Diagnosis and classification of adult Still's disease.

The cornerstone of adult onset Still's disease is the triad of daily fever, arthritis and rash. This syndrome remains enigmatic and most often a disease of exclusion. There are both musculoskeletal as well as systemic features. More importantly, reactive hemophagocytic syndrome may occur in patients. In this review we attempt to place this syndrome in perspective, including data on geoepidemiology, clinical and laboratory features.

Recent advances in the study of somatic mosaicism and diseases other than cancer.

Somatic mosaicism is well appreciated as a cause of cancer and, possibly, aging. Somatic mosaicism as the cause of other diseases is becoming more appreciated. It is especially important in the causation of deforming diseases (e.g., Proteus syndrome; Sturge-Weber syndrome) which are not inherited because early developmental expression is lethal. It also known to make an important contribution to neurological, dermatological, hematological and other diseases (and probably all diseases but many in which it is harder to detect). There have been exciting recent advances in the detection of somatic

Pectus carinatum: the effects of orthotic bracing on pulmonary function and gradual compression on patient compliance.

OBJECTIVES: The treatment of pectus carinatum (PC) deformity has been considered to be operative. Some authors have shown that postoperative pulmonary function is worsened. They have suggested that compromised chest wall expansion secondary to surgery leads to compromised pulmonary function. Several authors have advocated an orthotic brace for the treatment of PC. Pulmonary functions after orthotic brace treatment have not been investigated. METHODS: Between April 2006 and October 2012, 61 patients presented with PC. Orthotic braces allowing gradual compression were prepared according to the

CT of the hips in the investigation of protrusio acetabuli in Marfan syndrome. A case control study.

OBJECTIVES: To establish the prevalence of protrusio acetabuli (PA) in adults fulfilling the Ghent criteria for Marfan syndrome (MFS), and in a normal adult population. METHODS: 105 adults with probable MFS and 107 controls were included. CT of the hips was obtained. A qualitative assessment of PA was performed. A new method for estimating the degree of PA was introduced with measurement of the parameter CWD (circle-wall distance). Results were compared to an alternative method based on MRI [1]. RESULTS: 87 of the study group fulfilled the Ghent criteria of MFS (Ghent positives), and 18 did

Complete sacralization of L5 vertebrae: traits, dimensions, and load bearing in the involved sacra.

BACKGROUND CONTEXT: Spines with sacralized fifth lumbar vertebrae are quite commonly encountered. Sacralizations represent a transitional state at lumbosacral junctions and are more susceptible to degenerative changes resulting from the altered load-bearing patterns at these regions. PURPOSE: The present study is an attempt to analyze the morphology of sacra with complete and partially fused (sacralized) L5 vertebrae and compare it with those observed in normal sacra. STUDY DESIGN/SETTING: Measurements of dimensions and articular surface areas were recorded in samples bearing sacralized L5 v

Swimming Anatomy and Lower Back Injuries in Competitive Swimmers: A Narrative Review.

CONTEXT: Competitive swimmers are at high risk of overuse musculoskeletal injuries due to their high training volumes. Spine injuries are the second most common musculoskeletal injury in swimmers and are often a result of the combination of improper technique, high loads on the spine in strokes that require hyperextension, and repetitive overuse leading to fatigue of the supporting trunk muscles. The purpose of this review is to summarize the current evidence regarding swimming biomechanics, stroke techniques, and common injuries in the lumbar spine to promote a discussion on the prevention an

Osteoarthritis management through medical reversal: Glucosamine, Alzheimer's disease risk, and long-term outcomes after arthroscopic partial meniscectomy.

The adoption of interventions in osteoarthritis and musculoskeletal care frequently precedes the availability of robust long-term evidence, creating the conditions for subsequent medical reversal. Recent findings raising concern about possible adverse neurocognitive associations with glucosamine use challenge the assumption that widely used supplements are necessarily biologically inert, although their causal significance and direct relevance to routine osteoarthritis management remain uncertain. In contrast, 10-year follow-up evidence after arthroscopic partial meniscectomy for degenerative m

An annotated heterogeneous ultrasound database.

Ultrasound is a primary diagnostic tool commonly used to evaluate internal body structures, including organs, blood vessels, the musculoskeletal system, and fetal development. Due to challenges such as operator dependence, noise, limited field of view, difficulty in imaging through bone and air, and variability across different systems, diagnosing abnormalities in ultrasound images is particularly challenging for less experienced clinicians. The development of artificial intelligence (AI) technology could assist in the diagnosis of ultrasound images. However, many databases are created using a

The Pediatric Screening-Priority Evaluation Dysphagia: Validation of a new tool for screening swallowing disorders in infants and children.

AIM: To assess the psychometric properties of the Pediatric Screening-Priority Evaluation Dysphagia (PS-PED), a novel 14-item, non-invasive tool for identifying dysphagia risk in infants and children. METHOD: Internal consistency and interrater reliability were evaluated using Cronbach's alpha and intraclass correlation coefficient (ICC). Concurrent validity was assessed by correlating PS-PED scores with the Penetration Aspiration Scale (PAS). Analysis of variance examined score differences across neurological and neuromuscular conditions, congenital and musculoskeletal abnormalities, cardiov…

Noise-robust markerless video gait anomaly detection via two-stage acquisition and LSTM autoencoders.

