Capsular distention for adhesive capsulitis in a pediatric patient with acute flaccid myelitis: A case report.

INTRODUCTION: Acute Flaccid Myelitis (AFM) is a recently recognized pediatric condition characterized by flaccid paralysis that can cause lifelong deficits. Adhesive Capsulitis (AC) is a chronic condition that causes loss of passive range of motion in the shoulder due to joint contracture. CASE REPORT: This is a case report of a 15-year-old male who suffered from shoulder pain with progressive loss of passive range of motion after AFM, diagnosed with AC based on exam and radiology. After failing conservative management, the patient was successfully treated with ultrasound guided corticosteroi

Association between fasting glucose levels and adhesive capsulitis in a normoglycemic population: a case-control study.

BACKGROUND: Hyperglycemia is the most commonly cited risk factor for adhesive capsulitis. However, no study has established whether fasting glucose levels within the normoglycemic range are associated with idiopathic adhesive capsulitis (IAC). This study hypothesized that increments of fasting glucose levels within the normoglycemic range would be linked to IAC. This study investigated any association between normoglycemic fasting glucose levels and IAC. METHODS: This case-control study comprised a group of 151 patients with IAC without intrinsic shoulder lesions, extrinsic causes, or known m

Platelet-rich plasma injection in a patient with adhesive capsulitis due to chronic kidney disease.

Adhesive capsulitis is a common problem in patients with chronic kidney disease. Patients suffer from joint stiffness and painful joint movement.Conservative treatments consist non-steroid antiinflamatory drugs, intraarticular injections and physical therapy. Newer approches such as platelet-rich plasma injections (PRP) also can be applied but there is little evidence for the effectiveness of PRP in patients with adhesive capsulitis. A 70 year-old woman, receives dialysis treatment admitted to our out-patient clinic with stiffness and pain in her right shoulder. Her diagnosis was confirmed wit

Risk of Herpes Zoster in Patients with Adhesive Capsulitis of the Shoulder.

BACKGROUND: Physical diseases, such as infection, and emotional distress are associated with herpes zoster (HZ). However, the relationship between adhesive capsulitis of the shoulder (ACoS) and HZ remains unknown. OBJECTIVE: This study investigated the risk of HZ development in patients with ACoS. METHODS: We analyzed the Longitudinal Health Insurance Database, a subset of 1 million beneficiaries from the National Health Insurance Research Database. Patients newly diagnosed with ACoS during the 2000-2012 period were the case group. Each patient with ACoS was matched to a control, according t

Prevalence of rheumatic conditions in patients with diabetes mellitus in a tertiary care hospital.

Musculoskeletal disorders are common in diabetic subjects. The pathophysiology of these disorders in diabetic patients is not obvious. It could be due to connective tissue disorders, glycosylated end products, vasculopathy, neuropathy or combinations. A wide range of musculoskeletal syndromes have been described in association with diabetes, namely diabetic cheiro-arthropathy, adhesive capsulitis of shoulder, carpal tunnel syndrome, Dupuytren's contracture, hyperostosis, osteo-arthritis, hyperuricaemia, etc. This study was undertaken to find out the prevalence of these conditions in diabetes m

Risk of avulsion of the Achilles tendon after partial excision for treatment of insertional tendonitis and Haglund's deformity: a biomechanical study.

Surgical treatment of posterior heel pain caused by insertional (calcific) Achilles tendonitis or retrocalcaneal bursitis includes resection of diseased tendon or exostectomy. Currently, no guidelines exist to determine how much tendon may be excised without risking rupture of the Achilles tendon. Anatomic dissections revealed the average height of the insertion measured 19.8 mm (range, 13-25 mm). Average width at the proximal aspect of the insertion measured 23.8 mm (range, 17-30 mm) and distally measured 31.2 (range, 25-38 mm). To assess the risk of avulsion, the tendon insertion was partial…

Glenohumeral gliding manipulation following interscalene brachial plexus block in patients with adhesive capsulitis.

Previous studies describing nonconservative treatment of shoulder adhesive capsulitis include distention arthrography, brisement techniques, arthrotomy of the anterior/inferior axillary fold and subscapularis tendon, and manipulation under general anesthesia. The purpose of this study was to develop and describe an alternative treatment method that utilizes glide manipulation under interscalene brachial plexus block. Eight patients (four females and four males), age 31-55 years, with a mean age of 44 years, were treated conservatively for adhesive capsulitis of the shoulder and failed to produ

Pedicled radial forearm flaps for recalcitrant defects about the elbow.

Chronic, open wounds about the elbow frequently require flap coverage. Skin grafts, local transposition flaps, muscle transposition flaps, and two-staged pedicled flaps from the chest and abdomen as well as free tissue transfers have all been reported. Recalcitrant elbow defects in patients in whom these previous flaps have failed are especially challenging to the upper extremity reconstructive surgeon. We have successfully employed pedicled radial forearm flaps in three patients in whom one or more previous flaps have failed.

A double-blind comparison of naproxen gel and placebo in the treatment of soft tissue injuries.

A double-blind study was carried out in 120 patients who had received soft tissue injuries within the preceding 48 hours to compare the effectiveness of naproxen gel (10%) with placebo gel (base alone). The injuries were predominantly synovitis and tendinitis. Standard clinical evaluations of the patients' condition were made by physicians and patients on entry and after 3 and 7 days of treatment. Both treatments resulted in a significant improvement in symptoms, but naproxen gel was significantly superior to placebo gel (p less than 0.05). The response produced by naproxen was more rapid; all

[Drug treatment of soft tissue rheumatoid complaints. Oral administration of a non-steroidal antirheumatic drug with a short half-life].

In a clinical controlled study involving more than 100 patients with nonarticular rheumatism, the nonsteroidal anti-inflammatory agent Ibuprofen Klinge 600 proved clinically effective. The test parameters were subjective pain considered separately as pain at rest, pain on movement, and tenderness, and the time to appearance of fatigue. All three categories of pain were ameliorated by the treatment, the time interval to appearance of fatigue doubling or tripling during the course of 3 weeks of treatment. This therapeutic success was coupled with good toleration of nonsteroidal anti-inflammatory

Pain in the foot. 2. Causes of pain in the hindfoot, midfoot, and forefoot.

Pain in the foot has many causes. A careful history and physical examination, radiographs, and laboratory data usually yield the correct diagnosis so that specific treatment can be directed toward the problem to help relieve the patient's pain. Rarely is surgery necessary. Good conservative management usually produces a satisfactory clinical response.

Editorial Commentary: Platelet-Rich Plasma Shows Promise for Improving Shoulder Tendinopathy.

Platelet-rich plasma (PRP) injections continue to be used at increasing rates to treat common musculoskeletal conditions. PRP has a low-risk profile and emerging in vitro evidence to support its positive effects on soft-tissue healing. PRP has been shown to be of benefit for knee osteoarthritis, but less has been published regarding the shoulder. PRP delivers a high concentration of growth factors, cytokines, and other important inflammatory modulators. Its use is appealing for treating partial-thickness rotator cuff tears, subacromial bursitis, and rotator cuff tendinopathy since rotator cuff…
