Acute effects of intravenous heroin on the hypothalamic-pituitary-adrenal axis response: a controlled trial.

Heroin dependence is associated with a stressful environment and with dysfunction of the hypothalamic-pituitary-adrenal (HPA) axis. The pres … by cortisol levels in serum and saliva before and 20 and 60 minutes after substance administration. Craving, withdrawal, and anxiety levels were measured before and 60 minutes after substance application. Plasma concentrations of heroin and its main metabolites were assessed using high-performance liquid chromatography. Heroin administration reduces cravi …

Alpha-2 adrenergic receptor agonists: a review of current clinical applications.

The α-2 adrenergic receptor agonists have been used for decades to treat common medical conditions such as hypertension; attention-deficit/hyperactivity disorder; various pain and panic disorders; symptoms of opioid, benzodiazepine, and alcohol withdrawal; and cigarette craving. (1) However, in more recent years, these drugs have been used as adjuncts for sedation and to reduce anesthetic requirements. This review will provide an historical perspective of this drug class, an understanding of pharmacological mechanisms, and an insight into current applications in clinical anesthesiology.

Coronary artery dissection in Prader-Willi syndrome: case report and literature review.

Prader-Willi syndrome (PWS) is a syndrome characterized in babies by small birth weight, hypogonadism, flaccid muscle tone, and skeletal abnormalities, and in older children by intense food cravings leading to morbid obesity, hypoxemia, and right heart failure. To our knowledge, PWS has not been associated with coronary artery dissection. We report a 17-year-old woman with PWS who suffered an inferior myocardial infarction secondary to dissection of her right coronary artery.

Methadone maintenance treatment. When and how to refer patients.

Methadone, a long-acting oral narcotic analgesic, prevents withdrawal symptoms and craving over a 24-hour period in patients addicted to narcotics. Methadone maintenance treatment programs have been shown to decrease criminal behavior, improve psychosocial functioning, and decrease injecting narcotic use, thus reducing the spread of AIDS. For primary care physicians, knowing when and how to refer a patient to methadone maintenance treatment, as well as ensuring ongoing liaison with the treatment program, can help improve the quality and continuity of care for addicted patients.

Transdermal nicotine replacement and smoking cessation.

Transdermal nicotine systems are a useful adjunct to smoking cessation. The larger-sized nicotine patches provide steady-state replacement of approximately 50 percent of the average serum nicotine level obtained by smoking one to one and one-half packs of cigarettes a day. This level is sufficient to prevent many symptoms of nicotine withdrawal, including physical craving, irritability, anxiety, restlessness, hostility, hunger and difficulty concentrating. In double-blind, placebo-controlled trials, transdermal nicotine systems have been about twice as successful as placebo in helping patients…

Characterizing alcohol cue reactive and non-reactive individuals with alcohol use disorder.

PURPOSE: Exposure to alcohol-related cues is thought to elicit a conditional response characterized by increased craving in individuals with alcohol use disorder (AUD). In the context of AUD research, it is important to consider that not all individuals with an AUD are alcohol cue reactive. This study systematically examined subjective alcohol cue reactivity and its clinical and drinking correlates in individuals with an AUD enrolled in a human laboratory pharmacotherapy trial. METHODS: Individuals with current moderate-to-severe AUD (N = 52) completed a standard alcohol cue exposure paradigm…

Clinical and Functional Connectivity Outcomes of 5-Hz Repetitive Transcranial Magnetic Stimulation as an Add-on Treatment in Cocaine Use Disorder: A Double-Blind Randomized Controlled Trial.

BACKGROUND: Cocaine use disorder (CUD) is a global condition lacking effective treatment. Repetitive transcranial magnetic stimulation (rTMS) may reduce craving and frequency of cocaine use, but little is known about its efficacy and neural effects. We sought to elucidate short- and long-term clinical benefits of 5-Hz rTMS as an add-on to standard treatment in patients with CUD and discern underlying functional connectivity effects using magnetic resonance imaging. METHODS: A total of 44 patients with CUD were randomly assigned to complete the 2-week double-blind randomized controlled trial (…

Transcranial Magnetic Stimulation: A review about its efficacy in the treatment of alcohol, tobacco and cocaine addiction.

Substance Use Disorder (SUD) is a chronic and relapsing disease characterized by craving, loss of control, tolerance and physical dependence. At present, the combination of pharmacotherapy and psychosocial intervention is the most effective management strategy in preventing relapse to reduce dropout rates and promote abstinence in SUD patients. However, only few effective medications are available. Transcranial Magnetic Stimulation (TMS) is a non-invasive brain stimulation technique that modulates the cellular activity of the cerebral cortex through a magnetic pulse applied on selected brain a…

Modulating complex brain states using MVPA-based neurofeedback: A systematic review.

Multi-voxel pattern analysis (MVPA)-based fMRI neurofeedback is a promising tool for modulating brain states and influencing cognition and behaviour, yet its effectiveness remains unclear. This systematic review investigates its efficacy across emotion regulation, fear conditioning, associative and perceptual learning, attention, craving, semantic neurofeedback and motor rehabilitation, incorporating a meta-analytic synthesis of neural effects. A systematic search of PubMed, Web of Science, IEEE, and DPBL databases identified 29 studies. Published studies exhibit considerable variability in pr…

Cue-elicited prefrontal gamma-aminobutyric acid (GABA) dynamics in cannabis use disorder.

Chronic cannabis use drives neuroadaptations in reward and stress-related circuits that condition the brain to the drug and its associated cues. These changes underlie craving and heighten its sensitivity to cue- and stress-induced triggers, sustaining drug intake and promoting relapse. Task-based functional magnetic resonance imaging of drug cue reactivity paradigms has shown the dorsolateral prefrontal cortex (dlPFC) as one region commonly activated during cue-elicited craving. However, the state-induced dynamics of critical neurometabolites in cannabis use disorder remain to be investigated…
