We report a case of sarcomatoid carcinoma of the skin in a 63-year-old man who was treated with the carbon dioxide snow freezing method for a huge congenital pigmented nevus that extended from the right upper extremity to the right trunk during childhood. He had an exophytic red tumor on the nevus in the right upper extremity that grew slowly for 4 years and rapidly recently. Histological and immunohistochemical studies revealed both epithelial and mesenchymal malignancy in the same tumor. Basal cell carcinoma is the most common malignant tumor of the skin, and it develops most frequently on the head and neck regions. It is well-recognised that recurrent disease can occur following surgery for malignancy in the head and neck region. This is particularly true of basal cell carcinoma in which recurrences may occur over many years and despite the use of different treatment modalities. Reconstruction of large defects may become increasingly difficult and can be optimally managed by free tissue transfer. The authors report a case of basal cell carcinoma that has required treatment for over 20 years, unique in that on five different occasions, free flaps have been used for reconstruction.

The Bazex-Dupré-Christol syndrome (BDC syndrome) was first described in 1964. Only 14 affected families and 47 patients with this very rare dominant genodermatosis have been reported so far. The three main features of BDC syndrome are (1) follicular atrophoderma, especially on the dorsum of hands and feet, (2) multiple basal cell carcinomas of the face, occurring mainly during the second decade of life, reflecting the clinical relevance of BDC syndrome and (3) congenital generalized hypotrichosis, sometimes with pili torti and trichorrhexis nodosa. In order to complete the classical descriptions of astrocytic tumors, I report here ultrastructural findings regarding this group of neoplasms. Fibrillary astrocytomas consisted of a mixture of cell bodies and their processes. Nuclei were indented and contained various types of nuclear bodies or filamentous paracrystalline inclusions. BACKGROUND AND DESIGN: Forty-eight human immunodeficiency virus-infected patients with nonmelanoma skin cancers seen during a four-year period were evaluated in a retrospective, case-control study. Patients were followed up after therapy and recurrence rates were determined.
RESULTS: One hundred and sixteen nonmelanoma skin cancers were identified, 101 of which were basal cell carcinomas (87%), mostly superficial multicentric (67%) of the trunk (62%). PDT has been shown to be of value in inoperable basal and squamous skin cancers and in cutaneous metastases. Azone, a new investigative vehicle for HpD, is of value more for intralesional injections than for topical applications in tumors, except perhaps for superficial mucous membrane lesions. The more flexible gold head vapor is of definite value in the PDT program. For test models for PDT studies in dermatology and plastic surgery with HpD and other fluorochromes, single thickened resistant plaques of psoriasis and the common baso squamous acanthoma (seborrheic warty growth) have been used.
