The role of screening, brief intervention, and referral to treatment in the perinatal period.

Substance use during pregnancy is at least as common as many of the medical conditions screened for and managed during pregnancy. While harmful and costly, it is often ignored or managed poorly. Screening, brief intervention, and referral to treatment is an evidence-based approach to manage substance use. In September 2012, the US Centers for Disease Control and Prevention convened an Expert Meeting on Perinatal Illicit Drug Abuse to help address key issues around drug use in pregnancy in the United States. This article reflects the formal conclusions of the expert panel that discussed the use

[Prostatic inflammation-induced chronic pelvic pain: Roles of substance P and c-fos in the spinal cord].

OBJECTIVE: To explore the possible pain mechanism of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). METHODS: The models of CP/CPPS were established in male Wistar rats by the autoimmune method. The paw withdrawal threshold (PWT) was detected using Von Frey filament. The expressions of the substance P and c-fos in the prostate and spinal L5-S2 segments were determined by immunohistochemistry followed by analysis of their correlation with CP/CPPS. RESULTS: Compared with the control rats, the CP/CPPS models showed significantly decreased PWT (P < 0.05), remarkable prostatic inflamm

Aripiprazole, alcohol and substance abuse: a review.

Aripiprazole is an atypical antipsychotic used for schizophrenia, manic and mixed episodes associated with bipolar I disorder and as adjunctive therapy for major depressive disorder. It functions as a partial agonist at dopamine D2 and 5-HT1A receptors, and as an antagonist at the 5-HT2A receptor. The most recent results obtained from scientific research showed that dopaminergic mechanisms are involved in motivation, reward, and reinforcement of substance abuse. The use of aripiprazole and partial dopamine agonists could represent a novel strategy for normalizing dopamine neurotransmission. Ma

An imbalance between substance P and vasoactive intestinal polypeptide might contribute to the immunopathology of megaesophagus after Trypanosoma cruzi infection.

Megaesophagus is one of the major causes of morbidity in chronic Chagas disease, and extensive denervation, associated with an inflammatory process, is recognized as the key factor for alterations in motility and disease development. Here, we analyzed esophagus samples from necropsied, infected individuals--6 cases with megaesophagus and 6 cases without megaesophagus--for the relative areas of expression of 2 neuromediators, substance P and vasoactive intestinal peptide, which are known to activate or inhibit, respectively, local immune cells. Samples from 6 noninfected individuals were used a

Erectile dysfunction and subsequent substance abuse: a retrospective cohort study of US non-academic hospitals.

BACKGROUND: Erectile dysfunction (ED) affects millions and is associated with cardiovascular disease, diabetes, and depression-conditions that share risk factors with substance use disorders. AIM: This study investigates the temporal relationship between ED diagnosis and subsequent substance abuse/dependence diagnoses across different age groups. METHODS: This retrospective cohort study used the TriNetX database to identify adult males from US hospitals with ED diagnoses (ICD-10: N52) with at least 3 years of follow-up. Controls were propensity-matched 1:1 on age and race. Patients were stra

Target-controlled dialysis for antibiotics (TCD-ABx).

BACKGROUND: Effective antimicrobial therapy is an essential part of intensive care medicine and renal replacement therapy is an important and common intervention which significantly affects the pharmacokinetics of many antimicrobials. This is especially critical for substances with a narrow therapeutic range, creating a dilemma of weighing the risk of toxicity from increased drug exposure against risk of ineffective treatment and promotion of antimicrobial resistance. To address this problem, we investigate a target-controlled dialysis by in vitro experiments - a novel technique in which drug

Public health and international drug policy.

UNLABELLED: In September 2015, the member states of the United Nations endorsed sustainable development goals (SDG) for 2030 that aspire to human rights-centered approaches to ensuring the health and well-being of all people. The SDGs embody both the UN Charter values of rights and justice for all and the responsibility of states to rely on the best scientific evidence as they seek to better humankind. In April 2016, these same states will consider control of illicit drugs, an area of social policy that has been fraught with controversy, seen as inconsistent with human rights norms, and for wh

Structural stressors, neurocognition in reward-related decision making and substance use risk in Puerto Rican early adolescents at two sites: study design.

