The single antigen expressing lines (SALs) concept: an excellent tool for screening for HLA-specific antibodies.

Definition of the antibody specificity in the serum of patients waiting for a renal transplant or in need for platelet transfusion is a crucial step for finding adequate donors. Confounding factors are the complexity of the serum antibodies and the expression of several, up to six, different human leukocyte antigens (HLA) on peripheral blood lymphocytes used as target cells in the antibody screening. Single antigen-expressing (SAL) cell lines were generated by transfecting human major histocompatibility complex (MHC) class I sequences into K562, an erythroleukemia-derived cell line lacking MHC

Blood donor safety and selection criteria for individuals self-identifying as trans or non-binary.

Social movements in favour of greater diversity, equity and inclusion have led societies to re-examine the concepts of sex assigned at birth and gender identity. These have also impacted healthcare systems and blood operators. Gender identities extending beyond the concept of cisgender binary individuals might have an impact on the safety of blood components for recipients, and on the safety of the donation process itself. Furthermore, the procedures put in place by blood operators to welcome donors from sexual and gender diversity might have a major impact on donors' psychological well-being

Colour oscillations in arterioarterial anastomoses reflect natural differences in donor and recipient oxygenation and hematocrit.

Our aim was to show that the colour difference between brighter and darker red, occasionally observed as an oscillating boundary in the recipient and donor parts of an arterioarterial anastomosis in severe twin-twin transfusion syndrome (TTTS), is a consequence of natural differences in blood oxygenation and hematocrit developing between donor and recipient twins. As method we defined a theoretical model of the placenta with dimensions from pathology examination. From literature we determined the optical absorption and scattering properties of all tissue components, and hematocrit and oxygen s

Evaluation of two methods for counting residual leukocytes in leukoreduced packed red cells: Flow cytometry and fully automated cell counter.

BACKGROUND AND OBJECTIVES: Objective of the study is to explore the possibility of utilization of seven part fully automated hematology analyzer for enumeration of residual leukocytes (residual white blood cells [rWBCs]) in leukoreduced packed red cells (LR-PRCs) prepared from whole blood at a blood center as an alternate to the gold standard method, flow cytometry. In this study, we evaluate the performance characteristic of hematology analyzer against flow cytometry for the estimation of rWBCs in 39 LR-PRC units. MATERIALS AND METHODS: PRCs prepared from whole blood donations by 39 donors w

Sex, risk, and education in donor educational materials: review and critique.

Food and Drug Administration guidelines prohibit men who have sex with men (MSM) from donating blood to prevent the spread of the human immunodeficiency virus (HIV/AIDS). Although the deferral criteria leave "sex" undefined, donor educational materials distributed before the health questionnaire often offer a definition. This study analyzes educational materials for their contribution to the donation process and construction of HIV/AIDS. It applies a discourse analysis approach to a sample (n = 52) of such materials obtained in summer 2009 from blood collection organizations listed in the AABB

Validation of an automated immunoglobulin G-only cytomegalovirus (CMV) antibody screening assay and an assessment of the risk of transfusion transmitted CMV from seronegative blood.

BACKGROUND: Cytomegalovirus (CMV) antibody donor screening assays have predominantly included both immunoglobulin G (IgG) and immunoglobulin M (IgM) detection. However, since in the majority of cases both CMV IgG and IgM are detected concomitantly during early seroconversion, CMV assays based only on IgG are now widely applied for donor screening. STUDY DESIGN AND METHODS: The performance of an automated microparticle CMV IgG assay (Abbott AxSYM CMV IgG microparticle enzyme immunoassay [MEIA]) was compared with an established total antibody blood screening assay (Abbott CMV Total AB EIA). Hepatitis C virus transmission, 1988-1991, via blood components from donors subsequently found to be anti-HCV-positive.

The recipients of blood components, from the first 12 anti-hepatitis C virus (HCV) positive donors identified by blood donor screening, 1985-1991, were traced retrospectively and tested to assess the HCV transmission rate, HCV genotypes and disease severity. Three enzyme-linked immunosorbent assay (ELISA) positive but RIBA-indeterminate and HCV RNA-negative donors did not transmit HCV to their 9 traced recipients. Nine RIBA- and HCV RNA-positive donors had donated blood to 27 now living recipients of whom 16/27 (59%) were viraemic 1-5 years later. Nine recipients had resolved infection, as det

The Golden Hour and beyond: A neuroprotection bundle to reduce intraventricular hemorrhage.

Intraventricular hemorrhage (IVH) remains one of the most consequential complications facing extremely preterm infants, with enduring effects on survival and neurodevelopment. The first 60 min of life (the Golden Hour) offers a uniquely modifiable window to stabilize physiology, smooth cardio-cerebral transition, and implement neuroprotective strategies that may prevent or decrease the risk of IVH. This review synthesizes randomized trials, observational cohorts, and multicenter quality-improvement (QI) initiatives to define a pragmatic, evidence-based approach to Golden Hour care, and extends

Flat or fat? Inferior vena cava ratio is a marker for occult shock in trauma patients.

BACKGROUND: Identification of occult shock (OS) or hypoperfusion is critical in the initial management of trauma patients. Analysis of inferior vena cava (IVC) ratio on computed tomography (CT) scan has shown promise in predicting intravascular volume. We hypothesized that a flat IVC is a predictor of OS and associated with worse outcomes in major trauma patients. MATERIALS AND METHODS: We performed a 1-y retrospective analysis of our level 1 trauma center database to identify all major trauma activations that underwent evaluation with a CT scan of the torso, arterial blood gas, and serum lac

Correlates of spontaneous clearance of hepatitis C virus among people with hemophilia.

