The role of exosomes in ankylosing spondylitis: from biological features and functional cargo to clinical applications.

Exosomes are small extracellular vesicles secreted by nearly all cell types and widely distributed in body fluids. They not only mediate intercellular material transfer but also play an important role in the regulation of immune pathways. Given their diverse biological functions, studies investigating the regulatory roles of exosomes in ankylosing spondylitis (AS) are receiving increasing attention. The functions of exosomes in AS largely depend on their bioactive cargo, including microRNAs (miRNAs), circular RNAs (circRNAs), long non-coding RNAs (lncRNAs), proteins, and other molecules. In ad

Molecular mechanisms of Polyendocrine Metabolic Ovarian Syndrome and identification of novel druggable targets for drug discovery and repositioning.

Polyendocrine Metabolic Ovarian Syndrome (PMOS) is a heterogeneous disorder that mainly affects women of reproductive age. Genetic predisposition, insulin resistance, androgen excess, inflammation, mitochondrial dysfunction, and changes in gut microbial composition were notable contributors to the prognosis of PMOS. Several genetic variations, including mutations, polymorphisms, and variations in gene regulation, have been identified that predispose individuals to PMOS. A complex network involving metabolic, hormonal, and inflammatory pathways plays a major role in disease progression. To iden

Vav2 is a master regulator of repair against bacterial pore-forming toxins.

Despite antibiotic therapy, 25-35% of patients with necrotizing soft tissue infections (NSTIs) die. The etiologic agents for NSTIs include Streptococcus pyogenes and Clostridium perfringens, which secrete the cholesterol-dependent cytolysins (CDCs) streptolysin O and perfringolysin O to disrupt cell membranes. Whereas cells resist this damage by activating Ca2+-dependent repair pathways, including mitogen-activated protein kinase kinase (MEK)-dependent microvesicle shedding, dysferlin-mediated patch repair, and annexin-mediated membrane clogging, the upstream regulators of these responses have

Adrenal injury induced by real-ambient PM2.5: Mechanisms of disrupted Shh/Wnt signaling crosstalk.

Fine particulate matter (PM2.5) exposure is known to induce systemic endocrine disruption, yet the specific mechanisms underlying adrenal in … pathways. The data suggest that PM2.5 may hinder adrenal self-renewal and repair by impairing these critical pathways. Collectively, our findings indicate that PM2.5-induced adrenal dysfunction involves inflammatory responses and the disruption of Shh and Wnt signaling, highlighting their potential key role in preserving adrenal endocrine health.

PBX1 as a novel master regulator in cancer: Its regulation, molecular biology, and therapeutic applications.

PBX1 is a critical transcription factor at the top of various cell fate-determining pathways. In cancer, PBX1 stands at the crossroads of multiple oncogenic signaling pathways and mediates responses by recruiting a broad repertoire of downstream targets. Research thus far has corroborated the involvement of PBX1 in cancer proliferation, resisting apoptosis, tumor-associated neoangiogenesis, epithelial-mesenchymal transition (EMT) and metastasis, immune evasion, genome instability, and dysregulating cellular metabolism. Recently, our understanding of the functional regulation of the PBX1 protei

UBA1/GARS-dependent pathways drive sensory-motor connectivity defects in spinal muscular atrophy.

Deafferentation of motor neurons as a result of defective sensory-motor connectivity is a critical early event in the pathogenesis of spinal muscular atrophy, but the underlying molecular pathways remain unknown. We show that restoration of ubiquitin-like modifier-activating enzyme 1 (UBA1) was sufficient to correct sensory-motor connectivity in the spinal cord of mice with spinal muscular atrophy. Aminoacyl-tRNA synthetases, including GARS, were identified as downstream targets of UBA1. Regulation of GARS by UBA1 occurred via a non-canonical pathway independent of ubiquitylation. Dysregulatio

The neglected neglect: auditory neglect.

