Accurate identification of anatomical structures is essential for safe congenital cardiac surgery and for the development of assistive robotic systems. However, scalable annotation of open-heart surgical video remains a major challenge due to dynamic tissue motion, occlusion, and anatomical variability. The aim of this study was to develop and evaluate a human-in-the-loop segmentation and tracking pipeline for congenital open-heart surgery videos. A dataset of 72 annotated video clips comprising 27,461 frames was created from routine recordings of congenital cardiac surgery. Independent tracki

This article describes the operative stabilization of a flail chest due to traumatic serial rib fractures with extensive chest wall deformation and respiratory insufficiency. Initial conservative treatment including systemic and regional pain management and non-invasive positive pressure ventilation did not improve the pain or ventilation. Therefore, a single-port video-assisted thoracoscopic surgery (VATS) assisted internal fixation of the ribs was performed. The thoracoscopy enabled easy repositioning of the ribs and additionally an estimation of intrathoracic injuries.

The use of osteosynthesis material in shoulder and clavicle surgery is common in orthopedic surgery. Although migration is rare, it is widely documented and can lead to serious complications and even death. We present a rare case of trans-mediastinal migration of a Kirschner needle used to fix a clavicle fracture, which fortunately did not injure the mediastinal structures and was resolved favorably by a video-assisted thoracic surgery approach.

The aim of the study was to develop and assess the performance of a video-based augmented reality system, combining preoperative computed tomography (CT) and real-time microscopic video, as the first crucial step to keyhole middle ear procedures through a tympanic membrane puncture. Six different artificial human temporal bones were included in this prospective study. Six stainless steel fiducial markers were glued on the periphery of the eardrum, and a high-resolution CT-scan of the temporal bone was obtained. Virtual endoscopy of the middle ear based on this CT-scan was conducted on Osirix s

The conventional surgical approach for the removal of bronchogenic cyst in children is thoracotomy. Video-assisted thoracoscopic surgery is still under debate with the concerns of incomplete removal and injury to major intrathoracic structures. In this case report, we present an 8-year old patient who was successfully treated with robotic surgery (Intuitive Surgical, Mountain View, CA, USA). Robotic technology enables precise dissection and complete resection of the thoracic bronchogenic cyst without violating the capsule and adjacent major intrathoracic structures. She was discharged on posto

BACKGROUND: Robotic-assisted surgery is increasingly used for early stage non-small cell lung cancer resection, yet its impact on time to surgery and outcomes remains unclear.
METHODS: Using the National Cancer Database (2010-2022), adults with clinical stage I-II non-small cell lung cancer who underwent initial resection by open, video-assisted thoracoscopic surgery, or robotic-assisted surgery were identified for a retrospective cohort study. Prolonged time to surgery was defined as surgery >8 weeks after diagnosis. Multivariable logistic regression identified factors associated with longer

BACKGROUND: Adenomyosis is a chronic, debilitating condition characterised by the presence of endometrial- like glands and stroma within the myometrium. While hysterectomy remains the definitive treatment, uterus- sparing surgeries may be a possible option for patients desiring to maintain fertility. Surgical management, along with medical treatment and/or Assisted Reproductive Technology (ART), can improve outcomes.
OBJECTIVES: To provide a step-by-step video demonstration of robotic-assisted excision of diffuse adenomyosis affecting the posterior uterine wall.

We describe the case of a patient with myasthenia gravis and severe pulmonary emphysema who underwent concomitant bilateral video/robotic-assisted thymectomy with unilateral lung volume reduction surgery. We review the important pathophysiologic considerations that must be appreciated to ensure safe surgery in this unusual situation with two diseases that independently affect the respiratory system-each of which requires preoperative optimization.

