Moderate to Severe Obstructive Sleep Apnea in Military Personnel Is Not Associated With Decreased Exercise Capacity.

STUDY OBJECTIVES: Studies of older and less active patients with obstructive sleep apnea (OSA) have reported decreased exercise capacity as … personnel with moderate to severe OSA do not have decreased exercise capacity. The effect of OSA on exercise tolerance may be influenced by additional factors and is likely too small to be noted in this population. COMMENTARY: A commentary on this article appears in this issue on page 819.

Home-based Circuit Training and Community Walking for Intermittent Claudication.

BACKGROUND: Supervised exercise training is recommended for people with peripheral artery disease (PAD), yet it remains underutilized. Home-based exercise programs (HBEPs) are a potential alternative. The aim of this study was to assess the feasibility of conducting a full scale trial of a 12-week HBEP for people living with symptomatic PAD. METHODS: In a randomized feasibility trial, patients with intermittent claudication were allocated to either an HBEP or a nonexercise control. The HBEP group was given a Fitbit to use during a 12-week exercise program comprising of personalized step goals

Exercise intolerance and oxygen dynamics in nontuberculous mycobacteria with bronchiectasis.

BACKGROUND: Nontuberculous mycobacterial pulmonary disease (NTM-PD) patients often have exercise intolerance. Pulmonary rehabilitation (PR) … CONCLUSIONS: Exertional oxygen consumption, independent of ventilatory ability, is associated with exercise tolerance and ventilatory efficiency, while being related to tachypnea and bronchiectasis rather than cavitation in NTM-PD patients. These findings may be useful in considering exercise physiology-based PR for NTM-PD patients with exertional dyspnea.

Mechanism of exercise intolerance in heart diseases predicted by a computer model of myocardial demand-supply feedback system.

BACKGROUND AND OBJECTIVE: The myocardial demand-supply feedback system plays an important role in augmenting blood supply in response to exe … nary flow reserve deteriorates faster during graded exercise, which in turn, suggests a decrease in exercise tolerance for patients with stenosis, myocardial inefficiency and metabolic flow regulation dysfunction. The findings in this study may have clinical implications in diagnosing cardiovascular diseases.

Serum fetuin-A and Ser312 phosphorylated fetuin-A responses and markers of insulin sensitivity after a single bout of moderate intensity exercise.

Fetuin-A (Fet-A), secreted by the liver and adipose tissue, inhibits insulin receptor tyrosine kinase activity and modulates insulin action. Numerous studies have shown association of elevated serum Fet-A concentrations with obesity, non-alcoholic fatty liver disease, and type 2 diabetes. Both moderate body weight loss (5%-10%) and significant body weight loss have been shown to decrease serum Fet-A and improve insulin sensitivity. Currently, there are no studies examining the effects of a single bout of exercise on serum Fet-A or Ser312-pFet-A (pFet-A) responses. We hypothesized that a single

Moderate exercise training since adolescence reduces Walker 256 tumour growth in adult rats.

KEY POINTS: Cancer growth, cell proliferation and cachexia index can be attenuated by the beneficial programming effect of moderate exercise training, especially if it begins in adolescence. Walker 256 tumour-bearing rats who started exercise training during adolescence did not revert the basal low glycaemia and insulinaemia observed before tumour cell inoculation. The moderate exercise training improved glucose tolerance and peripheral insulin sensitivity only in rats exercised early in adolescence. The chronic effects of our exercise protocol are be beneficial to prevent cancer cachexia and

Physical allergies.

Allergic responses that occur as a result of exposure to physical stimuli are discussed. Most of these conditions are mediated by vasoactive substances, resulting in urticaria and/or angioedema. Susceptible individuals who engage in athletic activities may place themselves at particular risk for these problems. The physical allergies include cholinergic urticaria, exercise-induced anaphylaxis, cold urticaria, dermatographism, solar urticaria, and aquagenic urticaria. Management of these conditions includes patient education, selective avoidance, antihistamines, and, in some cases, induction of

Surgical treatment of congenital pulmonary stenosis due to dysplastic leaflets and small valve anulus.

Seven cases of pulmonary valve stenosis due to congenital dysplasia of the valvular apparatus have been corrected by patch-graft enlargement of the right ventricular outflow tract. The six surviving patients (85%) are now leading normal lives with good exercise tolerance. The satisfactory right ventricular performance, manifested by these subjects after the operation and in the long-term follow-up, supports the validity of the procedure in the presence of a normal right ventricle and tricuspid valve.