Whereas visual and somatosensory forms of neglect are commonly recognized by clinicians, auditory neglect is often not assessed and therefore neglected. The auditory cortical processing system can be functionally classified into 2 distinct pathways. These 2 distinct functional pathways deal with recognition of sound ("what" pathway) and the directional attributes of the sound ("where" pathway). Lesions of higher auditory pathways produce distinct clinical features. Clinical bedside evaluation of auditory neglect is often difficult because of coexisting neurological deficits and the binaural na

Protein kinase inhibitors against malignant lymphoma.

IntroductionTyrosine kinases (TKs) are intimately involved in multiple signal transduction pathways regulating survival, activation, proliferation and differentiation of lymphoid cells. Deregulation or overexpression of specific oncogenic TKs is implicated in maintaining the malignant phenotype in B-lineage lymphoid malignancies. Several novel targeted TK inhibitors (TKIs) have recently emerged as active in the treatment of relapsed or refractory B-cell lymphomas that inhibit critical signaling pathways, promote apoptotic mechanisms or modulate the tumor microenvironment.Areas coveredIn this r

Protein breakdown in muscle wasting: role of autophagy-lysosome and ubiquitin-proteasome.

Skeletal muscle adapts its mass as consequence of physical activity, metabolism and hormones. Catabolic conditions or inactivity induce signaling pathways that regulate the process of muscle loss. Muscle atrophy in adult tissue occurs when protein degradation rates exceed protein synthesis. Two major protein degradation pathways, the ubiquitin-proteasome and the autophagy-lysosome systems, are activated during muscle atrophy and variably contribute to the loss of muscle mass. These degradation systems are controlled by a transcription dependent program that modulates the expression of rate-lim

Germ cell proteins in melanoma: prognosis, diagnosis, treatment, and theories on expression.

Germ cell protein expression in melanoma has been shown to correlate with malignancy, severity of disease and to serve as an immunologic target for therapy. However, very little is known about the role that germ cell proteins play in cancer development. Unique germ cell pathways include those involved in immortalization, genetic evolution, and energy metabolism. There is an ever increasing recognition that within tumors there is a subpopulation of cells with stem-cell-like characteristics that play a role in driving tumorgenesis. Stem cell and germ cell biology is intertwined. Given the enormo

Modulating co-stimulation.

The modulation of co-stimulatory pathways represents a novel therapeutic strategy to regulate autoimmune diseases. Auto-reactive CD4+ T cells play a critical role in initiating the immune response leading to inflammation and autoimmune diseases. Blocking co-stimulatory signals prevents T-cell activation, thus diminishing autoimmune responses and possibly preventing the progression of autoimmune disease. Blockade of several co-stimulatory pathways has been investigated in animal models and has led to clinical trials testing specific blocking agents in humans. In this review we will describe the

Pathway-level profiling of the sepsis proteome reveals immune and transcriptional dysregulation.

BACKGROUND: Sepsis triggers a complex and heterogeneous host response, yet decades of biomarker studies have yielded few targeted therapeutics that improve patient outcomes. Single analyte approaches fail to capture the coordinated biological programs that drive organ dysfunction. Focusing on single or limited panels of biomarkers to endotype disease fundamentally misrepresents sepsis biology, which reflects simultaneous disruption of multiple cellular networks. However, pathway-level bioinformatic analyses interpret proteins as components of larger biological systems, enabling detection of co

Ciliary ARLs drive renal cystogenesis.

BACKGROUND: Polycystic kidney disease (PKD) is the leading genetic cause of renal failure, resulting in the accumulation of fluid filled cysts and gross enlargement of the kidney. Mutations in PKD1 or PKD2, which encode ciliary polycystin proteins, are the most common cause of PKD. These proteins function in a cilia-dependent cyst activation (CDCA) pathway-one that requires cilia for its pro-cystic function-yet the molecular driver(s) of this pathway are unknown. ARL13B is a regulatory GTPase enriched in cilia with guanine nucleotide exchange factor (GEF) activity and links to renal cystogenes

Determinants of sustained slow pathway conduction and relation to reentrant tachycardia in patients with dual atrioventricular nodal transmission.