We propose a markerless gait anomaly detection system for orthopedic screening. The framework combines MediaPipe-based joint tracking, unsupervised LSTM-autoencoder modeling, and targeted preprocessing to address clinical video noise. Trained only on normal gait, the model detects abnormal patterns in sarcopenia (SA) and Parkinson's disease (PD) patients, achieving detection rates of 97% and 88%, respectively. Our method achieves state-of-the-art performance in sarcopenia detection, surpassing recent sensor-based approaches that require wearable devices or handcrafted features. Furthermore, to

Long COVID: current research and future directions.

Long coronavirus disease (COVID) is defined as the continuation or development of new symptoms three months after the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, and that last for at least two months, with no other explanation for their cause. This disease includes various clinical manifestations that affect multiple organ systems, such as complications in respiratory, cardiovascular, neurological, and musculoskeletal systems. The most commonly reported symptoms include fatigue, cognitive dysfunction, dyspnea, and chest pain; however, the prevalence and severity of

Variants in the WDR44 WD40-repeat domain cause a spectrum of ciliopathy by impairing ciliogenesis initiation.

WDR44 prevents ciliogenesis initiation by regulating RAB11-dependent vesicle trafficking. Here, we describe male patients with missense and nonsense variants within the WD40 repeats (WDR) of WDR44, an X-linked gene product, who display ciliopathy-related developmental phenotypes that we can model in zebrafish. The patient phenotypic spectrum includes developmental delay/intellectual disability, hypotonia, distinct craniofacial features and variable presence of brain, renal, cardiac and musculoskeletal abnormalities. We demonstrate that WDR44 variants associated with more severe disease impair …

Calcineurin inhibitor-induced pain syndrome (CIPS) affects the hips in a renal transplant recipient: A case report.

Calcineurin inhibitor-induced pain syndrome is a rare but debilitating complication of organ transplantation. This case report describes a man in his forties who developed bilateral hip pain, an atypical presentation of calcineurin inhibitor-induced pain syndrome, after undergoing renal transplantation. Initially, avascular necrosis was suspected as a potential cause of pain. The initial radiographs revealed no abnormalities. However, high trough levels of calcineurins and subsequent magnetic resonance imaging of the hip revealed bilateral symmetric bone marrow edema, which was consistent with

Cerebral Palsy: A Narrative Review on Childhood Disorder.

Cerebral palsy, one of the most common reasons for infirmity in children and young people in developed countries, refers to several neurological diseases that impact movement and coordination. Central nervous system damage received during the first stages of brain development can cause cerebral palsy, a non-progressive condition that manifests as impairments of movement and posture. Two cases per 1000 are reported, and the causes include those mentioned for high-risk infants. Mental retardation, sensory deficiencies, failure to thrive, seizures, and behavioral or emotional issues are some of t

Two children with hypophosphatasia with a heterozygous c.1559delT variant in the ALPL gene, the most common variant in Japanese populations.

Hypophosphatasia (HPP), a genetic disorder characterized by decreased tissue-nonspecific alkaline phosphatase (TNSALP) activity, is caused by loss-of-function mutations in the ALPL gene, which encodes TNSALP. The most frequent pathogenic variant in Japanese patients with HPP is a frameshift mutation in the ALPL gene, c.1559delT, and its carrier frequency is reported to be one in 480 in the Japanese population. We report the cases of two Japanese children with HPP who had a heterozygous c.1559delT variant in the ALPL gene. One case (involving a neonate) exhibited respiratory insufficiency assoc

Approach to Hypophosphatemic Rickets.

Hypophosphatemic rickets typically presents in infancy or early childhood with skeletal deformities and growth plate abnormalities. The most common causes are genetic (such as X-linked hypophosphatemia), and these typically will result in lifelong hypophosphatemia and osteomalacia. Knowledge of phosphate metabolism, including the effects of fibroblast growth factor 23 (FGF23) (an osteocyte produced hormone that downregulates renal phosphate reabsorption and 1,25-dihydroxyvitamin-D (1,25(OH)2D) production), is critical to determining the underlying genetic or acquired causes of hypophosphatemia

Orthopaedic manifestations of neurofibromatosis type 1: A case report.

Neurofibromatosis type 1 (NF1) or von Recklinghausen disease is one of the most common autosomal dominant genetic diseases. It is characterized by 'café-au-lait' spots and multiple tumors starting from the central and peripheric nervous system. The diagnosis is determined on two out of seven criteria: i) A total of 6 or more light brown spots larger than 5 mm in diameter (pre-puberty) or 15 mm in diameter (post-puberty); ii) a total of 2 or more neurofibromas or one plexiform neurofibroma; iii) axillary or inguinal freckling; iv) optic glioma; v) a total of 2 or more Lisch nodules; vi) bone ab
