Altered processing of contextual information during fear extinction in PTSD: an fMRI study.

Medial prefrontal cortical areas have been hypothesized to underlie altered contextual processing in posttraumatic stress disorder (PTSD). We investigated brain signaling of contextual information in this disorder. Eighteen PTSD subjects and 16 healthy trauma-exposed subjects underwent a two-day fear conditioning and extinction paradigm. On day 1, within visual context A, a conditioned stimulus (CS) was followed 60% of the time by an electric shock (conditioning). The conditioned response was then extinguished (extinction learning) in context B. On day 2, recall of the extinction memory was te…

Observational fear learning involves affective pain system and Cav1.2 Ca2+ channels in ACC.

Fear can be acquired vicariously through social observation of others suffering from aversive stimuli. We found that mice (observers) developed freezing behavior by observing other mice (demonstrators) receive repetitive foot shocks. Observers had higher fear responses when demonstrators were socially related to themselves, such as siblings or mating partners. Inactivation of anterior cingulate cortex (ACC) and parafascicular or mediodorsal thalamic nuclei, which comprise the medial pain system representing pain affection, substantially impaired this observational fear learning, whereas inacti…

Glucocorticoids do not reduce subjective fear in healthy subjects exposed to social stress.

BACKGROUND: Previous experiments in patients with phobia have shown that the administration of glucocorticoids reduces fear in phobic situations. Extensive evidence indicates that elevated glucocorticoid levels inhibit memory retrieval processes. In patients with phobia, exposure to a phobic stimulus (socio-evaluative stress test) provokes retrieval of stimulus-associated fear memory that leads to a fear response. It is therefore possible that glucocorticoids reduce phobic fear by inhibiting retrieval of the previously acquired fear memory. Whether glucocorticoids reduce subjective fear also i…

Physical exercise can reverse the deficit in fear memory induced by maternal deprivation.

Maternal deprivation during the first 10 days of life induces significant behavioral alterations in rodents which persist through adulthood. Physical exercise reduces the cognitive deficits associated with pharmacologic and pathological conditions. Here we investigated whether forced physical exercise alters memory deficits caused by postnatal maternal deprivation. Male rats were divided into four groups: (1) control, (2) deprived, (3) exercised, and (4) deprived+exercised. In groups 2 and 4, pups were deprived from their mothers for 3h/day during the first 10 days post-birth. In groups 3 and

Rapid eye movement sleep deprivation does not affect fear memory reconsolidation in rats.

There is increasing evidence that sleep may be involved in memory consolidation. However, there remain comparatively few studies that have explored the relationship between sleep and memory reconsolidation. At present study, we tested the effects of rapid eye movement sleep deprivation (RSD) on the reconsolidation of cued (experiment 1) and contextual (experiment 2) fear memory in rats. Behaviour procedure involved four training phases: habituation, fear conditioning, reactivation and test. Rats were subjected to 6h RSD starting either immediately after reactivation or 6h later. The control ra…

Fear conditioning in adolescents with anxiety disorders: results from a novel experimental paradigm.

OBJECTIVE: Considerable research examines fear conditioning in adult anxiety disorders but few studies examine youths. Adult data suggest that anxiety disorders involve elevated fear but intact differential conditioning. We used a novel paradigm to assess fear conditioning in pediatric anxiety patients. METHOD: Sixteen individuals with anxiety disorders and 38 healthy comparisons viewed two photographs of actresses displaying neutral expressions. One picture served as the conditioned stimulus (CS), paired with a fearful expression and a shrieking scream (CS+), whereas the other picture served…

Axillary nerve palsy after retrograde humeral nailing: clinical confirmation of an anatomical fear.

INTRODUCTION: Locked antegrade or retrograde nailing of humeral shaft and proximal humerus fractures is a well established treatment option. Anatomic-morphological studies revealed a potential high risk of axillary nerve injury within proximal interlocking screw insertion. However, clinical experiences do not seem to confirm this, as there is a lack of interlocking screw insertion associated axillary lesions in literature. CASE REPORT: We report about a 69-year-old man with a humeral shaft fracture (AO-type 12-A3) stabilized by a retrograde implanted interlocking nail. Proximal interlocking s

Automatic processing in spider phobia: implicit fear associations over the course of treatment.

This study evaluates the cognitive model of anxiety by investigating treatment-related changes in automatic associations to evaluate schematic processing. Spider-phobic participants (n = 31) and healthy controls (n = 30) completed fear-based Implicit Association Tests (IATs), which are reaction-time measures that tap implicit associations without requiring conscious introspection. The specific tasks involved classifying pictures of snakes and spiders along with semantic categorizations (good vs.bad, afraid vs. unafraid, danger vs. safety, and disgusting vs. appealing). Phobic individuals were …

Conditioned memory modulation, freezing, and avoidance as measures of amygdala-mediated conditioned fear.

Three conditioned aversive responses were used to infer the existence of an unobservable central state of "conditioned fear," and the roles of certain amygdala subregions in producing these responses were investigated. Rats received tone-shock pairings in one compartment of a shuttle box and no tones or shocks in the other, distinctive, compartment. They were then trained to find food in one arm of a Y-maze. After the final training trial they were exposed to different sets of stimuli in the shuttle box with no shock. Twenty-four hours later rats that had received immediate posttraining exposu…

Do panic patients process unconditioned fear vs. conditioned anxiety differently than normal subjects?

