Described is the first catalytic, asymmetric synthesis of (-)-podophyllotoxin and its C(2)-epimer, (-)-picropodophyllin. Asymmetry is achieved via the enzymatic desymmetrization of advanced meso diacetate 20, through PPL-mediated ester hydrolysis. Multiple sclerosis may present an increased risk for venous thromboembolism. Ophthalmological symptoms include loss of vision, visual field loss, changes in color vision, diplopia and nystagmus. First-line treatments for multiple sclerosis are beta-interferon, glatiramer acetate, dimethyl fumarate and teriflunomide. To the best of our knowledge, no ophthalmologic side effects have been reported with glatiramer acetate. We present a woman with multiple sclerosis on glatiramer acetate therapy with a central retinal vein occlusion in the absence of other risk factors.

Tenofovir disoproxil fumarate is a nucleotide reverse transcriptase inhibitor and has been extensively used in the first-line treatment of viral infectious diseases such as chronic hepatitis B. Despite its good safety, the development of Fanconi syndrome is a rare adverse effect of long-term tenofovir therapy. Here, we report a case of a 62-year-old diabetic woman with a history of chronic hepatitis B who was exposed to tenofovir and developed drug-associated Fanconi syndrome. After discounting tenofovir, the patient's bone pain was markedly reduced.

Tenofovir disoproxil fumarate (TDF) is widely used as an effective first-line therapy for chronic hepatitis B (CHB) infection. While TDF demonstrates successful viral suppression, it has been linked to the development of renal proximal tubular (PT) dysfunction, leading to Fanconi syndrome. However, Fanconi syndrome has been rarely reported in CHB-monoinfected patients, and there were no reports of TDF-associated nephrotic syndrome. Here, we report a case of combined Fanconi and nephrotic syndrome in CHB patients after TDF exposure.

We report 3 cases of patients with HIV/AIDS in whom Fanconi syndrome and nephrogenic diabetes insipidus developed secondary to use of an antiretroviral regimen containing tenofovir disoproxil fumarate and didanosine. These patients presented with a history of polydipsia, polyuria, weight loss, anorexia, and wasting. Interestingly, 1 patient was not taking protease inhibitors. This response is a well-documented yet uncommon complication of tenofovir use in the HIV population. We recommend continued monitoring for renal toxicity when using NRTI combination of tenofovir and didanosine.

BRIEF DESCRIPTION OF CASE PRESENTATION: Two older patients with advanced PD and dementia presented with a peculiar type of hallucination, in the form of seeing and feeling threads on the surfaces or coming out from the fingers. They continued manipulating these imaginary threads before putting them aside. Quetiapine was helpful in both cases.

Studies in several countries have shown a low rate of clozapine use despite the fact that approximately 30% of schizophrenia cases are treatment-resistant. An 81-year-old female presented with a right pontine infarct and later developed recurrent vivid hallucinations. After a workup for delirium and hallucinosis was unrevealing, a diagnosis of peduncular hallucinosis (PH) was proposed. Quetiapine is one of the drugs used most in current Psychiatry due to its therapeutic efficiency and clinical safety. We report the case of a 26-year-old male patient with severe mental retardation due to Y-chromosome partial deletion who initiated treatment with quetiapine for the control of his aggressiveness, and who developed severe rhabdomyolysis two weeks later. In spite of the confirmed clinical safety of quetiapine, doctors must monitor the appearance of rare but serious adverse effects as that presented in this clinical case.
