Dorsal and Ventral Parts of Thalamic Nucleus Submedius Project to Different Areas of Rat Orbitofrontal Cortex: A Single Neuron-Tracing Study Using Virus Vectors.

The rodent orbitofrontal cortex is involved in a variety of cognitive and behavioral functions that require thalamic input to be successfully expressed. Although the thalamic nucleus submedius (Sm) is a major source of afferents to the orbitofrontal cortex, thalamocortical projection from the Sm has not been fully elucidated. In the present study, we first divided the rat Sm into dorsal and ventral parts according to the distribution of vesicular glutamate transporter 2-immunoreactive varicosities, which were somatosensory afferents from the brain stem. Subsequently we investigated dendritic a

Attitudes towards medication non-adherence in elderly kidney transplant patients: a Q methodology study.

BACKGROUND: Non-adherence to the post-transplant regime is a common problem in kidney transplant patients and may lead to rejection or even graft failure. This study investigated attitudes towards the post-transplant regime of immunosuppressive medication among the ever growing population of elderly kidney recipients. METHODS: Q methodology was used to explore attitude profiles. Participants (> 65 years) were asked to rank-order opinion statements on issues associated with (non-)adherence. The rankings were subject to by-person factor analysis, and the resulting factors were interpreted and d

Sleep-related breathing and sleep-wake disturbances in ischemic stroke.

BACKGROUND: Sleep-related breathing disturbances (SDB) and sleep-wake disturbances (SWD) are often neglected in stroke patients. Recent studies suggest that they are frequent and have an impact on stroke outcome. METHODS: We review current knowledge about frequency, clinical presentation, and consequences of poststroke SDB and SWD, and discuss treatment options. RESULTS: SDB, presenting with obstructive, central, or mixed apneas, is present in 50%-70% of stroke patients. We recommend screening for SDB in all stroke patients by respirography. Continuous positive airway pressure (CPAP) is the

The somatic marker hypothesis: a critical evaluation.

The somatic marker hypothesis (SMH; [Damasio, A. R., Tranel, D., Damasio, H., 1991. Somatic markers and the guidance of behaviour: theory and preliminary testing. In Levin, H.S., Eisenberg, H.M., Benton, A.L. (Eds.), Frontal Lobe Function and Dysfunction. Oxford University Press, New York, pp. 217-229]) proposes that emotion-based biasing signals arising from the body are integrated in higher brain regions, in particular the ventromedial prefrontal cortex (VMPFC), to regulate decision-making in situations of complexity. Evidence for the SMH is largely based on performance on the Iowa Gambling

Neuropsychological follow-up of twenty two adult patients after surgery for craniopharyngioma.

The optimal management of craniopharyngioma today remains an unconcluded debate. This prospective study examines the cognitive status and quality of life of 22 patients one year after surgery for craniopharyngioma. Pterional approach (5 left, 15 right) was used in 20 patients (90%), whereas two were operated on using a rhinoseptal approach. Neuropsychological examination was performed twelve months after the operation. For twelve patients, neuropsychological examination was performed before operation. The preoperative evaluation revealed that six patients had impaired neuropsychological testin

Osteoporosis in the Oldest Old - From Diagnosis to Treatment: A Narrative Review.

BACKGROUND: Osteoporosis and fragility fractures are major causes of disability, loss of independence, institutionalization, and mortality in the oldest old. Despite their exceptionally high fracture risk, adults aged ≥80 years remain underrepresented in randomized controlled trials. Diagnosis and treatment are complicated by the interaction of skeletal fragility with falls, sarcopenia, frailty, multimorbidity, cognitive and functional impairment, and competing mortality. SUMMARY: This narrative review discusses the diagnosis, fracture-risk assessment, non-pharmacological management, and phar

Executive (dys)functions and ODD and CD: A systematic review and meta-analysis.

OBJECTIVE: Precursors of antisocial behavior in children may be associated with neuropsychological deficits, namely in executive functions (EF). The main objective of this systematic review and meta-analysis was to synthesize evidence of the relation between EF performance and early manifestations of antisocial behavior in children and adolescents. METHOD: The procedures of this systematic review and meta-analysis were based on the PRISMA recommendations. The review was based on a systematic search in the EBSCOhost and Pubmed databases. Fifty studies were included in this review, 37 of which

Hypertension and Frailty Syndrome in Old Age: Current Perspectives.

