Application of angiotensin II to healthy rat sciatic nerve can produce neuropathy without associated vasculopathy.

Elevated angiotensin II (AII) levels have been associated with hypertension, diabetes, and polyneuropathy. It is unknown whether AII applied to healthy nerve can be used to model a confined neuronal injury by producing localized vasculopathy and associated neuropathy. In this study, angiotensin II (2.2 μg/ml) or saline was infused constantly via osmotic pump onto the sciatic nerve of 20 rats for 12 weeks. Nerve conduction studies were repeated every 4 weeks, and sciatic nerve was collected for pathological analysis at 12 weeks. Animals infused with AII showed a significant decrease in nerve fi

Myotonic dystrophy mimicking postpolio syndrome in a polio survivor.

We describe a 38-yr-old polio survivor with newly developed weakness from myotonic dystrophy. He suffered muscle atrophy and weakness in his legs as a result of poliomyelitis at the age of 3 yrs. After a stable interval of about 30 yrs, he felt new weakness and fatigue in his legs. Electromyography revealed generalized myotonic discharges, early recruitment, and findings of chronic denervation in his left leg. Genetic testing was consistent with myotonic dystrophy type 1. A biopsy from the right gastrocnemius revealed findings of both myotonic dystrophy and chronic denervation. This case repor

Machado-Joseph disease presenting as motor neuron disease.

Machado-Joseph disease (MJD) is a spinocerebellar degeneration with a wide phenotypic presentation. A 64-y-old male with a history of gait disability and fasciculations was referred to our unit with the diagnosis of motor neuron disease (MND), which was supported by the presence of upper motor neuron signs and diffuse loss of motor units on electromyography. Trunk and cranial-innervated muscles were clearly affected. Respiratory function was mildly affected. Cerebellar ataxia developed over time and the diagnosis of MJD was confirmed by genetic studies. MJD should be considered in the differen

Facial onset sensory and motor neuronopathy (FOSMN) syndrome responding to immunotherapies.

We report the first non-Caucasian case of facial onset sensory and motor neuronopathy (FOSMN) syndrome partially responding to various immunotherapies. A 55-year-old man had first felt paresthesia on his right cheek at age 45. This gradually extended to the scalp. Paresthesia of bilateral fingers and dysphagia appeared 6 years later. On admission, facial sensory impairment and bulbar palsy were found. There were no sensory or motor deficits evident in any limb, except for decreased deep tendon reflex and vibratory sensation. Videofluorography (VF) revealed decreased pharyngeal clearance. The s

[Facial diplegia and acute inflammatory demyelinating neuropathy secondary to varicella].

INTRODUCTION: Primo-infection by varicella-zoster virus (VZV) may be associated with several neurologic complications. Bilateral facial palsy is a rather uncommon manifestation. CASE REPORT: We report the case of a 38-year-old woman who developed bilateral facial diplegia and paresthesia affecting all four limbs with subacute onset several days after varicella virus primoinfection. Ancillary tests showed hyperproteinorachia and signs of demyelinating polyneuropathy in nerve conduction tests. The diagnosis of Guillain-Barré syndrome was retained and a treatment with intravenous immunoglobuline

Triple-stimulation technique in multifocal neuropathy with conduction block.

In patients with multifocal neuropathy with conduction block (CB), CBs located between the root and Erb's point are not detected in nerve conduction studies. We therefore examined whether the triple-stimulation technique (TST) might provide a useful means of detecting CB proximal to Erb's point. Clinical assessments, extensive nerve conduction studies (NCS), conventional transcranial magnetic stimulation, and TST were performed on 10 patients with multifocal motor neuropathy with CB (MMNCB) and 6 patients with Lewis-Sumner syndrome. Conduction blocks located proximal to Erb's point were detect

Acute cervical motor radiculopathy induced by neck and limb immobilization in a patient with Parkinson disease.

A 68-year-old woman with Parkinson disease (PD) presented with acute monoplegia of her left upper extremity after the neck and limb immobilization for several hours. Her sensory function was normal, and the chest X-ray showed left phrenic nerve palsy. Electrophysiological studies showed multi-segment muscle involvement (C3 to T1) including denervation potentials and reduced interference of motor units in needle electromyography. M wave amplitude in peripheral nerve stimulation was preserved except for the ulnar nerve, suggesting both axonal injury and conduction block at the anterior spinal ro

The effect of infrared laser on sensory radial nerve electrophysiological parameters.

OBJECTS: It has been claimed that laser may have bio-stimulation effect on the nerve tissues. This study has been designed to investigate the effect of different doses of infra-red (IR) laser exposure on the electrophysiological parameters of sensory nerves. METHODS AND SUBJECTS: Forty healthy subjects (20-35 years old) with no history of neurological conditions participated in this study. IR laser (780 nm, 20 mw) was applied over five blocks (1 cm2 each and 0.5 J/cm2) of 5 cm length of the left and right superficial radial nerve. The IR laser radiation was started from proximal to distal in

Inappropriate secretion of antidiuretic hormone in a patient with chronic inflammatory demyelinating polyneuropathy.

