The relation between sexual practices and location of anogenital warts is discussed. Results were derived from clinical examination and interview of 56 consecutive male patients referred to CO2-laser treatment of therapy-resistant genital warts, at a clinic at the Department of Dermatovenereology, Gentofte Hospital, located in the northern area of Copenhagen. Seventy-eight patients with condyloma acuminatum (51 with a lesion in the vulva, 11 in the uterine cervix, 1 in the vagina, 15 in multiple sites) have been treated by using three types of laser beam. Intramuscular injections of beta-interferon were used to treat a first group of 64 outpatients with clinically diagnosed genital condylomata at single doses of 3 x 10(6) IU for 10 consecutive days. Four hundred and fifty patients were enrolled into a randomised clinical trial in a public sexually transmitted diseases clinic to evaluate the efficacy of podophyllin, cryotherapy, and electrodesiccation for treatment of external genital warts. Recent studies of genital wart therapy have evaluated laser surgery, interferon, and 5-fluorouracil. On the basis of considerations of efficacy, cost, adverse effects, and recurrence rates, cryotherapy with liquid nitrogen remains the recommended therapy for most sexually transmitted warts. None of the treatment methods eradicates the papillomavirus, and genital wart recurrences are seen with each method. The influence of treatment of genital warts on the natural course and sequelae of genital papillomavirus infection is unknown.

A single blind crossover study compared the traditional Ayre spatula with the new Aylesbury spatula. The study group consisted of all women attending the department of genitourinary medicine during a 16 month period. The same incidence (3.6%) of dyskaryotic smears was found using the Ayre spatula (74/2077) as the Aylesbury spatula (72/2003). A 29-year-old man noted a wart around the external urethral orifice. The lesion was diagnosed as condyloma acuminata and he had the tumor excised and circumcision on September 10, 1985. He complained of urethral bleeding on December 21, 1985. Physical and endoscopic examination revealed papillo-granular condyloma acuminata at the distal urethra. He received transurethral fulguration on January 17, 1986 and intraurethral instillation of 5-fluorouracil cream weekly. The lesions were successfully treated.

An infrared contact coagulator was used for the treatment of genital condyloma acuminata in 89 women. After direct application to the condyloma, the device produces a precisely adjustable tissue necrosis without tissue adhesion. Of the 89 women, 74 returned for scheduled follow-up visits after one, three, and six months. Giant condyloma acuminatum (Buschke-Loewenstein tumor) is an unusual variety of venereal wart characterized by a large size, cauliflower-like appearance, and locally invasive nature. A new CO2 laser system, of special characteristics, has been found to be effective for CO2 laser dermatological surgery. The unit is compact, freely movable, and suitable for office practice. A highly flexible operating probe makes for precision surgery. The initial investigative clinical series included resistant warts in many areas including extensive plantar warts, oral lesions, and condylomata. There were also excisions of broad base granuloma pyogenicum, large seborrheic warty lesions, thermal photocoagulation of rhinophyma, and laser dermabrasion of tattoos. Flaps were done in animals.