Minimal intensity physical activity (standing and walking) of longer duration improves insulin action and plasma lipids more than shorter periods of moderate to vigorous exercise (cycling) in sedentary subjects when energy expenditure is comparable.

BACKGROUND: Epidemiological studies suggest that excessive sitting time is associated with increased health risk, independent of the performance of exercise. We hypothesized that a daily bout of exercise cannot compensate the negative effects of inactivity during the rest of the day on insulin sensitivity and plasma lipids. METHODOLOGY/PRINCIPAL FINDINGS: Eighteen healthy subjects, age 21±2 year, BMI 22.6±2.6 kgm(-2) followed randomly three physical activity regimes for four days. Participants were instructed to sit 14 hr/day (sitting regime); to sit 13 hr/day and to substitute 1 hr of sittin

Cardiopulmonary Exercise Testing in Repaired Tetralogy of Fallot: Multiparametric Overview and Correlation with Cardiac Magnetic Resonance and Physical Activity Level.

Patients with repaired Tetralogy of Fallot (rToF) typically report having preserved subjective exercise tolerance. Chronic pulmonary regurgitation (PR) with varying degrees of right ventricular (RV) dilation as assessed by cardiac magnetic resonance imaging (MRI) is prevalent in rToF and may contribute to clinical compromise. Cardiopulmonary exercise testing (CPET) provides an objective assessment of functional capacity, and the International Physical Activity Questionnaire (IPAQ) can provide additional data on physical activity (PA) achieved. Our aim was to assess the association between CPET…

Exercise therapy in primary biliary cirrhosis: the importance of moving while sitting on a surgical waiting list-a case study.

BACKGROUND: It is being increasingly recognised that reduced cardiorespiratory fitness is associated with poorer outcomes after major surgery. Exercise limitation and reduced aerobic capacity are common in people with end-stage liver disease. There is limited evidence about the role of exercise therapy in the management of primary biliary cirrhosis (PBC) and no studies have looked at the effect of exercise in people with PBC who are awaiting liver transplantation. This case study is the first to report that personalised exercise therapy improves cardiorespiratory fitness in a patient with PBC

Exercise attenuates cisplatin-induced anorexia-cachexia via modulation of hypothalamic inflammation and orexigenic signaling.

PURPOSE: Chemotherapy-induced anorexia cachexia (CAC) is a common adverse effect of cancer treatment and is associated with poor prognosis and reduced treatment tolerance. Despite its clinical relevance, effective interventions targeting central mechanisms of CAC remain limited. This study investigated whether aerobic exercise modulates hypothalamic appetite regulation and inflammatory signaling in a mouse model of cisplatin-induced CAC. METHODS: Eight-week-old C57BL/6 mice were assigned to a normal control (CON, n = 7), cisplatin-treated sedentary (CIS-CON, n = 7), or cisplatin-treated exerc

Non-invasive vagus nerve stimulation and exercise capacity in healthy volunteers: a randomized trial.

BACKGROUND AND AIMS: Vagal parasympathetic dysfunction is strongly associated with impaired exercise tolerance, indicating that coordinated autonomic control is essential for optimizing exercise performance. This study tested the hypothesis that autonomic neuromodulation by non-invasive transcutaneous vagus nerve stimulation (tVNS) can improve exercise capacity in humans. METHODS: This single-centre, randomized, double-blind, sham-controlled, crossover trial in 28 healthy volunteers evaluated the effect of bilateral transcutaneous stimulation of vagal auricular innervation, applied for 30 min…

Increased maternal exercise of moderate intensity improves pregnancy outcomes of gestational diabetes mellitus patients through maintaining the balance of the gut microbiota.

BACKGROUND: Exercise therapy can reduce insulin resistance during pregnancy and improve glucose tolerance in women with gestational diabetes mellitus (GDM), leading to better pregnancy outcomes. This study aimed to investigate the effects of different exercise levels on GDM from the perspective of gut microbiota. METHODS: Ninety patients with GDM were enrolled and divided into two groups: the L (n = 50) and the M (n = 40) groups. The L group performed 150 min of aerobic exercise per week, while the M group exercised for 200 min per week. After 8 weeks of intervention, fecal samples from each
