Integrating epidemiology and toxicology in neurotoxicity risk assessment.

BACKGROUND: The liver is the second most commonly injured intra-abdominal organ in children. CT scanning is currently regarded as the "gold standard" in screening for intra-abdominal injury following blunt trauma. However, the risks associated with performing CT in children are not insignificant and, in addition, CT is not always readily available. This study investigates the utility of alanine aminotransferase (ALT) in screening for liver injury in paediatric trauma. METHODS: Two groups of patients were compiled from a prospectively identified trauma registry-one with liver injuries and anot

A comparison of multiplicity adjustment strategies for correlated binary endpoints.

Several Bonferroni-type adjustments have been proposed to control for family wise type I error among multiple tests. However, many of the approaches disregard the correlation among endpoints. This can result in a conservative hypothesis testing strategy. The James procedure is an alternative approach that accounts for multiplicity among correlated continuous endpoints. Here a simulation study compares four Bonferroni-type alpha-adjustments (Bonferroni, Dunn-Sidák, Holm, and Hochberg) and the James p-value adjustment when used for multiple correlated binary variables. These procedures provided

Sensitivity analysis for the estimation of rates of change with non-ignorable drop-out: an application to a randomized clinical trial of the vitamin D3.

The vitamin D(3) trial was a repeated measures randomized clinical trial for secondary hyperparathyroidism in haemodialysis patients where the efficacy of the vitamin D(3) infusions for suppressing the secretion of parathyroid hormone (PTH) was compared among four dose groups over 12 weeks. In this trial, patients terminated the study before the scheduled end of the study due to their elevated serum calcium (Ca) level, that is, the administration of the vitamin D(3) was expected to cause hypercalcaemia as an adverse event. In this setting of monotone missingness, there is a potential for bias

Displaying clinical data relationships using scaled rectangle diagrams.

A method is presented to draw rectangles to represent categorical data relationships. The idea is an adaptation of a scaled Venn diagram. Rectangles are drawn with area proportional to the frequency of categories and the rectangles are positioned to overlap each other so that the areas of overlap are in proportion to the joint frequencies of the characteristics. The diagrams are especially useful to illustrate symptom co-occurrence.

Laplace Transform-Based Nonparametric Test of Exponentiality against DMRL class with preservation under the Homogeneous Poisson Shock Model and applications in survival analysis and reliability.

This paper introduces a novel, computationally efficient nonparametric test for assessing the null hypothesis of exponentiality against alternatives belonging to the Decreasing Mean Residual Life (DMRL) class. The test statistic is developed using Laplace transform techniques in conjunction with the theory of U-statistics, ensuring asymptotic normality and scale invariance. In addition, we establish the preservation of the proposed methodology under the Homogeneous Poisson Shock Model, further extending its theoretical robustness in reliability contexts. Critical values are obtained through ex

Using Off-Treatment Sequential Multiple Imputation for Binary Outcomes to Address Intercurrent Events Handled by a Treatment Policy Strategy.

The estimand framework proposes different strategies to address intercurrent events. The treatment policy strategy seems to be the most favoured as it is closely aligned with the pre-addendum intention-to-treat principle. All data for all patients should ideally be collected; however, in reality patients may withdraw from a study leading to missing data. This needs to be dealt with as part of the estimation. A common intercurrent event we focus on is treatment discontinuation. Several areas of research have been conducted exploring models to estimate the estimand when intercurrent events are h

A generalized difference-in-differences estimator for stepped-wedge cluster-randomized trials.

Staggered treatment adoption arises in the evaluation of policy impact and implementation in many settings, including both randomized stepped-wedge trials (SWTs) and non-randomized quasi-experiments with panel data. In both settings, getting an interpretable, unbiased effect estimate requires careful consideration of the target estimand and possible treatment effect heterogeneities. This paper proposes a novel non-parametric approach for interpretable estimation in SWTs. By constructing an estimator using weighted averages of two-by-two difference-in-differences comparisons as building blocks,

Semi-parametric sensitivity analysis for trials with irregular and informative assessment times.

Many trials are designed to collect outcomes at or around pre-specified times after randomization. If there is variability in the times when participants are actually assessed, this can pose a challenge to learning the effect of treatment, since not all participants have outcome assessments at the times of interest. Furthermore, observed outcome values may not be representative of all participants' outcomes at a given time. Methods have been developed that account for some types of such irregular and informative assessment times; however, since these methods rely on untestable assumptions, sen

A causal inference framework for leveraging external controls in hybrid trials.

We consider the challenges associated with causal inference in settings where data from a randomized trial are augmented with control data from an external source to improve efficiency in estimating the average treatment effect (ATE). This question is motivated by the SUNFISH trial, which investigated the effect of risdiplam on motor function in patients with spinal muscular atrophy. While the original analysis used only data generated by the trial, we explore an alternative analysis incorporating external controls from the placebo arm of a historical trial. We cast the setting into a formal c

Identifying typical trajectories in longitudinal data: modelling strategies and interpretations.

