OBJECTIVES/HYPOTHESIS: Traditional semi-occluded vocal tract therapies have the benefit of improving vocal economy but, do not allow for connected speech during rehabilitation. In this study, we introduce a semi-occluded face mask (SOFM) as an improvement upon current methods. This novel technique allows for normal speech production, and will make the transition to everyday speech more natural. We hypothesize that use of an SOFM will lead to the same gains in vocal economy seen in traditional methods.

Surgery of the upper airway is performed in dogs for the correction of brachycephalic airway syndrome and laryngeal paralysis and for temporary or permanent tracheostomy. Although technically simple to perform, upper airway surgeries can lead to the development of significant postoperative complications. This article reviews complications associated with common surgical conditions of the upper airway. It involves a discussion of brachycephalic airway syndrome and associated respiratory and gastrointestinal complications. It also covers laryngeal paralysis with a focus on unilateral arytenoid l

High-speed digital imaging can provide valuable information on disordered voice production in voice science. However, the large amounts of high-speed image data with limited image resolutions produce significant challenges for computer analysis, and thus effective and efficient image edge extraction methods allowing for the batch analysis of high-speed images of vocal folds is clinically important. In this paper, a novel algorithm for automatic image edge detection is proposed to effectively and efficiently process high-speed images of the vocal folds. The method integrates Lagrange interpolat

This case report describes the diagnostic trial of an inspiratory wheeze in a 1.5-year-old Swiss Alpine goat. Left grade 4 laryngeal hemiplegia was diagnosed via laryngoscopy, whereas the severity of the hemiplegia was assessed according to the grading system used in horses. The results of clinical, radiographic, sonographic and endoscopic examinations as well as haematological, biochemical and serological analyses did not reveal the cause of the hemiplegia. Treatment with an antibiotic and vitamin B complex resulted in only slight improvement. A postmortem examination four months later reveal

REASONS FOR PERFORMING STUDY: Laryngoplasty is the technique of choice for treatment of laryngeal hemiplegia, with the aim of improving airway function and/or eliminating respiratory noise. However, there are no quantitative data in the literature describing the effect of laryngoplasty on upper airway noise or its relationship to upper airway mechanics in horses with laryngeal hemiplegia.
OBJECTIVES: To determine whether laryngoplasty reduces respiratory noise in exercising horses with laryngeal hemiplegia; and to establish whether the degree of upper airway obstruction can be predicted by up

Cricotracheal resection has been advocated in the management of severe subglottic stenosis. One of the possible complications of this procedure is injury to the recurrent laryngeal nerve (RLN). We describe a new technique in which electrodes are placed directly through the thyroid cartilage to monitor the RLN intraoperatively. Nine cats' left vocal cords were monitored, and 3 cats had unilateral vocal cord injury postoperatively: 1 right cord and 2 left cords. Even though this technique was efficacious, our ability to monitor the RLNs was difficult secondary to difficulty interpreting the nerv

OBJECTIVES: The effects of denervation on myosin heavy chain (MHC) expression in specific laryngeal muscles are characterized using gel electrophoresis. Observed temporal changes in MHC composition will then be used as a biologic marker in studies designed to develop strategies for laryngeal reinnervation and gene therapy.
STUDY DESIGN: Animal study using an adult rat model for laryngeal paralysis.
METHODS: In anesthetized rats the left recurrent and superior laryngeal nerve were divided. Animals were survived for 7, 14, 28, 90, and 180 days. Animals were euthanized and the thyroarytenoid (T

The dog's recurrent laryngeal nerve defect (1cm) was repaired by near vessels implantation of heat denaturation muscles as a bridge. Three months after operation, regenerative nerve neurite smoothly passed through muscle bridge, grew into distal isogenous nerves and formed function joint. The implanted vessels grew and branched in the muscle bridge. Six months later, the motor end plates of caryngeal muscle grew well, laryngeal electromyography showed that nerves contraction of laryngeal muscles built. Laryngoscopy showed that vocal cord moved to the normal position. The voice perfectly recove