People with hemophilia were formerly at very high risk of infection with hepatitis C virus (HCV). Approximately 20% of HCV-infected patients spontaneously clear the virus. To identify correlates of spontaneous clearance of HCV, we studied a cohort of HCV-infected hemophilic subjects without human immunodeficiency virus infection who had never been treated with interferon. Plasma HCV RNA was persistently undetectable in 192 (27.0%) of 712 HCV-seropositive subjects. In multivariate analyses, HCV clearance was more likely in subjects infected with HCV at younger age, especially with infection bef

Time Trend and Prevalence Analysis of Transfusion Transmitted Infections among Blood Donors: A Retrospective Study from 2001 to 2016.

CONTEXT: Transfusion-transmitted infections (TTIs) remain a major threat to the patients receiving blood. The incidence of the transmission of various infectious agents has reduced ever since the introduction of various molecular techniques for detection. AIMS: The study tried to figure out accurate estimates of risk and trend of TTI over a period of sixteen years, essential for monitoring the safety of blood supply and evaluating the efficacy of the currently employed screening procedures. METHODS AND MATERIAL: A total of 57942 blood donors' records were analyzed for the period from January

Motivations for blood donation by HIV-positive individuals on antiretrovirals in South Africa: A qualitative study.

OBJECTIVES: We performed a mixed-methods study to explore the motivations associated with blood donation by donors with known, but undisclosed HIV-positive status and ARV use (HIV+/ARV+), seeking potential strategies to reduce such donations and mitigate risk for blood recipients. Here, we report predominantly the qualitative component. BACKGROUND: A safe and sustainable blood supply is dependent in part, on effective pre-donation donor assessment. We previously described failure by HIV+/ARV+ blood donors to disclose their status. Such donations may lead to transfusion-transmitted HIV. METHO

Excess Conventional Ultrafiltration Volume Increases Risk for Postoperative Cardiac Surgery-Associated Acute Kidney Injury.

INTRODUCTION: Conventional ultrafiltration (CUF) is employed during cardiopulmonary bypass to remove excess fluid and concentrate specific blood components. The study aimed to investigate the impact of CUF volume on postoperative cardiac surgery-associated acute kidney injury (CSA-AKI). METHODS: This single-center, retrospective study included adult patients undergoing on-pump cardiac surgery between 2018 and 2023. Patients were classified into four categories for comparative analysis: the none-CUF group and three groups according to tertiles of the weight-adjusted CUF volume (tertile 1, 0.1-

The Use of Whole Blood Transfusion During Non-Traumatic Resuscitation.

BACKGROUND: Evidence from military populations showed that resuscitation using whole blood (WB), as opposed to component therapies, may provide additional survival benefits to traumatically injured patients. However, there is a paucity of data available for the use of WB in uninjured patients requiring transfusion. We sought to describe the use of WB in non-trauma patients at Brooke Army Medical Center (BAMC). MATERIALS AND METHODS: Between January and December 2019, the BAMC ClinComp electronic medical record system was reviewed for all patients admitted to the hospital who received at least

Time-deferred consequences: A deep dive into delayed adverse reactions among whole blood donors.

BACKGROUND: The Blood Donor Health is an important component in blood transfusion services. The retention of blood donors is important, if not less than the recruitment of new blood donors. Despite the recognized importance of donor health, several challenges impede the retention of donors. These include the donor experience in the form of adverse reactions ranging from mild to severe which can be immediate or delayed. Such experiences can deter blood donors from returning to donor pool. Our study focuses on the incidence, types, and impacts of delayed adverse reactions (DARs) among whole bloo

Guideline for the management of hip fractures 2020: Guideline by the Association of Anaesthetists.

We convened a multidisciplinary Working Party on behalf of the Association of Anaesthetists to update the 2011 guidance on the peri-operative management of people with hip fracture. Importantly, these guidelines describe the core aims and principles of peri-operative management, recommending greater standardisation of anaesthetic practice as a component of multidisciplinary care. Although much of the 2011 guidance remains applicable to contemporary practice, new evidence and consensus inform the additional recommendations made in this document. Specific changes to the 2011 guidance relate to a

ACTA2 mutation and postpartum hemorrhage: a case report.

BACKGROUND: ACTA2 encodes smooth muscle specific α-actin, a critical component or the contractile complex of vascular smooth muscle. Mutations in ACTA2 are the most common genetic cause of thoracic aortic aneurysm, and are also the cause of other disorders, including Moyamoya disease, coronary artery disease and stroke as well as Multisystemic Smooth Muscle Dysfunction Syndrome. We note that ACTA2 is also expressed in uterine smooth muscle, and this raises the possibility that women harboring ACTA2 mutations might exhibit uterine smooth muscle dysfunction. In vitro generated Rh(null) red cells recapitulate the in vivo deficiency: a model for rare blood group phenotypes and erythroid membrane disorders.

Lentiviral modification combined with ex vivo erythroid differentiation was used to stably inhibit RhAG expression, a critical component of the Rh(rhesus) membrane complex defective in the Rh(null) syndrome. The cultured red cells generated recapitulate the major alterations of native Rh(null) cells regarding antigen expression, membrane deformability, and gas transport function, providing the proof of principle for their use as model of Rh(null) syndrome and to investigate Rh complex biogenesis in human primary erythroid cells. Using this model, we were able to reveal for the first time that
