Latin American hemorrhagic fever.

Several viral and bacterial agents are responsible for hemorrhagic fever in Latin America, but there are three agents that are only present in this region: (1) the Junin virus, (2) the Machupo virus, and (3) the Guanaritovirus, members of the Tacaribe complex of the family Arenaviridae and responsible for the Argentinian, Bolivian, and Venezuelan hemorrhagic fever. The clinical manifestations of hemorrhagic fever are similar for the three viruses, mainly myalgia, arthralgia, headache, relative bradycardia, conjunctivitis, nausea, vomiting, diarrhea, and hemorrhagic disorders. In this article, …

Trichinosis with severe myopathic involvement mimicking polymyositis. Report of a family outbreak.

We describe an outbreak of trichinosis in 3 members of a rural family. In the 3 patients eating raw pork was the source of infection. They presented with myalgias and severe proximal muscle weakness mimicking polymyositis. The diagnosis was made by demonstration of larvae of Trichinella spiralis in the muscle biopsy and also by the presence of anti-Trichinella antibodies detected by double immunodiffusion in their sera. We call attention to the unusual clinical presentation of trichinosis in our patients that was manifested by severe muscle weakness that may be confused with polymyositis.

Misdiagnoses: a timely reminder.

This case report of an occult infection masquerading as palindromic arthritis was misdiagnosed for almost 2 years. The episodic bouts of erysipelas were not obvious, and it was even suggested that the silicone implanted 7 years previously could possibly be inciting an autoimmune disease. Approximately 2 years after the onset of the recurrent episodes of fever, myalgias, and arthralgias, coagulase-negative staphylococci were found residing in the valve stems of bilumen breast implants, which were removed. The symptom complex disappeared and has not recurred.

Allopurinol hypersensitivity syndrome as a cause of hepatic fibrin-ring granulomas.

Hepatic fibrin-ring granulomas were found in a 35-yr-old man who developed fever, myalgias, rash, eosinophilia, and abnormal liver function tests 4 wk after the beginning of allopurinol treatment. All clinical and biochemical abnormalities spontaneously resolved within 6 wk after cessation of therapy. There was no evidence for Q fever or Hodgkin's disease, which are the recognized causes of hepatic fibrin-ring granulomas. It is suggested that allopurinol hypersensitivity might be an additional cause of these peculiar granulomas.

A possible association between influenza vaccination and small-vessel vasculitis.

We describe two patients who experienced small-vessel vasculitic syndromes after influenza immunization. Their illnesses were characterized by fever, arthralgias, and myalgias; uveitis and optic neuritis occurred in one patient, while the other had palpable purpura, which histologically was proved to be a cutaneous necrotizing venulitis. No viral or bacterial infection, medications, or underlying systemic rheumatic disease could be implicated. To our knowledge, small-vessel vasculitis occurring after influenza vaccination has not been previously reported.

Acute Myocarditis Following Dupilumab Initiation.

A 48-year-old male patient developed an acute rash, fever, myalgia, abdominal pain, tachycardia, near syncope, and chest pain after receiving one dose of dupilumab for chronic rhinosinusitis with nasal polyps (CRSwNP). He presented with hypotension, elevated troponin, hypereosinophilia, ST elevation with T wave abnormalities, and a mildly dilated left ventricle with severely reduced systolic function. He was diagnosed with acute myocarditis. He was treated with high-dose steroids, which rapidly alleviated his symptoms and led to a substantial improvement in his heart function.

Case Report: 18 F-FDG PET/CT for FUO Due to Whipple Disease Infection.

We present the case of a 60-year-old man admitted to the hospital presenting with high FUO (fever of unknown origin), strong headache, face erythematous-desquamative cutaneous lesions, long history of abdominal pain, and diffuse myalgia. He was also previously treated with immunosuppressants and currently managed with corticosteroids for a seronegative rheumatic disease. Given the immunocompromised state, an infective etiology was suspected. The patient was referred, as part of standard clinical practice, to perform an 18 F-FDG PET/CT to identify potential causes of FUO.

[A common dilemma in medicine : fortuitous association or causal relationship ?].

Making the differential diagnosis between a simple fortuitous association and a true causal relationship is a challenge commonly encountered not only in epidemiology, but also in clinical practice. The nine criteria supporting a causal relationship published by Bradford-Hill in 1965 remain relevant, even if some amendments were proposed in recent years. The present clinical scenario illustrates this problem by analyzing the controversies concerning the relationships between hypercholesterolemia (LDL) and coronary heart disease on the one hand and between statin therapy and myalgia on the other…

Residual and Late Onset Symptoms Appeared in a Patient with Severe Fever with Thrombocytopenia in a Convalescence Stage.

Severe fever with thrombocytopenia syndrome (SFTS) is an emerging tick-borne infectious disease caused by Dabie bandavirus (formerly SFTS virus, SFTSV). Its manifestations during the convalescent phase have not been widely described. We report a patient presenting with hematospermia, fatigue, myalgia, alopecia, insomnia, and depression during the recovery phase of SFTS. Since these symptoms are widely observed in patients with viral hemorrhagic fevers, there might be common mechanisms between SFTS and other viral hemorrhagic fevers. Close monitoring may be required during the recovery phase of…

Toxocariasis Presenting with Hypereosinophilia and Lymphadenopathy: A Case Report.