Individual-level longitudinal data on biological, behavioural, and social dimensions are becoming increasingly available. Typically, these data are analysed using mixed effects models, with the result summarised in terms of an average trajectory plus measures of the individual variations around this average. However, public health investigations would benefit from finer modelling of these individual variations which identify not just one average trajectory, but several typical trajectories. If evidence of heterogeneity in the development of these variables is found, the role played by temporal

The Outlier in All of Us: Why Implementing Pharmacogenomics Could Matter for Everyone.

The field of pharmacogenomics originally emerged in the 1950s from observations that a few rare individuals had unexpected, severe reactions to drugs. As recently as just 6 years ago, prominent views on the subject had largely remained unchanged, with authors from the US Food and Drug Administration (FDA) citing the purpose of pharmacogenetics as "tailoring treatment for the outliers." It should not be surprising if this is the prevailing view--the best-studied pharmacogenomic drug examples are indeed just that, genetic explanations of extreme responses or susceptibilities among usually a very

Two-stage hierarchical group testing for multiple infections with application to the infertility prevention project.

Screening for sexually transmitted diseases (STDs) has benefited greatly from the use of group testing (pooled testing) to lower costs. With the development of assays that detect multiple infections, screening practices now involve testing pools of individuals for multiple infections simultaneously. Building on the research for single infection group testing procedures, we examine the performance of group testing for multiple infections. Our work is motivated by chlamydia and gonorrhea testing for the infertility prevention project (IPP), a national program in the United States. We consider a

Group-based trajectory modeling: a novel approach to examining symptom trajectories in acute bipolar episodes.

BACKGROUND: Pattern analysis can aid understanding of trajectories of symptom evolution. However, most studies focus on relatively homogeneous disorders with a restricted range of outcomes, prescribed a limited number of classes of medication. We explored the utility of pattern analysis in defining short-term outcomes in a heterogeneous clinical sample with acute bipolar disorders. METHOD: In a naturalistic observational study, we used Group-based trajectory modeling (GBTM) to define trajectories of symptom change in 118 bipolar cases recruited during an acute DSM IV episode: major depression

A space-time conditional intensity model for invasive meningococcal disease occurrence.

A novel point process model continuous in space-time is proposed for quantifying the transmission dynamics of the two most common meningococcal antigenic sequence types observed in Germany 2002-2008. Modeling is based on the conditional intensity function (CIF), which is described by a superposition of additive and multiplicative components. As an epidemiological interesting finding, spread behavior was shown to depend on type in addition to age: basic reproduction numbers were 0.25 (95% CI 0.19-0.34) and 0.11 (95% CI 0.07-0.17) for types B:P1.7-2,4:F1-5 and C:P1.5,2:F3-3, respectively. Influence of phosphorus on Cu sorption kinetics: stirred flow chamber experiments.

A stirred flow reactor was used to study the influence of phosphorus on the adsorption and desorption kinetics of copper in two acid soils on granite and amphibolite. The presence of P was found to significantly increase Cu adsorption in both soils, albeit at different types of sites (mainly in slow adsorption sites in the soil on granite, and both in fast and slow adsorption sites in that on amphibolite). The increased Cu sorption at fast sites in the amphibolite soil was due to its high content in Fe oxyhydroxides, which bound P and released OH(-) as a result, thereby raising the pH and lead

Validity and utility of bipolar spectrum models.

The bipolar spectrum model suggests that several patient presentations not currently recognized by the DSM warrant consideration as part of a mood disorders continuum. These include hypomania or mania associated with antidepressants; manic symptoms which fall short of the current DSM threshold for hypomania; and depression attended by multiple non-manic markers that are associated with bipolar course. Evidence supporting the inclusion of these groups within the realm of bipolar disorder (BP) is examined. Several diagnostic tools for detecting and characterizing these patient groups are describ

A naturalistic evaluation of the relation between dietary restraint, the urge to binge, and actual binge eating: a clarification.

OBJECTIVE: The current study attempted to characterize the natural course of events linking dietary restraint to urges to binge and actual binge episodes. METHOD: Using preprogrammed, hand-held computers, 39 women with bulimia-spectrum eating disorders monitored ongoing eating episodes, dietary restraint, and binge cravings over a 7-29-day interval. RESULTS: Multilevel modeling analyses indicated that restraint was not systematically elevated before binging. However, elevated restraint preceded strong binge cravings. CONCLUSION: Results suggest that dietary restraint may contribute to binge

Statistical methods in G-protein-coupled receptor research.

In this chapter we provide an introduction to statistical methods appropriate in G-protein-coupled receptor research, including examples. Topics covered include the choice of appropriate averages and measures of dispersion to summarize data sets, and the choice of tests of significance, including t-tests and one- and two-way analysis of variance (ANOVA) plus posttests for normally distributed (Gaussian) data and their nonparametric equivalents. Techniques for transforming non-normally distributed data to more Gaussian distributions are discussed. Concepts of statistical power, errors, and the
