Ligamentous laxity and scar formation.

The character of scar tissue is relative to or influenced by ligamentous laxity. Patients who are able to passively hyperextend the terminal interphalangeal joint of the index finger to 45 degrees and beyond have a tendency to leave broad scars. This proved to be statistically significance (p less than .001). It is suggested that in female patients in particular, assessment of the passive hyperextensibility of the terminal interphalangeal joint of the index finger can be used preoperatively to determine whether the patient will be left with a cosmetically displeasing scar following any operati

Association between bariatric surgery and outcomes of total joint arthroplasty: a meta-analysis.

BACKGROUND: Total joint arthroplasty (TJA) alongside bariatric surgery (BS) is frequently operated on in obese arthritis patients. The controversy persists regarding BS before TJA, and the extent of its impact on the prognosis of TJA patients remains uncertain. To explore this, we conducted a meta-analysis. METHODS: As of 1 October 2023, the latest search on PubMed, Cochrane Library, Embase, and Web of Science was conducted to compare outcomes between patients who underwent preoperative BS and those who did not. The analysis focused on parameters such as length of stay (LOS), infection risk,

Diagnosis and Management of Traumatic Patellar Instability in the Pediatric Patient.

Instability of the patella is a common cause of knee pain and dysfunction in pediatric and adolescent patients and can be due to several factors. Although some patients will recall a specific traumatic event others may not, requiring the diagnosis to be made on the basis of physical examination and imaging. Congenital dislocation and connective tissue disorders should also be considered, even in the setting of trauma. There are radiographic parameters that may identify causes of instability such as trochlear and patellar abnormalities, and magnetic resonance imaging can identify signs of traum

Surgical outcomes of primary hip and knee replacements in patients with Parkinson's disease: a nationwide registry-based case-controlled study.

We evaluated the duration of hospitalisation, occurrence of infections, hip dislocations, revisions, and mortality following primary hip and knee replacement in 857 patients with Parkinson's disease and compared them with 2571 matched control patients. The data were collected from comprehensive nationwide Finnish health registers. The mean follow-up was six years (1 to 13). The patients with Parkinson's disease had a longer mean length of stay (21 days [1 to 365] vs 13 [1 to 365] days) and an increased risk for hip dislocation during the first post-operative year (hazard ratio (HR) 2.33, 95% c

Ultrasonographic evaluation of breech presentation as a risk factor for hip dysplasia.

In order to gain more information of breech position as a risk factor for congenital hip dysplasia or dislocation, the hips of 408 newborns delivered in the breech position were examined by ultrasound. Clinical examination was performed by both experienced paediatricians and orthopaedic surgeons. The infants were re-examined by ultrasound at 2-3 months of age. Twenty-five newborns (6.1%) had neonatal hip instability. Breech presentation as a risk factor was confirmed, with first borns, breech position with extended knees, and high birthweight as special high-risk groups. Ultrasound showed subl

Total hip arthroplasty in congenital dislocated hips.

Experience with six typical cases demonstrates that total arthroplasty for congenital dislocated hips may be associated with malalignment of the ipsilateral knee, leg-length inequality, pelvic obliquity and structural changes in the lumbosacral spine. The preoperative planning may include computerized tomography scan to obtain accurate information about the optimal placement of the socket and the size and shape of the prosthetic components. Loosening of the stem or socket rather than polyethylene wear is the expected cause of late failure. The design of the femoral component should fit the can

High-flexion posterior-substituting versus cruciate-retaining prosthesis in total knee arthroplasty: functional outcome, range of motion and complication comparison.

PURPOSE: Currently, a high-flexion cruciate-retaining knee prosthesis has been designed to allow greater advantage after total knee arthroplasty (TKA). The present study was conducted to compare functional outcome, range of motion (ROM) as well as complications in subjects who underwent either a high-flexion cruciate-retaining (HFCR, Group I) or a high-flexion posterior-substituting (HFPS, Group II) prosthesis TKA. METHODS: Thirty-four subjects which had TKA with HFCR prosthesis and thirty-three subjects which had TKA with HFPS prosthesis were enrolled in our study and were assessed preoperat

Unlocking the Mystery of Patella Dislocation-Diagnostic Methods in Pediatric Populations: A Comprehensive Narrative Review.

Background/Objectives: The diagnostic guidelines for pediatric patellofemoral instability (PFI) remain incomplete. PFI remains a challenging issue as it affects the biomechanics of the knee joint, triggers anterior knee pain, and is linked to the development of early-onset osteoarthritis. The diagnostic process is complicated by numerous anatomical factors that must be considered. This review aims to consolidate current knowledge presented in the literature on radiological diagnostics for PFI in pediatric populations, with the application of all imaging techniques-including ultrasonography (US

Medial Patellofemoral Ligament Reconstruction with Quadriceps Tendon Autograft and Double Bundle Semitendinosus Tendon Autograft: A Retrospective Comparative Study.

Patellar dislocations often result in damage to the medial patellofemoral ligament (MPFL), a key stabilizer preventing lateral patellar translation. Various reconstruction techniques, including semitendinosus (ST) and quadriceps tendon (QT) autografts, have been developed to restore stability, with QT emerging as a promising option due to lower risk of complications. We aimed to compare the functional outcomes of patients who underwent MPFL reconstruction using double bundle ST autograft and those who underwent reconstruction using partial QT autograft. Patients who underwent MPFL reconstructi

Taping for an Acute and Subacute Patellofemoral Dislocation, Recurrent Subluxation.

BACKGROUND: A primary acute patellofemoral dislocation needs to be managed well to ensure adequate tissue healing. Evidence that surgical intervention is better than conservative management is equivocal, particularly in the skeletally immature individual. Conservative management is less expensive and less invasive, and it may be better at maintaining knee function than surgical intervention. Along with appropriate immobilization, using rigid strapping tape, conservative management should consist of a tailored rehabilitation program to improve dynamic lower limb loading and quadriceps control,

[Bone dynamics of the acetabulum in experimental subluxation of the hip joint and its repair process].

Subluxation of the hip and subsequent acetabular dysplasia were experimentally produced by fixing the knee of young rabbits in extension position. Changes of the acetabulum during the repair process after reduction were investigated radiologically, histologically and histomorphometrically. Radiologically, subluxation and acetabular dysplasia appeared two weeks after fixation and disappeared as the knee became flexible after removal of fixation. Histologically, abnormalities of growth cartilage and hypertrophy of trabeculae in the secondary spongiosa were prominent two weeks after fixation. His
