used to keep the rings from recoiling.

Our aim is to elucidate the rare case of Wegener granulomatosis in childhood: its clinical symptoms, treatment, complications and outcome. We present clinical case of 12-year-old boy with affection of airways, kidneys, skin, joints and eyes. The ways of origin and the methods of diagnosing of the ectopic thyroid gland have been described. The authors have shown the case of follicular carcinoma of ectopic lingual thyroid gland in 35 years woman. The effective surgery (labiomandibuloglossotomy and total extirpation of the tumor) and following irradiation therapy with 131I haven't revealed recurrence of the disease during 2.5 years observation. The permanent substitution therapy with thyroxine has secured the euthyreotic state for the patient.

The authors report a case presenting highly complex symptomatology. In fact, when the patient came under observation he had had a cough, dyspnea, dysphagia and dysphonia for approximately three months. The biopsy, taken by direct laryngoscopy, indicated the presence of candidiasis in the subglottic and tracheal areas. Laboratory tests indicated complete anergy and patient tested serum positive to HIV. During hospitalization acute dyspnea arose requiring emergency tracheostomy.

Complications and changes in tracheal mucosa after minitracheotomy were evaluated in 28 patients. Tracheal mucosa was inspected fibreoptically after the insertion of a minitracheotomy cannula, and then at 3-day intervals until the cannula was removed. Thereafter, assessments were made every third day until the mucosa was considered normal. A retrospective analysis was undertaken reviewing management of 67 patients treated for subglottic stenosis (SGS) at the Children's Hospital National Medical Center (CHNMC) between 1980 and 1985. Forty-seven (90%) of the patients were followed for at least 18 months from the time diagnosis was made. We report two patients with laryngeal manifestations of Wegener's granulomatosis to demonstrate the marked variability of presentation in this disease. The reported literature is reviewed and shows a similar spectrum with early and late onset of laryngeal granulomata, subglottic in location. Of all the patients with Wegener's granulomatosis reviewed, 8.5% will eventually develop a subglottic lesion. Therapy depends upon response to steroids, cytotoxic drugs, and radiation therapy. Usually surgical therapy will be necessary.

Although tracheostomy in relation to maxillofacial injuries has been performed for a long time, opinions differ about the necessity and indications for this procedure. In the present study 44 cases of tracheostomy in patients with maxillofacial trauma are reported and the results analysed.
