Medical illustration in China: Lam Qua and his Western-style paintings.

Lam Qua, a Chinese artist who painted in the Western style, was hired by American ophthalmologist and missionary Peter Parker to paint Parker's Chinese patients. Most of Lam Qua's paintings depict patients with rather large tumors located in various areas across the body; there are a few that are also of dermatologic interest. This contribution explores Lam Qua's paintings that depict a cutaneous horn, gangrenous necrosis, and an ulcerating tumor.

A simplified fascial model of pelvic anatomical surgery: going beyond parametrium-centered surgical anatomy.

The classical surgical anatomy of the female pelvis is limited by its gynecological oncological focus on the parametrium and burdened by its modeling based on personal techniques of different surgeons. However, surgical treatment of pelvic diseases, spreading beyond the anatomical area of origin, requires extra-regional procedures and a thorough pelvic anatomical knowledge. This study evaluated the feasibility of a comprehensive and simplified model of pelvic retroperitoneal compartmentalization, based on anatomical rather than surgical anatomical structures. Such a model aims at providing an

Bilateral prophylactic mastectomy in BRCA mutation carriers: what surgeons need to know.

Aim of this paper is to allows to analyze a topic of great relevance and media interest such as the role of prophylactic mastectomy in healthy women with BRCA mutation proposing to the surgeons some useful informations for decision-making. Less than 15% of all breast cancers are associated with germline genetic mutations. The majority of hereditary breast tumors are due to mutations in BRCA1 and BRCA2 genes that are responsible for only one third of hereditary cases. The risk estimates are extremely heterogeneous with a mean cumulative lifetime breast cancer risk of approximately 72% in BRCA1

Communication through simulation: Developing a curriculum to teach interpersonal skills.

BACKGROUND: Nontechnical issues are linked to up to 60% of adverse events in surgery and have become one of the core competencies of surgical education. Simulation-based training allows development of these skills. We hypothesized that we could both assess and improve the communication skills of interdisciplinary teams through a simulated crisis situation. METHODS: We designed a simulated crisis involving the intraoperative development of a tension pneumothorax. Audiovisual analysis was used to evaluate team interaction, and a combination of proctored assessment and self-assessment was used t

Ureteral inguinal hernia: an uncommon trap for general surgeons.

Inguinal hernias involving the ureter, a retroperitoneal structure, is an uncommon phenomenon. It can occur with or without obstructive uropathy, the latter posing a trap for the unassuming general surgeon performing a routine inguinal hernia repair. Ureteral inguinal hernia should be included as a differential when a clinical inguinal hernia is diagnosed concurrently with unexplained hydronephrosis, renal failure or urinary tract infection particularly in a male. The present case describes a patient with a known ureteroinguinal hernia who proceeded to having a planned hernia repair and ureter

Clinical experience of surgically treating giant neurofibromatosis-1.

The surgical treatment for giant neurofibromatosis-1 (NF-1) requires comprehensive measures. Presently, there is no systematic description of surgical treatment. Because of its high level of risk, we want to share our clinical experience. From 2011 to 2014, patients (n = 8, 5 female and 3 male patients, aging from 31 to 45 years-old) were included in the study. The tumours were located on the trunk (n = 5) or face (n = 3). In addition to routine examination, blood storage was also prepared. Preoperative consultation from related departments was critical at first. Related artery embolisation wa

Vivien Thomas (1910-1985): the backstage pioneer and educator.

Vivien Thomas (1910-1985) was an African-American scientist, pioneer, and renowned educator. Thomas's contributions to cardiovascular surgery were unequivocal, and yet it was only after his death that he gained more widespread recognition. Thomas's more notable work involves aiding in the discovery of the cause of traumatic shock, designing and guiding the first operation to treat Tetralogy of Fallot, carrying out the first atrial septectomy, and helping develop the electrical defibrillator. Thomas struggled amidst the adversities of racism and the Great Depression (1929-1941), armed merely wi

Jan Mikulicz-Radecki (1850-1905): a fundamental contributor to world surgery; surgeon of the head, neck, and esophagus.

