Quantitative imaging capabilities, the large number of radiotracers labeled with short-lived positron-emitting isotopes, and appropriate tracer kinetic models offer a broad range of possibilities for probing different aspects of normal and diseased human myocardium. To some extent, PET has already had an impact on clinical cardiology and can decisively influence patient management. At the same time, PET offers tools for elucidating mechanisms of disease and for monitoring responses to treatment. Previous coronary angiographic data have suggested that the left main length of the coronary artery correlated with dominance (origin of the posterior descending branch) and bicuspid aortic valves. In our autopsy study with direct measurements of left main lengths, 41 patients with tricuspid aortic valves and 13 with bicuspid aortic valves were examined for right, left, or codominance. Regional left ventricular function was studied serially by quantitative two-dimensional echocardiography (2-D echo) in 20 dogs after left anterior descending coronary artery ligation. Normal values for regional myocardial thickening were established in 20 healthy dogs and used as a standard to recognize abnormally contracting segments (ACS). The effect of oral administration of diltiazem, a new 1,5-benzothiazepine derivative, on the frequency of anginal attack at rest was studied in 8 patients. Our own experience showed that diltiazem gave excellent results in patients with ST elevation (variant type) but its effect was insufficient in the patients whose ST segment was depressed during the attack.

Myocardial imaging with 133-Xe and a gamma camera was employed to evaluate total and regional myocardial blood flow. The technique detected vasodilatation after injection of papaverine or diatrizoate. Contrast medium caused transient vasodilatation with return to baseline flow within five minutes.