BACKGROUND: Structural stressors (SS) or mutually reinforcing systems that prevent groups of individuals from accessing opportunities and resources, can impact one's health. Indicators of SS include area-level group differences in income, education and exposure to violence. SS are fundamental causes of health disparities and can be indexed by area-level measures. Area-level SS may influence substance use risk in adolescents and future substance use disorders (SUDs). SUDs often start during adolescence, a critical risk period and an ideal time for prevention and intervention efforts. Since SUD

Trends of repeated emergency department visits among adolescents and young adults for substance use: A repeated cross-sectional study.

Emergency Department (ED) visits for substance-related concerns among young people have been increasing in recent years. Understanding the factors related to repeated ED visits (two or more ED visits per year) for substance use concerns among young people is critical to developing a more efficient mental healthcare system that does not overburden ED and that provides efficient care for substance use patients. This study examined trends of substance use-related ED visits and factors related to repeated ED visits (two or more ED visits per year, in comparison to one ED visit per year) among adol

Transcriptomic signatures in tetrapartite brain region identifies shared and unique gene signatures for substance-use.

INTRODUCTION: Chronic substance use is a neuropsychiatric disorder marked by persistent craving, reward seeking, and progression to addiction. The midbrain governs hunger, reward, and pleasure; the DLPFC modulates craving, decision making, and tolerance; the NAc influences feeding, reward, stress, and drug self-administration; and the amygdala regulates emotion and memory. METHODS: To understand these complex and dynamic events in the context of substance use disorders, we profiled transcriptomes from these four regions and integrated clustering, biclustering, WGCNA, and pathway enrichment an

Does parental racial socialization protect Black youth from the impact of racism on problematic substance use over the long term?

OBJECTIVES: Parental racial socialization refers to the messages children receive about race from their parents. Little is known about parental racial socialization's long-term impact on problematic substance use or whether it can protect against adversities commonly experienced by Black Americans. We examined whether parental racial socialization reduces the risk of problematic substance use and buffers the impact of neighborhood disadvantage and racial discrimination on problematic substance use, with attention to the types of messaging. METHOD: Data are from a sample (n = 825) of the Woodl

A Longitudinal Investigation of Blood Neurofilament Light Chain Levels in Chronic Cocaine Users.

The identification of a blood marker of brain pathology that is sensitive to substance-induced neurotoxicity and dynamically responds to longitudinal changes in substance intake would substantially improve clinical monitoring in the field of substance use and addiction. Here, we explored the hypothesis that plasma levels of neurofilament light chain (NfL), a promising marker of neuroaxonal pathology, are elevated in chronic cocaine users and longitudinally associated with changes in cocaine use. Plasma NfL levels were determined using single molecule array (SIMOA) technology at baseline and at

Substance Use among Belgian Higher Education Students before and during the First Wave of the COVID-19 Pandemic.

The COVID-19 pandemic prompted many countries to issue far-reaching policy measures that may have led to increased substance use. Higher education students may have been disproportionally affected due to the rearrangement of educational life and their susceptibility to psychosocial distress and substance use. The current study examined associations between pandemic-related stressors, psychosocial distress, and self-reported alcohol, tobacco, and cannabis use before and during the first wave of the pandemic. Data were collected in Belgium as part of the COVID-19 International Student Well-being

The role of substance use disorders in experiencing a repeat opioid overdose, and substance use treatment patterns among patients with a non-fatal opioid overdose.

BACKGROUND: A non-fatal opioid overdose (NFOO) increases the risk of another overdose and identifies high-risk patients. We estimated the risk of repeat opioid overdose for patients with and without substance use disorder (SUD) diagnoses and the change in substance use treatment utilization rates associated with the first NFOO. METHODS: We selected patients (>18 years of age) from Kaiser Permanente Northern California with a NFOO between 2009-2016 (n = 3,992). Cox proportional hazards models estimated the 1-year risk of opioid overdose associated with SUD diagnoses (opioid, alcohol, cannabis,

Sex and orientation identity matter in the substance use behaviors of sexual minority adolescents in the United States.

BACKGROUND: Health sciences researchers are beginning to understand the differing experiences and health risks among sexual minority subgroups (i.e., those who describe themselves as homosexual/gay/lesbian, bisexual, or unsure/questioning). Such research can promote the allocation of resources to high-risk groups and the development of interventions tailored to their needs. The present study extends this line of research to substance use among adolescents. METHODS: The lifetime and/or past 30-day alcohol, tobacco, cigarette, e-cigarette, marijuana, prescription drug, and illicit drug use of s