A 54-year-old man suffered from a relapse of chronic inflammatory demyelinating polyneuropathy (CIDP), and developed quadriplegia and somnolence requiring mechanical ventilation for respiratory failure. Serum Na concentration remained at low levels during the clinical course, and a diagnosis of inappropriate secretion of antidiuretic hormone (SIADH) was made. The present case had not only acute aggravation of CIDP with autonomic dysfunction but also intracranial hypertension caused by increased CSF protein (maximum level, 1,315 mg/dl). It seemed likely that injury of the afferent fibers of the

Determining the sensitivity and specificity of common diagnostic tests for carpal tunnel syndrome using latent class analysis.

BACKGROUND: The accuracy of a diagnostic test used to classify a patient as having disease or being disease-free is a valuable piece of information to be used by the physician when making treatment decisions. If a standard reference test is available, determining the sensitivity and specificity of a new test is straightforward. However, if that reference test is incorrectly assumed to be perfectly sensitive and specific, the errors of the reference test can result in an underestimation of the accuracy of the test being evaluated. Latent class analysis can be applied to determine the sensitivit

Avoidance of direct injury to the peripheral nerve with maxacalcitol (22oxa-1,25(OH)2D3) in rats.

BACKGROUND: Secondary hyperparathyroidism is one of the most common complications in patients with long-term end-stage renal disease. A recent report has stated that high doses of vitamin D derivatives injected directly into the parathyroid glands can reduce serum parathyroid hormone levels and suppress further enlargement of the parathyroid glands without side-effects. Maxacalcitol (22oxa-1,25(OH)(2)D(3); OCT) is a new vitamin D derivative that can be administered intravenously, and is available in Japan. Direct injection of OCT into enlarged parathyroid glands is a promising new treatment fo

The sural sensory nerve is usually spared in Guillain-Barre syndrome.

Sensory nerve conduction (NC) studies that were performed on patients with Guillain-Barre syndrome (GBS) were reviewed. These were compared with the sensory NC findings in patients with peripheral neuropathy of the axonal-loss type (axonopathies) and chronic inflammatory demyelinating polyneuropathy (CIDP). In GBS, the sural sensory nerve frequently showed normal NC findings compared to the median and ulnar sensory nerves. The reverse pattern was seen in axonopathies and CIDP, in which the sural nerve showed more abnormalities than the median and ulnar nerves. This pattern of sensory NC abnorm

Objective evaluation of sensory function in patients with carpal tunnel syndrome using the current perception threshold.

The purpose of this study was to determine the sensitivity and specificity for the current perception threshold (CPT) test during clinical examination of idiopathic carpal tunnel syndrome (CTS). We studied 71 patients (102 hands) with CTS; 50 healthy individuals (100 hands) served as a control group. Using a neurometer, we investigated standard CPT values to obtain the sensitivity and specificity of the CPT test in the clinical examination of CTS. In the CTS group, cases that had abnormal values in the range analysis, the ratio analysis, or both were called "true-positive CPTs"; in the control

Bilateral hypoglossal nerve involvement in chronic inflammatory demyelinating polyneuropathy.

BACKGROUND: Virtual reality (VR) is showing increasing promise for assessing, understanding, and treating mental health difficulties. Virtual humans (VHs) represent a key aspect within many VR mental health applications. While VHs can play diverse roles and display varied characteristics, their design and influence have rarely been the primary focus of mental health research. OBJECTIVE: We aimed to carry out a systematic review of how VHs in immersive VR have been used in applications for mental health, focusing on their roles and interaction types, and the human characteristics being tested.

Bronchopulmonary afferent nerves.

Vagal afferent nerves are the primary communication pathways between the bronchopulmonary system and the central nervous system. Input from airway afferent nerves to the CNS is integrated in the brainstem and ultimately leads to sensations and various reflex outputs. Afferent nerves innervating the airways can be classified into various distinct phenotypes. However, there is no single classification scheme that takes all features, including conduction velocity, cell body diameter, ganglionic origin, and stimuli to which they respond (modality) into account. At present, bronchopulmonary afferen

Worsening of multifocal motor neuropathy during pregnancy.

Three women with multifocal motor neuropathy (MMN) were treated during pregnancy. Compared with their pregestation strength, the women became weaker in previously involved muscles and showed new weakness in previously unaffected muscles. All were treated with IV immunoglobulin during pregnancy and improved in strength. After pregnancy, strength in all patients returned to the prepregnancy state. The authors conclude that pregnancy may worsen MMN.

Reinnervation of denervated muscle in a split-nerve transfer model.

This study was performed to quantify the reinnervation of denervated muscle in a split-nerve transfer model and to determine any possible downgrading effects on the donor nerve and its end organ. Fifty-four adult Wistar rats weighing 200 to 250 g were used. The experimental design consisted of two groups. The motor nerve branch to the anterior tibial muscle and gastrocnemius muscle of the right hind limb were dissected in all rats. In the experimental group (N = 36), the motor nerve branch of the tibial nerve to the gastrocnemius muscle was exposed, cut, and ligated. The motor nerve branch to
