A Rare Neglected Ipsilateral Humeral Triad: Comminuted Proximal Humerus Nonunion with Avascular Necrosis, Mid-Shaft Fracture, and High Radial Nerve Palsy Managed with Staged Reconstruction.

INTRODUCTION: Ipsilateral humeral shaft and proximal humerus fractures are rare and typically an outcome of high-energy trauma. The coexistence of humeral head avascular necrosis (AVN) and high radial nerve palsy in a delayed presentation makes management particularly challenging and is rarely reported in the literature. This case is important as it highlights a unique combination of bony and neural pathology requiring a carefully staged reconstructive approach. CASE REPORT: A 39-year-old Indian man arrived 4 months after a road traffic accident with left shoulder and arm discomfort, deformit

Reverse total shoulder arthroplasty for acute proximal humeral fracture has comparable 10-year outcomes to elective indications: results from the New Zealand Joint Registry.

HYPOTHESIS AND BACKGROUND: Recently, the indication of reverse total shoulder arthroplasty (RTSA) has expanded beyond rotator cuff arthropathy to include treatment of complex acute proximal humeral fracture (PHF). Limited previous studies have compared the long-term clinical and functional outcomes of patients undergoing RTSA for PHF vs. elective indications for degenerative conditions. The purpose of this study was to compare implant survivorship, reasons for revision and functional outcomes in patients undergoing RTSA for acute PHF with those undergoing elective RTSA in a population-based co

Outcomes With Surgery vs Functional Bracing for Patients With Closed, Displaced Humeral Shaft Fractures and the Need for Secondary Surgery: A Prespecified Secondary Analysis of the FISH Randomized Clinical Trial.

IMPORTANCE: Existing evidence indicates that surgery fails to provide superior functional outcome over nonoperative care in patients with a closed humeral shaft fracture. However, up to one-third of patients treated nonoperatively may require secondary surgery. OBJECTIVE: To compare the 2-year outcomes of patients who required secondary surgery with the outcomes of patients with successful initial treatment. DESIGN, SETTING, AND PARTICIPANTS: This 2-year follow-up of the Finnish Shaft of the Humerus (FISH) randomized clinical trial comparing surgery with nonoperative treatment (functional br

Comparison of the Complications, Reoperations, and Clinical Outcomes between Open Reduction and Internal Fixation and Total Elbow Arthroplasty for Distal Humeral Fractures in the Elderly: A Systematic Review and Meta-Analysis.

Distal humeral fractures are challenging injuries seen in the elderly. Open reduction and internal fixation (ORIF) are the gold standard treatments. Total elbow arthroplasty (TEA) is an alternative to ORIF. This study aimed to pool and analyze the outcomes and complications in elderly patients with distal humeral fractures treated with either ORIF or TEA by performing a meta-analysis. We searched the PubMed, Embase, Google Scholar, and Cochrane Library databases for studies that compared the clinical and functional outcomes of ORIF and TEA in patients aged 60 years or older. After screening an…

Minimal important difference and patient acceptable symptom state for common outcome instruments in patients with a closed humeral shaft fracture - analysis of the FISH randomised clinical trial data.

BACKGROUND: Two common ways of assessing the clinical relevance of treatment outcomes are the minimal important difference (MID) and the patient acceptable symptom state (PASS). The former represents the smallest change in the given outcome that makes people feel better, while the latter is the symptom level at which patients feel well. METHODS: We recruited 124 patients with a humeral shaft fracture to a randomised controlled trial comparing surgery to nonsurgical care. Outcome instruments included the Disabilities of Arm, Shoulder, and Hand (DASH) score, the Constant-Murley score, and two n

[Recent fractures of the humeral diaphysis. Apropos of a continuous series of 200 cases, of which 107 were treated with a hanging cast alone. (Arterial intra-osseous vascularization of the humerus)].

