Pneumatosis intestinalis revisited: associated imaging findings, clinical correlation, management, risk stratification and future perspectives.

BACKGROUND: Pneumatosis intestinalis (PI) is the imaging finding of air within the bowel wall. It constitutes a diagnostic challenge due to its broad differential and overlap in imaging appearances between benign and serious causes. A multidisciplinary approach is essential for accurate diagnosis and timely intervention, to optimize outcomes and minimize unnecessary procedures. OBJECTIVE: This article aims to synthesize the literature to lay out the current state of the art in diagnosing and managing PI and discuss future perspectives. We review a series of associated imaging features includi…

Autonomous robotic execution of nonlinear toolpaths for geometry-matched osteotomy in tooth autotransplantation: an in vitro study.

Tooth autotransplantation requires recipient socket preparation that matches donor root morphology while preserving surrounding bone. We developed an autonomous multi-axis robotic system that executes nonlinear, surface-conforming milling trajectories to create a geometry-matched socket and compared it with a static tooth-supported guide in forty 3D-printed mandibular models representing single-rooted and double-rooted anatomies. Recipient sockets were planned by offsetting the donor root surface by 0.5 mm and eliminating insertion axis undercuts. The robot executed the planned milling path wi

Thoracic splenosis initially misdiagnosed as lung neoplasms: An unusual case and literature review.

OBJECTIVE: Thoracic splenosis, implantation of autologous splenic tissue into the chest after diaphragmatic splenic injury, is rare and frequently mistaken for malignancy, leading to inappropriate treatment. This case highlights the risk of misdiagnosing post-traumatic thoracic splenosis as lung cancer. CASE REPORT: A 69-year-old smoking male patient presented with 1 month of thoracic pain; computed tomography revealed a 3.3 cm left lower-lobe mass suspected as lung cancer. For the past 20 years, he underwent splenectomy after splenic and diaphragmatic injury following a severe motor vehicle

Travel-Acquired Furuncular Cutaneous Myiasis Mimicking a Bacterial Abscess in a Pediatric Patient.

Furuncular cutaneous myiasis can closely mimic common bacterial abscesses, especially in regions where Dermatobia hominis is not endemic. Here, we present a case of a healthy six-year-old male who returned from a Caribbean cruise and developed four painful nodules on his scalp and arm that were initially presumed to be bacterial abscesses. Incision and drainage yielded little to no purulence. Operative exploration under general anesthesia revealed four intact D. hominis larvae, all of which were successfully extracted. This case underscores the importance of obtaining a travel history and cons

["Not everything that looks like a tumor..." - Pulmonary tularemia with hilar lymphadenopathy].

INTRODUCTION: Tularemia is a rare zoonotic disease caused by Francisella tularensis, whose pulmonary manifestations occur only sporadically in Central Europe and can be particularly challenging to diagnose. MEDICAL HISTORY: We report on a 58-year-old patient presenting with B-symptoms in whom a thoracic mass was detected on computed tomography. INVESTIGATIONS AND DIAGNOSIS: Further imaging studies revealed a metabolically active mediastinal mass with lymphadenopathy, suggestive of bronchial carcinoma. Only surgical mediastinoscopy with evidence of granulomatous-histiocytic inflammation, as w

[Mind the bat: Rare Infection as Lung Cancer Mimic - a Case Report].

HISTORY: A 63-year-old male patient presented with a 5-day history of fever and productive cough. His history revealed renovation work in an attic with exposure to dead bats. No improvement occurred under treatment with amoxicillin/clavulanic acid. DIAGNOSTICS: Chest CT demonstrated a lesion in the right upper lobe with mediastinal lymph node involvement, highly suspicious for lung carcinoma. Bronchoscopy, EBUS-TBNA, and thoracoscopic biopsy revealed inflammation without evidence of malignancy. PCR testing of the obtained tissue was positive for Francisella tularensis, subsequently confirmed

New-onset hepatic lesions in oncological patients: A challenging diagnosis.

The appearance of new-onset liver lesions is frequent during imaging follow-up of oncological patients. Most of these lesions will be metastases. But in the presence of atypical radiological findings, there are other diagnoses to consider. Hepatic abscesses, focal nodular hyperplasia-like in patients treated with platinum salts, or hepatocarcinoma in cirrhotic patients are examples of lesions that may appear in the imaging follow-up and should not be confused with metastases. It is essential to establish the nature of the lesion as this will determine the therapeutic management and might avoid

Meigs syndrome was misdiagnosed as a malignant ovarian tumor: a case report.