Laryngeal reinnervation with the ansa cervicalis has been proposed as a treatment for human unilateral vocal fold paralysis (UVFP). This study tested the assumption that results from reinnervation could be improved if combined with medialization surgery. Six canine subjects underwent recurrent laryngeal nerve section and reinnervation with a branch of the ansa cervicalis. After reinnervation, vocal function was assessed before and after arytenoid adduction. Although laryngeal function improved significantly following reinnervation, results were significantly enhanced by the addition of mediali

Paralysis in the head and neck can affect any motor or mixed cranial nerves and the cervical roots. Most conspicuous deficits, however, involve the larynx and the face. The capacity for denervated striated muscle to undergo reinnervation, and the presence of remaining sources of information, have allowed coordinated rehabilitation of incapacitated cervical neuromuscular systems. The object of further related research should focus on the long-term efficacy of the reinnervated muscle machinery and the potential complexities of electronic integration.

We investigated the effect of variation in superior laryngeal nerve stimulation (SLNS) on vocal fold vibration. Photoglottography (PGG), electroglottography (EGG), and subglottic pressure (Psub) were measured in seven mongrel dogs using an in vivo canine model of phonation. The PGG, EGG, and Psub signals were examined at three SLNS frequencies (100 Hz, 130 Hz, and 160 Hz) using a constant rate of air flow. Increasing SLNS, which causes a contraction of the cricothyroid muscle, produced a marked increase in Fo, little change in Psub, an increase in the open quotient, and a decrease in the close

Functional rehabilitation of the larynx after unilateral vocal cord paralysis was attempted in the dog by selective reinnervation of the laryngeal muscles. The intralaryngeal branches of the right recurrent nerve were dissected. The adductor branch was anastomosed with the ansa cervicalis; the abductor branch was anastomosed with the trunk of the phrenic nerve either within the larynx or through the recurrent nerve, the adductor branch of which was sectioned. Results could be analyzed in seven dogs: mobility of the vocal cord was checked, and electromyography, stimulation of the nerves, and hi

Left laryngeal hemiplegia was induced by resection of the left recurrent laryngeal nerve in 12 dogs. A neuromuscular pedicle graft formed from the first cervical nerve and sternothyroideus muscle was transplanted after 1 week to the denervated cricoarytenoideus dorsalis muscle in 8 dogs. The remaining 4 dogs served as controls. Left arytenoid abduction was blindly evaluated by laryngoscopy with video photography at time 0, at 1 week, and at 19 weeks in all dogs. At 19 weeks, biopsy specimens of the left cricoarytenoideus dorsalis muscle and the neuromuscular pedicle were taken from 4 of the tr

Microinjections of botulinum toxin have recently been shown to be effective in the treatment of strabismus, and it has also been suggested that microinjections of this myoneural blocking agent might be of value in the treatment of spastic dysphonia. The success of such a microinjection technique would rely on a precise knowledge of the distribution of myoneural junctions in the thyroarytenoid muscle. In view of this potential application as well as the need for such information in reinnervation procedures, we have used computer graphics to reconstruct the three-dimensional distribution of moto

Horseradish peroxidase (HRP) is used to trace axonal connections from the motor end-plate to the driving neuron. This technique has confirmed that the neurons activating the sternothyroid muscle are located in the cervical spinal cord, while those controlling the posterior cricoarytenoid (PCA) are found in the nucleus ambiguus ipsilaterally. Eight rabbits underwent a sternothyroid ansa pedicle implantation to the PCA at the time of sectioning the recurrent laryngeal nerve ipsilaterally. After two months, four of these animals received HRP injections into the previously implanted PCA. Brainstem

This study of reinnervation of the larynx results from the research for a functional method of treatment for laryngeal paralysis of peripheral origin. The physiological reality of reinnervation of a denervated muscle by a nerve other than that anatomically destined for it, or "neurotisation", is now an established fact. This experimental study in 12 dogs consisted of neurotisation of a posterior crico-thyroid muscle, denervated by section of the recurrent laryngeal nerve, by implantation of a graft of sub-hyoid muscle with a nerve pedicle. Only one positive result at six months was seen. 8 fai