Hypertension is both a health problem and a financial one globally. It affects nearly 30 % of the general population. Elderly people, aged ≥65 years, are a special group of hypertensive patients. In this group, the overall prevalence of the disease reaches 60 %, rising to 70 % in those aged ≥80 years. In the elderly population, isolated systolic hypertension is quite common. High systolic blood pressure is associated with an increased risk of cardiovascular disease, cerebrovascular disease, peripheral artery disease, cognitive impairment and kidney disease. Considering the physiological change

Beyond age and comorbidities: Frailty and cognitive impairment are associated with early medical complications after surgery for lumbar spinal stenosis in patients aged 75 years and older.

BACKGROUND: Risk assessment before surgery for lumbar spinal stenosis in the very elderly relies largely on age and comorbidity scores. We asked: (1) is frailty (modified 5-item Frailty Index, mFI-5) associated with 30-day medical complications beyond age and comorbidity burden? (2) which geriatric parameters are independently associated? (3) is frailty associated with surgical complications? HYPOTHESIS: Frailty and specific geriatric impairments are more strongly associated with early medical complications than age or comorbidity indices. PATIENTS AND METHODS: Retrospective single-center ob

Transjugular Intrahepatic Portosystemic Shunt Outcomes in the Elderly Population: A Systematic Review and Meta-Analysis.

BACKGROUND: Transjugular intrahepatic portosystemic shunt (TIPS) is a procedure typically utilized to treat refractory ascites and variceal bleeding. However, TIPS can lead to significant complications, most commonly hepatic encephalopathy (HE). Advanced age has been described as a risk factor for HE, as the elderly population tends to have decreased cognitive reserve and increased sarcopenia. We conducted a systematic review and meta-analysis of the available literature to summarize the association between advanced age and risk of adverse events after undergoing TIPS. METHODS: A comprehensiv…

Development and external validation of an explainable machine learning model for identifying depressive symptoms in middle-aged and older adults with low haemoglobin: a prediction-model study.

BACKGROUND: Early screening for depressive symptoms in middle-aged and older adults is important for timely risk identification. Low haemoglobin may identify a clinically accessible vulnerable subgroup in which chronic disease burden, fatigue, functional impairment, and reduced physiological reserve often coexist. However, it should be understood as a high-risk phenotype rather than a direct measure of nutritional deficiency or frailty. Evidence in this population remains limited. METHODS: This study developed a model using data from the 2015 China Health and Retirement Longitudinal Study (CH

Blood pressure variability and frailty: mechanisms, evidence, and clinical implications.

Blood pressure variability (BPV), fluctuations in blood pressure across beat-to-beat, 24-h, day-to-day, and visit-to-visit timescales, has emerged as a risk marker that is independent of mean blood pressure. Frailty, a multidimensional syndrome of diminished physiological reserve, shares core biology with BPV, including vascular aging, impaired baroreflex function, autonomic dysregulation, and chronic low-grade inflammation. This narrative review synthesizes mechanistic, epidemiologic, and clinical evidence linking BPV and frailty. Mechanistically, arterial stiffness and endothelial dysfunctio

Prehabilitation for patients undergoing radical cystectomy: Patient selection and targeted interventions.

Radical cystectomy (RC) is associated with high perioperative morbidity, reflecting common preoperative vulnerability from frailty, sarcopenia, malnutrition, systemic inflammation, deconditioning (including during neoadjuvant chemotherapy), and psychological distress. Although prehabilitation is safe and feasible and has demonstrated improvements in functional capacity, quality of life, and select postoperative outcomes, real-world uptake remains low, in part due to uncertainty regarding appropriate patient selection and how to operationalize evidence-based interventions. We aimed to provide a

Assessing hygiene and zoonotic risk perception in the Danube Delta: insights from the pre- and early-COVID-19 periods.

Zoonoses, or diseases that are naturally transmitted between animals and humans, are a global public health problem, accounting for over 60% of all infectious and parasitic diseases affecting humans. Therefore, raising awareness of the risks within communities is an activity that must be constant. The issue of zoonoses is of particular importance in regions characterized by high biodiversity, such as the Danube Delta. In this complex ecosystem, the frequency of interactions between the human population, native wildlife, and domestic animals creates favorable conditions for the transmission and

A comprehensive assessment of resting state networks: bidirectional modification of functional integrity in cerebro-cerebellar networks in dementia.

In resting state fMRI (rs-fMRI), only functional connectivity (FC) reductions in the default mode network (DMN) are normally reported as a biomarker for Alzheimer's disease (AD). In this investigation we have developed a comprehensive strategy to characterize the FC changes occurring in multiple networks and applied it in a pilot study of subjects with AD and Mild Cognitive Impairment (MCI), compared to healthy controls (HC). Resting state networks (RSNs) were studied in 14 AD (70 ± 6 years), 12 MCI (74 ± 6 years), and 16 HC (69 ± 5 years). RSN alterations were present in almost all the 15 rec