Panic patients were evaluated with two models of experimental anxiety that are believed to generate distinct emotional states: (1) a stimulated public speaking test (SPS), a presumed indicator of unconditioned fear, and (2) conditioning of skin conductance responses (CSCR) to a tone associated with an aversive white noise, an index of conditioned anxiety. Subjective states were evaluated through the visual analogue mood scale (VAMS) and a bodily symptoms scale (BSS). In the SPS test, panic patients showed higher baseline levels of VAMS-measured anxiety than controls. Unlike controls, panic pat…

Infant vocalization, adult aggression, and fear behavior of an oxytocin null mutant mouse.

Previous studies have shown that oxytocin (OT)-deficient female mice produced by homologous recombination fail to lactate but exhibit normal parturition and reproductive behaviors. We examined the ultrasonic vocalizations of infant mice and the subsequent aggressive and fear behavior of adult male OT knockout (OT-KO) mice. Infant OT-KO mice were less vocal than wild-type (WT) control mice during separations from the mother and peers. Adult OT-KO males were generally more aggressive in isolation-induced and resident-intruder tests of aggression and less fearful in the plus maze and acoustic sta…

"Cleaning-up cognition" in triple-response fear assessment through individualized functional behavior analysis.

Improvements in behavioral assessment spurred by the triple-response concept have been overshadowed by a preoccupation with content of assessment and a lack of regard for the context of assessment. The aims of this article are to (a) clarify the imprecise use of the verbal-subjective-cognitive mode and to reinterpret cognitive events based on evidence and methods derived from clinical behavior analysis, (b) discuss the limitations of the triple-response assessment framework, and (c) suggest an alternative functional idiographic approach to assessment and treatment that may direct attention tow

A cross-sectional study of fear of surgery in female breast cancer patients: Prevalence, severity, and sources, as well as relevant differences among patients experiencing high, moderate, and low fear of surgery.

BACKGROUND: Fear of surgery has been associated with more postoperative pain, disability, and a lower quality of life among patients undergoing various surgical procedures. While qualitative studies indicate breast cancer patients to be afraid of surgery, detailed quantitative analyses are lacking. The present research aimed at investigating the prevalence, severity, and sources of fear of surgery in this patient group and to compare patients reporting different degrees of such fear. METHODS: This cross-sectional study included 204 breast cancer patients, 18-70 years old, and scheduled for su…

Fear-antagonistic actions and safety behaviors in the treatment of acrophobia: A randomized controlled trial.

Exposure therapy effectively treats anxiety disorders, yet non-response and premature dropout motivate continued work on procedural enhancements. Fear-antagonistic actions, behaviors that directly oppose fear-driven action tendencies (e.g., walking briskly toward and leaning over a railing rather than standing back from the edge), can augment exposure, but performing them necessarily precludes safety behaviors. This study examined whether fear-antagonistic actions confer augmentation effects beyond safety behavior elimination alone. University students and community adults with long-standing, …

The FEAR Framework for Recovery After Total Hip and Knee Arthroplasty: A Systematic Review.

Fear-related psychological barriers are increasingly recognized contributors to persistent pain, functional limitation, and dissatisfaction following total hip and knee arthroplasty. This systematic review synthesizes evidence on fear-related constructs and postoperative recovery and introduces the FEAR Framework as a structured model for clinical interpretation. This review (1) summarizes associations between fear-related constructs and postoperative outcomes; (2) organizes these constructs into four domains: Fall Confidence, Exposure Avoidance, Anticipatory Anxiety, and Reinjury Fear; (3) re…

Are you afraid? Neurophysiological responses to multidimensional fear in an interactive semi-immersive environment.

INTRODUCTION: Objective fear recognition is crucial for clinical evaluation and understanding emotional processing. This study uses a multi-modal biophysical approach (EEG, HRV, EDA) within an interactive, semi-immersive environment to investigate coordinated brain-body responses across multidimensional fear scenarios. METHODS: Ten participants experienced four fear-related scenes mapped to Kantian cognitive dimensions (Embodied, Conceptual, Discursive, Integrative) featuring real-time adaptive audiovisuals. EEG, HRV, and EDA metrics were concurrently recorded and normalized to a resting base…

Letter to the editor: Acupuncture as a phenotype-specific adjunct for fear-of-falling in Parkinson's disease.

INTRODUCTION: Fear-of-falling (FoF) in Parkinson's disease (PD) is a complex symptom with heterogeneous drivers. Recent work by Onder et al. (2026) delineated three distinct FoF phenotypes, highlighting anxiety as an independent predictor and emphasizing the need for personalized therapeutic strategies beyond conventional motor-focused approaches. METHODS: This perspective analyzes the pathophysiological rationale and existing preclinical and clinical evidence supporting acupuncture as a phenotype-specific adjunctive therapy. We evaluate evidence for its mechanisms (e.g., HPA axis modulation,…