In 24 patients with dual atrioventricular (AV) nodal pathways, multiple incremental atrial pacing studies were performed to obtain atrial (A) to His (H) basic driven (A1 and H1) and extrastimulus (A2 and H2) intervals. Discontinuous A1-A2 and H1-H2 intervals were analyzed for relations between initial coupling times and subsequent A-H responses, and to examine curves of sequential paced cycle lengths (A-A intervals) versus A-H intervals. Seventeen patients showed sustained slow pathway (SP) conduction with demonstration of discontinuous A-A and A-H curves. Sustained SP conduction occurred at c

Iron-responsive transcription factor SreA regulates calcium homeostasis independently of calcineurin-Crz1 pathway in Fusarium graminearum.

Calcium is an abundant intracellular ion that is intricately involved in various cellular processes. The precise regulation of calcium homeostasis is critical for fungal physiology. In fungi, calcium signaling is primarily mediated by the calmodulin-calcineurin pathway, with the transcription factor Crz1 acting as a key downstream regulator of calcium homeostasis. However, other transcription factors that contribute to calcium homeostasis in Fusarium graminearum are yet to be systematically identified. Here, we screened 675 transcription factor deletion mutants of F. graminearum under calcium

Disruptions of cell signaling pathways in myotonic dystrophy type 1 skeletal muscle, their pathogenic impact, and potential for combinatorial therapeutics.

Myotonic dystrophy type 1 (DM1) is caused by a CUG expansion located in the 3' UTR of dystrophia myotonica protein kinase mRNAs. The pathogenic model underlying DM1 implicates the accumulation of mutant dystrophia myotonica protein kinase transcripts in nuclei where they form toxic RNA foci. This, in turn, disrupts the availability of RNA-binding proteins causing widespread missplicing of mRNAs. Over the years, multiple signaling pathways have also been reported to be disrupted in DM1, especially in skeletal muscle. Here, we focus on several pathways including protein kinase R, PKC, glycogen s

Molecular Signaling Pathways, Regulatory and Coactivator Networks, and Emerging Mechanisms in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is the most common primary liver malignancy and a leading cause of cancer-related mortality worldwide. Despite advances in diagnosis and therapy, the prognosis for advanced HCC remains poor due to late-stage diagnosis, high recurrence rates, therapeutic resistance, and pronounced molecular heterogeneity. HCC development is driven by complex somatic gene alterations, epigenetic reprogramming, dysregulated signaling pathways, metabolic changes, and an immunosuppressive tumor microenvironment. Molecular profiling studies have identified key oncogenic pathways involv

PDE10A as a novel diagnostic and therapeutic target in cancer: insights and challenges.

Phosphodiesterase 10A (PDE10A) is a dual-substrate enzyme that hydrolyzes both cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP), playing a critical role in regulating intracellular signaling pathways. While its function has been extensively studied in the central nervous system, emerging evidence highlights its broader physiological and pathological relevance, including its involvement in cancer. Functionally, it modulates key signaling pathways such as cAMP/protein kinase A (PKA) and cGMP/protein kinase G (PKG), influencing cell proliferation, differentiation, a

Novel pacing maneuvers for mapping the upstream insertion of unidirectional accessory pathways.

BACKGROUND: Anatomic characterization of unidirectional accessory pathways (APs) is inherently limited to the localization of the downstream insertion site. The inability to define the full anatomic course of unidirectional pathways can limit the safety and effectiveness of ablation in the setting of complex pathways, slanted pathways, unstable catheter positioning at downstream insertions, or insertions near the conduction system. OBJECTIVE: We aimed to develop novel pacing maneuvers to localize upstream insertions of unidirectional APs. METHODS: Two methods were evaluated: localizing the s
