Fatigue and pain in children and adults with multiple osteochondromas in Norway, a cross-sectional study.

BACKGROUND: Multiple Osteochondromas (MO) is a rare skeletal disorder frequently needing orthopaedic surgery. High prevalence of pain has been reported, however fatigue has not previously been investigated. PURPOSE: Our aims were to investigate prevalence of fatigue and pain in Norwegian children and adults with MO. Furthermore to compare prevalence of fatigue with reported prevalence in other groups and explore some factors that may contribute to fatigue in this population. METHODS: Questionnaire data was obtained from 11 children and 21 adults, approximately one third of the estimated MO p

Clinical practice and nursing management of pre-operative skin or skeletal traction for hip fractures in elderly patients: a cross-sectional three-institution study.

BACKGROUND: Femoral fractures are a major healthcare problem worldwide. One of the most difficult issues is their preoperative care, which is still managed by either skeletal or skin traction in some countries, including Italy. These issues are discussed and compared with the contemporary literature. OBJECTIVE: This study aims to analyse the distribution of these treatment options within the orthopaedic community and the reasons for their use, as well as to identify how this may impact nursing care in terms of pain management, hygiene care, venous thromboembolism (VTE)prophylaxis and preventi

An audit of complications of fiberglass cast and hybrid cast for fractures of the foot, ankle and forearm in a Swedish emergency department.

Patients of all ages present to the Emergency Department (ED) with fractures that require immobilization using a cast. Various casting materials are used, all with advantages and disadvantages and there are considerable risks associated with fracture management using cast immobilization. The frequency and severity of complications from fiberglass or hybrid casts applied in the emergency setting has not previously been studied. The aim of this audit was to describe all the complications that occurred within 30 days in patients who had a fiberglass cast applied for immobilization of uncomplicate

To log-roll or not to log-roll - That is the question! A review of the use of the log-roll for patients with pelvic fractures.

Pelvic fractures are one of the most serious injuries in polytrauma - massive haemorrhage is a major complication and the leading cause of death. The assessment and early management of these injuries in the Emergency Department (ED) focuses on the recognition of haemodynamic instability and should follow Advanced Trauma Life Support (ATLS) guidelines. The log-roll technique, although an accepted practice in trauma care, has been reported to be inadvisable when a pelvic fracture is suspected. This paper, which includes a case study, briefly explains the pathophysiology and initial management of

Increasing incidence of All-Terrain Vehicle trauma admissions in the pediatric and adult populations: An evaluation of injury types and severity.

OBJECTIVES: All Terrain Vehicles (ATVs) are increasing in popularity and becoming larger and faster at a production level. As a Level I Trauma Center, we perceived a disproportionately high volume of ATV-related admissions. Our goal was to study injury patterns and severity in adult and pediatric populations. METHODS: All ATV-related trauma admissions at a single Level I trauma center were retrospectively analyzed over a seven-year period. RESULTS: On-road incidents were more likely to result in a higher average Injury Severity Score (ISS) (p < 0.05). Higher ISS also occurred in children, un

Neurological assessment.

Neurological system assessment is an important skill for the orthopaedic nurse because the nervous system has such an overlap with the musculoskeletal system. Nurses whose scope of practice includes such advanced evaluation, e.g. nurse practitioners, may conduct the examination described here but the information will also be useful for nurses caring for patients who have abnormal neurological assessment findings. Within the context of orthopaedic physical assessment, possible neurological findings are evaluated as they complement the patient's history and the examiner's findings. Specific neur

Acute to chronic pain transition in extremity trauma: A narrative review for future preventive interventions (part 1).

BACKGROUND: Several studies have been conducted over the last decade that describe the issue of pain and prognostic factors of acute to chronic pain transition post extremity trauma (ET). However, no thorough interventions to prevent chronic pain development in ET patients have yet been proposed. OBJECTIVES: This first part of a series of two articles aimed to provide a comprehensive analysis of literature on the epidemiology of pain post-ET, the impact of persistent pain on ET patients and the relevant biological transformations involved in acute to chronic pain transition. The second articl

Exploring the experiences of parents caring for infants with developmental dysplasia of the hip attending a dedicated clinic.

Specialised DDH (developmental dysplasion of the hip) clinics are developing around Ireland but are, however, variable in how they are operated. A DDH clinic was set up in the South-east of Ireland in 2002 with the goal of achieving an integrated care pathway between the orthopaedic surgical team and nursing team, working to an explicit protocol while also fostering a strong collaboration with the ultrasound department. This paper aims to explore the effectiveness of this dedicated clinic in the Southeast of Ireland.

The assessment and management of pain in an orthopaedic out-patient setting: A case study.

The management of pain is an important aspect of an orthopaedic nurse's role. The aim of this paper is to use an individual case study to demonstrate the role of an out-patient orthopaedic nurse in the identification, assessment and management of pain. This paper describes how pain was identified and managed for a patient in the orthopaedic outpatient department, highlighting that pain and its management are not isolated to the in-patient setting. The case study illustrates the importance of recognising pain and taking into account the numerous factors that can influence pain perception. The a

Development and testing of a patient-reported discharge readiness measure for daytime knee arthroscopy.

BACKGROUND: The lack of a specific assessment tool for knee arthroscopy patient leads to a lack of validity and accuracy of the assessment results, which affects the overall success of discharge preparation. This study aims to develop and psychometrically test the Patient-Reported Scale for assessing Discharge Readiness in patients undergoing daytime Knee Arthroscopy. METHODS: This was a quantitative empirical research design. Development of the patient-reported instrument entailed three stages: item pool generation, pretesting evaluation, and psychometric testing. Item pools were generated t