BACKGROUND: Meigs syndrome is characterized by the association of a benign ovarian tumor, typically an ovarian fibroma, with pleural effusion and ascites. CASE SUMMARY: This report presents a case of a 54-year-old woman who was misdiagnosed with malignant ovarian neoplasm due to the presence of significant abdominal distension and elevated CA125 levels. Initial imaging at multiple external facilities suggested a left-sided malignant ovarian tumor, leading to unnecessary delays in treatment. Upon admission to our institution in April 2024, imaging confirmed a large pelvic mass, and subsequent

A simple nomogram tool for predicting fetal chromosomal abnormalities based on ultrasound soft markers: a research note.

OBJECTIVES: Ultrasound soft markers (USMs) are associated with increased risk of fetal chromosomal abnormalities but lack standardized risk assessment methods, often leading to unnecessary amniocentesis procedures. We aimed to develop a practical nomogram tool to quantify this risk and help clinicians make more objective decisions about invasive testing, particularly in resource-limited settings. RESULTS: We retrospectively analyzed 565 pregnancies with USMs who underwent amniocentesis between 2016 and 2024. Our nomogram integrated six readily available clinical factors: maternal age, thicken

Pediatric nodular fasciitis of the orofacial region: A rare case report and literature review.

INTRODUCTION: Nodular fasciitis is a rare, benign, rapidly growing, self-limited myofibroblastic/fibroblastic pseudotumor that often mimics sarcomas, making its diagnosis particularly challenging. While it typically occurs in the subcutaneous tissues of the trunk and extremities, intraoral presentation is extremely uncommon. CASE PRESENTATION: We report a rare case of oral nodular fasciitis in a 10-year-old patient who presented with delayed eruption of the right lower premolars and a firm, rapidly enlarging, painless swelling in the corresponding region. An excisional biopsy was performed, f

An Unusual Presentation of Myocardial Ischemia and Coexisting Asthma Exacerbation With a Pseudo-Wellens Sign: A Case Report.

Cardiovascular complications, including myocardial ischemia, can occur during acute asthma exacerbations due to overlapping inflammatory and physiological mechanisms. ECG abnormalities and elevated cardiac biomarkers may mimic acute coronary syndrome, complicating diagnosis and management. We present a case of a 60-year-old woman with a history of long-standing asthma and hypertension who presented with chest pain and shortness of breath during an asthma exacerbation. Clinical evaluation revealed diffuse wheezing, deep T-wave inversions on ECG, and elevated cardiac biomarkers, initially raisin

Post-Surgical Pyoderma Gangrenosum After Breast Cancer Surgery: A Multidisciplinary Case Report.

Post-surgical pyoderma gangrenosum is a rare neutrophilic dermatosis that may occur after surgical procedures, mimicking a wound infection. Early recognition is crucial to prevent unnecessary debridement and worsening of lesions due to pathergy. We report the case of a 67-year-old woman who underwent nipple-sparing mastectomy for invasive breast carcinoma with immediate reconstruction using a tissue expander. In the early postoperative period, she developed an extensive sterile necrotic-ulcerative inflammation of the left breast, unresponsive to broad-spectrum antibiotics and repeated surgical

Chiari Malformation Type I With Concurrent Bilateral Optic Disc Drusen: Is Follow-up Necessary?

Pseudopapilledema caused by optic disc drusen (ODD) mimics the appearance of papilledema and usually presents as a diagnostic challenge. A young boy with known Chiari malformation type 1 (CM-1) was referred to the pediatric ophthalmology clinic for eye assessment to exclude papilledema due to elevated intracranial pressure (ICP). Despite the ophthalmic examination revealing bilateral optic disc elevation, multimodal imaging techniques such as fundus autofluorescence, optical coherence tomography (OCT), and B-scan ultrasonography are recommended to confirm the distinction between bilateral ODD

A Rare Presentation of Duplicate Superior Vena Cava Precipitating Premature Atrial Contractions: A Case Report and a Review of the Literature.

Millions of central lines are placed each year worldwide for life-saving measures. We present a case of left internal jugular (IJ) triple lumen catheter (TLC) placement for life-saving vasopressors, which appeared to be in the left mediastinum after a confirmed chest X-ray. After correlation with a previous MRI of the heart with and without contrast, duplication of the superior vena cava (SVC), also known as persistent left SVC (PLSVC), was discovered. PLSVC often causes no symptoms to affected individuals and is usually first found as an incidental finding discovered during thoracic surgeries

Primary Human Papillomavirus Testing and Other New Technologies for Cervical Cancer Screening.

Cervical cancer screening has saved the lives of millions in regions where routine gynecologic care is readily accessible. As screening continues to evolve away from cervical cytology to primary human papillomavirus (HPV) testing, robust prospective cohort data have allowed for precise risk stratification and improved our ability to identify those at greatest risk of high-grade dysplasia and decrease unnecessary diagnostic procedures. New technologies such as p16/Ki-67 dual stain testing and HPV methylation panels, which offer comparable performance to co-testing and can be developed into high