Human toxocariasis is a parasitic disease transmitted to humans by ingestion of Toxocara spp. larvae. Rural area residents and children have higher risk for this parasitic infection. The disease is frequently asymptomatic; however, it may cause symptoms such as cough, fever, headache, myalgia or other conditions such as lymphadenopathy and granulomatous hepatitis. Albendazole is used for the treatment of toxocariasis diagnosed by serologic and immunological methods. Herein, we report two cases of toxocariasis that were investigated due to hypereosinophilia and lymphadenopathy for a long time.

Pneumonia complicating Shigella sonnei dysentery in an HIV-infected adult male.

An HIV-1-infected adult presented with a short history of dyspnoea, productive cough and myalgia with fever. Shortly after presentation, he developed the abrupt onset of high-volume watery diarrhoea: stool culture grew Shigella sonnei. At the same time, he became hypoxaemic, and thoracic imaging showed bilateral lower lobe consolidation/collapse. Culture of sputum and blood was negative. The patient recovered with fluid resuscitation and ciprofloxacin monotherapy. This is the first reported case of pneumonia complicating S. sonnei dysentery in an HIV-infected adult.

Muscle phosphoglycerate mutase deficiency: a study of a family with metabolic myopathy.

Muscle phosphoglycerate mutase deficiency results in a myopathic condition characterised by repeated cramps, possible myoglobinuria and muscle pain. We present a family with muscle phosphoglycerate mutase (PGaM) deficiency, mild glycogen storage and some exertional myalgia. Investigations of muscle biopsy tissue showed a residual PGaM activity in muscle due to a small amount of brain isoenzyme. The enzyme was also measured in blood cells and a full investigation of muscle metabolism was also carried out. Electron microscopy revealed mitochondria with similar morphological features in two patie…

Toxic shock syndrome following diagnostic peritoneal lavage.

We report the case of a 15-year-old girl who developed high fever, syncope, abdominal pain, nausea and vomiting, myalgia, pharyngitis, and a desquamating rash eight days after a diagnostic peritoneal lavage. The diagnostic peritoneal lavage wound was erythematous and tender. Incision of the site yielded 10 mL of exudate that cultured Staphylococcus aureus. The patient was treated with a first-generation cephalosporin and recovered without sequelae. To our knowledge, this is the first reported case of toxic shock syndrome following diagnostic peritoneal lavage.

Unilateral seizures in a patient with hairy cell leukemia treated with interferon.

A patient is described who developed unilateral seizures whilst being treated with recombinant interferon for hairy cell leukemia. Special features included the relatively low dose of interferon, the focal aspect of the epilepsy and the high resistance to anticonvulsants. Oligoclonal banding of cerebrospinal fluid proteins may have resulted from polyclonal activation of bone marrow plasma cells during interferon treatment. Disturbances of consciousness, dysphasia, visual hallucinations, upper motor neuron deficit, tremor, dizziness, numbness, myalgia and headache, all of them neurological comp…

Cotton-wool spots as a sign in leptospirosis (Weil's disease).

A 44-year-old black male presented with fever, myalgia and weakness. He had elevated blood urea nitrogen, creatine phosphokinase and serum glutamic-oxaloacetic transaminase. During the first 6 days of this undiagnosed illness azotemia increased, a pericardial friction rub occurred, and hematuria was present. On the 7th day bilateral subconjunctival hemorrhages, anterior uveitis, and peripapillary cotton-wool spots were noted. This combination of findings suggested leptospirosis, which was subsequently confirmed by specific antibody titers. Therapeutic response was achieved with high-dose syste…

Comparison between the safety of the HPV vaccine versus placebo: a systematic review and meta-analysis of randomized clinical trials.

OBJECTIVE: Human Papillomavirus (HPV) is a virus that targets epithelial tissues. Virtually all cases of cervical cancer are related to HPV, … results indicated that HPV vaccination was associated with increased local reactions, fatigue, and myalgia compared to the placebo. However, there were no significant differences in serious adverse events, gastrointestinal reactions, cutaneous effects, headache, or fever between the vaccine and placebo groups. CONCLUSION: Local reactions, fatigue, and m …

Risk and Health Factors for Temporomandibular Disorders Following Radiotherapy for Head and Neck Cancer.

BACKGROUND: Temporomandibular disorders (TMD) are multifactorial and complex musculoskeletal conditions frequently associated with pain or d … herapy. Potential predictors were analysed using linear and logistic regression analyses. RESULTS: Myalgia was associated with TMD diagnosis at baseline, and jaw exercise lowered the risk of developing myalgia at the follow-ups. The degree of pain at baseline and jaw exercise were associated with the changes in pain at follow-ups. Jaw exercise was associ …

Atypical Presentation of Babesiosis With Neurological Manifestations as Well as Hematological Manifestations.

Babesiosis is an infectious disease that is typically known to present with fevers, chills, and myalgias; and less commonly with anorexia, headache, nausea, and vomiting. The least common are shortness of breath, sore throat, neck stiffness, emotional lability, photophobia, and dark urine. Even more unusual are severe neurologic manifestations like altered mental status, motor deficits, and ataxia. We present two cases of patients, both in their seventies, with multiple comorbidities, who were admitted with similar symptoms of confusion/cognitive impairment, slurred speech, ataxia, fever, myal…