Were reviewed 200 fractures of humeral diaphysis, two cases have been excluded because they were incomplete; 175 patients out of 200 have been reexamined a long time after; 23 have not been reexamined but held a sufficient follow up. Most of the transversal fracture were met among young adults (56 per 100 traffic accidents). The intraosseous vascularisation was studied elucidating partly the greatest frequency of pseudarthrosis in the part 1/3 middle-1/3 inferior especially after exaggerated periostal elevation. Among 107 cases treated by hanging cast were noted 4 pseudarthrosis, 12 shoulder's

Purchase of both proximal and distal fragments by the calcar screw is critical when using a locking plate in the treatment of proximal humeral fractures : the role of the calcar screw as a medial bridging screw.

AIMS: Inferior positioning of the calcar screw is important in the purchase of medial screws when treating proximal humeral fractures with a locking plate. However, the impact of the overall trajectory of the calcar screw has not been investigated. The aim of this study was to investigate whether the failure of a calcar screw to purchase both proximal and distal fragments would contribute to an increased risk of failure of fixation in these patients. METHODS: Patients who underwent locking plate fixation for a proximal humeral fracture between January 2012 and December 2022 were retrospective…

Open reduction and internal fixation of three- and four-part proximal humeral fractures by intrafocal distraction: a clinical and radiographic study of thirty-two cases with five to ten years of follow-up.

PURPOSE: The disadvantage of ORIF of proximal humerus fracture (PHF) by intrafocal distraction is that once the device is inserted, it cannot be removed. This study was designed to evaluate the tolerance of such a titanium alloy device at five years of minimal follow-up (FU) and secondarily to assess the relevance of the osteosynthesis of these fractures. METHOD: Thirty-two patients operated for PHF and implanted with a titanium alloy device between 2009 and 2011 were reviewed: fifteen three-part fracture (3-PF) with mean age 65.2 ± seven years and mean FU at 86 months, and seventeen four-par

Biomechanical and clinical comparison of a novel dual-wing locking plate versus the PHILOS plate for neer three- and four-part proximal humeral fractures: a finite element analysis combined with a retrospective cohort study.

BACKGROUND: Neer three- and four-part proximal humeral fractures treated with conventional PHILOS plating are associated with high complication rates, primarily due to tuberosity re-displacement and calcar screw cut-out. We developed a novel dual-wing locking plate featuring bilateral tuberosity-capturing wing extensions and an omnidirectional ball-head calcar screw. This study compared its biomechanical performance and clinical outcomes with those of the PHILOS plate. METHODS: A three-dimensional finite element model was reconstructed from CT data using Mimics 21.0, Geomagic 2021, and Rhino …

[Proximal and distal humerus fractures in advanced age].

In contrast to distal humeral fractures, humeral head fractures represent a common injury to the old patient. For both fracture localizations osteoporosis and multi-morbidity are of significant importance. The classification for humeral head fractures in one-, two-, three- and four-part fractures is generally accepted. Valgus impacted head fractures as well as head-splitting fractures are considered a separate entity. In none or minimally displaced fractures good functional results are achieved by conservative means. Although numerous therapeutical procedure are offered in the treatment of dis…

Low and high body mass index and lower numbers of screws in the articular segment are risk factors for non-union of distal humeral fractures in the elderly: A multi-center retrospective study (TRON study).

BACKGROUND: Distal humerus fracture in the elderly is a challenging fracture for orthopedic surgeons. Non-union is one of the serious complications of distal humerus fracture after surgery. This retrospective multicenter study aimed to estimate the incidence of distal humeral non-union after open reduction and internal fixation, determine factors related to non-union, and compare the postoperative results of cases with non-union to cases with the union. METHODS: Among 423 patients diagnosed with distal humeral fracture and who were treated by surgical therapy in 2010-2020 from our database ca

Humeral torque in professional baseball pitchers.

BACKGROUND: Spontaneous fracture of the humeral shaft in throwers is a rare but well-known phenomenon. Although it has been hypothesized that the biomechanics of the throw cause such fractures, it is not clear how or when the fractures occur in the pitching motion. METHODS: The torque acting about the long axis of the humerus was calculated in 25 professional baseball pitchers throwing in game situations. RESULTS: Peak humeral axial torque reached a mean value of 92 +/- 16 Nm near the time of maximum shoulder external rotation at the end of the cocking phase. This torque tended to externally