Although data are lacking, the authors believe robotic-assisted thoracoscopic chest wall resection is safe and oncologically effective and might be associated with decreased postoperative pain and morbidity compared with traditional open techniques. The criteria for robotic thoracoscopic resection are similar to those used for video-assisted thoracoscopic surgery-guided resection with the exception that a robotic approach is feasible even for superior sulcus and first rib-based lesions. A totally robotic or hybrid approach may be used, with the latter including an incision over the chest wall

TECHNIQUE: Uni-portal video-assisted thoracoscopic lobectomy (UVATS) has become a well-established technique; however, performing a complete lymphadenectomy, particularly the removal of lymph nodes located below the left hemithorax (LHLN), remains a challenging procedure. To our knowledge, there are limited reports addressing the use of pause ventilation during UVATS for LHLN dissection. In this study, we propose the application of pause ventilation for facilitating LHLN dissection under UVATS. Through transnasal humidified rapid-insufflation ventilatory exchange (THRIVE), we innovatively impl

Thoracoscopic intercostal nerve cryoablation is increasingly used as an adjunct to the Nuss procedure for pectus excavatum to reduce postoperative pain, yet detailed stepwise descriptions remain limited. This video tutorial presents a practical, structured description of the operative technique with emphasis on reproducible procedural steps. The patient is placed supine with both arms elevated, and single lung ventilation is established, commencing with the left-sided procedure. Thoracoscopic access is obtained through small lateral ports, followed by carbon dioxide insufflation to improve vis

AIM: Soft tissue sarcomas are rare, accounting for only 1% of adult malignancies, with liposarcoma being a common subtype. However, mediastinal liposarcomas are extremely uncommon, comprising less than 1% of cases. Dedifferentiated liposarcoma (DDLPS) is a particularly aggressive variant, characterized by a transition from well-differentiated to high-grade non-lipogenic sarcoma. Due to its rarity and often asymptomatic nature until significant progression, mediastinal DDLPS presents a unique diagnostic and therapeutic challenge.

Primary hyperparathyroidism (pHPT) is typically caused by a parathyroid adenoma, although some cases arise from ectopic glands. Accurate preoperative localization is essential for guiding surgical strategy, particularly for mediastinal lesions. We report successful video-assisted thoracoscopic surgery (VATS) for an upper mediastinal parathyroid adenoma. A 68-year-old woman was referred for hypercalcemia during evaluation for recurrent ureteral stones. Laboratory tests revealed elevated serum calcium (13.2 mg/dl) and intact parathyroid hormone (iPTH) levels (266 pg/ml). Contrast-enhanced comput

Many institutions perform thoracoscopic surgery for pediatric extrapulmonary sequestration (EPS). This approach achieves good cosmetic outcomes due to the small skin incision in comparison to open surgery. However, an extension or additional incision is sometimes necessary to extract resected specimens from the thoracic cavity, which reduces the advantages of this procedure. We herein report 5 cases of two-site thoracoscopic surgery (TTS) using an extraction wound with left-sided EPS. In this procedure, a camera port was inserted inside the skin incision at the sixth intercostal space of the m

Ranked high in worldwide growing health issues, pleural diseases affect approximately one million people globally per year and are often correlated with a poor prognosis. Among these pleural diseases, malignant pleural mesothelioma (PM), a neoplastic disease mainly due to asbestos exposure, still remains a diagnostic challenge. Timely diagnosis is imperative to define the most suitable therapeutic approach for the patient, but the choice of diagnostic modalities depends on operator experience and local facilities while bearing in mind the yield of each diagnostic procedure. Since the analysis

BACKGROUND: The high resolution of computed tomography has found the pulmonary ligaments that consists of a double serous layer of visceral pleura, forms the intersegmental septum, and enters the lung parenchyma. This study aimed to investigate the clinical feasibility of thoracoscopic segmentectomy (TS) of the lateral basal segment (S9), posterior basal segment (S10), and both through the pulmonary ligament (PL).
METHODS: Between February 2009 and November 2021, 542 patients underwent segmentectomy for malignant lung tumors at Tokyo Women's Medical University Hospital (Tokyo, Japan). This st