We present some of many valuable and unique achievements of Jan Mikulicz-Radecki with special regard to his contribution to laryngology. He constructed esophagogastroscope, and was one of the first to perform endoscopy of esophagus and ventricle. He published several papers describing new approaches to maxillary sinus through inferior meatus, surgical management of tonsillar cancer via lateral pharyngotomy, correction of post-traumatic nasal deformations, and the use of iodophorm in healing wounds. Among Mikulicz's many celebrated scientific achievements, the most important remains the develop

[The birth of modern surgery].

In ancient India and Persia surgeons were highly respected and their operating skill with nose plastic and catatact couching is documented. In mediaeval Europe surgery was classified as a handicraft profession and belonged to the barbers guild with the soapcup as symbol, much inferior to the academic trained medical doctors. In war surgery leg amputation after shotwounds demanded great rapidity, since no anestetic but alcohol was available. In the 18th century surgeons becamea accepted as medical doctors e.g. John Hunter in London and OlofAcrel in Stockholm. A great step in the development of

How effective are current therapies for acromegaly?

Currently available therapies for acromegaly are transsphenoidal surgery (TSS), radiotherapy (RT) and medical therapy with the dopamine agonists and somatostatin analogues. The goals of these therapies for acromegaly are to normalize excessive hormone secretion, thus normalizing serum levels of growth hormone (GH) and of insulin-like growth factors (IGF-I), to reduce the clinical signs and symptoms of acromegaly and to reduce tumor size in order to relieve any symptoms due to tumor mass effect. These goals should be accomplished while preserving pituitary function and with as few side effects

Effective use of human simulators in surgical education.

BACKGROUND: We initiated a teaching module utilizing a human simulator midway through 2001-2002 to improve student skills specific to the evaluation of patients in shock during a required clerkship in surgery for fourth-year medical students. We tested the hypothesis that student skills would improve after implementation of this module and identified factors that predicted student performance. MATERIALS AND METHODS: Students (n = 86) chose one of two hospital sites for a clerkship that focuses on the care of acutely ill surgical patients. A case-based lecture focusing on the diagnosis and man

The medical responsibility: current view from the Council of Physicians side.

The medical responsibility has been clearly defined in the Royal Decree no. 78 dated November 11, 1967 concerning the medical practice. Moreover, several articles from the Ethical Code (Code de Déontologie) have clarified some social and economical responsibilities in the medical practice (articles 99 to 103) and the quality of patient care (article 36). The National Council has also published at least 31 advises facing the daily reality and the growing insecurity. That atmosphere is coming from the jurisprudence, the increasing responsibility insurance fees, the obligation of results instead

The contribution of Allison and Nissen to the evolution of hiatus herniorrhaphy.

Even though it was not until 1950 that Barrett introduced the term "reflux esophagitis", this entity is now the most common disease afflicting the western world. Diaphragmatic herniation, recognized by Sennertus in 1541, was first repaired by Potemski (1889). Before World-War II, the condition was considered rare; symptomatology, as in external herniae, was ascribed to pinching of the stomach by the hernial ring. Only large protrusions, with signs of impending incarceration, volvulus, or strangulation, were operated upon. Modern understanding derives from studies of short, strictured esophagi.

Thomas Peel Dunhill: pioneer thyroid surgeon.

Thomas Peel Dunhill, a name by now almost completely forgotten in his native Australia, was born in 1876 near Kerang in the State of Victoria. Although he qualified as a pharmacist in 1898, Dunhill had already decided to study medicine and graduated in 1903 from the Clinical School of the Melbourne Hospital. He was regarded as an outstanding student. In 1905 Dunhill was invited to join the Senior Medical Staff at St Vincent's Hospital by Mother Berchmans Daly, the then Mother Rectress. In 1906 Dunhill was awarded the MD and in 1907 he performed his first thyroid lobectomy under local anaesthes
