SIRA+P: Development and Testing.

OBJECTIVES: To describe the development of a new skin risk assessment scale called Skin Injury Risk Assessment and Prevention (SIRA+P) and to establish initial reliability and validity of the scale among patients ranging in age from birth, including pre-term, to adulthood, regardless of age or acuity of illness. STUDY DESIGN: The single-site study was a retrospective chart review to evaluate the measurement properties of SIRA+P. Charts of 385 patients of all ages and in all units (including the neonatal and pediatric intensive care units) of a free-standing children's hospital were included.

Born too small: who survives in the public hospitals in Lilongwe, Malawi?

OBJECTIVE: Malawi has the highest estimated preterm birth rate in the world. The survival rate of these babies is not known. The main objective of this study was to demonstrate the short-term survival of infants with birth weight below 2500 g nursed in Bwaila Hospital, a district hospital, and the tertiary level Kamuzu Central Hospital (KCH) in Lilongwe. The specific objectives were to calculate birth weight specific survival rates, compare the two hospitals regarding the chances of survival and review the use of antenatal corticosteroids. DESIGN: 1496 babies were included prospectively in th

Reporting an outbreak of Candida pelliculosa fungemia in a neonatal intensive care unit.

BACKGROUND: Fungemia in preterm infants is associated with high mortality and morbidity. This study reports an outbreak of unusual fungemia in a tertiary neonatal intensive care unit (NICU). METHODS: Ten Candida pelliculosa bloodstream isolates were identified from six infants hospitalized in the NICU from February to March 2009. Environmental study was performed, and genetic relatedness among the 10 clinical isolates of C pelliculosa and six control C pelliculosa strains was characterized by randomly amplified polymorphic DNA assay. In vitro susceptibility of isolates to six antifungal agent

Why do mothers still sun their infants?

OBJECTIVE: To determine the prevalence of maternal beliefs about the therapeutic uses of sunlight in infancy in tropical Australia. METHODOLOGY: Data were collected by interviewing 114 post-partum patients in Townsville (19 degrees 16'S), Queensland. Each woman was asked a series of open-ended and set-response questions about ancestry, pigmentation, residential history, parity, maternal and paternal education, and beliefs regarding the reputed therapeutic uses of sunlight. RESULTS: Half of the women had at least one risky belief about the perceived benefits of sunning their baby. Thirty-six

Illness severity measured by CRIB score: a product of changes in perinatal care?

AIM: To determine the perinatal factors associated with initial illness severity (measured by the CRIB (clinical risk index for babies) score) and its relation to survival to discharge. METHODS: A retrospective study was made of intensive care nursing records on 380 inborn babies, of less than 31 weeks gestation or 1501 g birthweight, admitted to one unit between 1984-6 and 1991-4. RESULTS: Between the two time periods mean initial illness severity score increased significantly from 2.8 to 3.9. This was the result of an increase in the maximum appropriate inspired oxygen concentration in the

Congenital adrenal hyperplasia: a potential diagnosis for the neonate in shock.

Congenital Adrenal Hyperplasia (C.A.H.) is an autosomal recessive disorder which is often life threatening during the neonatal period prior to establishment of the diagnosis and instigation of appropriate treatment. In females the condition is usually detected at birth due to genital ambiguity. Unfortunately males or extremely virilized females often remain undetected until they suffer a potentially fatal adrenal crisis. Typically, a crisis occurs within the first couple of weeks of life and is preceded by a history of failure to thrive, lethargy and vomiting which may be misdiagnosed as resul

Development and internal validation of an interpretable machine learning model using first-24-h postpartum nursing variables to predict delayed lactogenesis II after cesarean delivery.

BACKGROUND: Delayed lactogenesis II is a common early postpartum breastfeeding problem after cesarean delivery and may interfere with the establishment of exclusive breastfeeding. Although demographic and obstetric risk factors have been widely studied, the predictive value of routinely recorded early postpartum nursing process variables remains insufficiently explored. This study aimed to develop and internally validate a dynamic 24-h postpartum landmark interpretable machine learning model using clinical, perioperative, neonatal, and first-24-h nursing process variables to predict delayed la

Establishing and Optimizing Kangaroo Mother Care Practices Among Very-Low-Birth-Weight Neonates in a Tertiary Neonatal Intensive Care Unit (NICU): A Quality Improvement Approach.

Background Kangaroo Mother Care (KMC) is an evidence-based practice that improves outcomes in very-low-birth-weight (VLBW) neonates by lowering neonatal mortality, hypothermia, sepsis, and hospital stay duration while enhancing breastfeeding, physiological stability, and neurodevelopmental outcomes. Despite recommendations for initiating KMC within the early postnatal period (within the first 24-72 hours of life) and maintaining prolonged (six to eight hours/day) daily skin-to-skin contact, implementation and continuation of KMC remain inconsistent in neonatal intensive care units (NICUs). Thi

The effect of family integrated care based on Swanson theory of caring on premature infants: A non-randomized cohort study.

Preterm birth significantly impacts neonatal growth and development, and the family integrated care (FICare) nursing model offers several benefits. This study investigated the impact of FICare on preterm infants, based on Swanson caring theory. This theory includes 5 core processes (knowing, being with, doing for, enabling, and maintaining belief) that guide person-centered care. A non-randomized cohort study was conducted. A total of 52 preterm infants (28-36 weeks of gestation) were enrolled, 33 in the intervention group (FICare nursing model based on Swanson caring theory) and 19 in the con

Healthcare worker's views and experiences of factors impacting confidence to provide perinatal bereavement support: a qualitative study in Malawi and Zimbabwe.

BACKGROUND: Worldwide, around 1.9 million babies are stillborn, and 2.5 million newborns die soon after birth annually, the overwhelming majority occur in low- and middle-income countries (LMICs). Perinatal death has devastating impacts for families and communities, but not all parents receive adequate respectful bereavement support. Healthcare workers in LMICs often report that bereavement support is challenging, but few studies have explored factors that impact their confidence to deliver optimal care. OBJECTIVE: To explore healthcare worker's views and experiences of factors impacting thei

Validation of the Turkish caring professional scale among mothers of hospitalized neonates: A psychometric study based on Swanson's theory of caring.

INTRODUCTION: The aim is to adapt the CPS into Turkish and evaluate its psychometric properties specifically among mothers of hospitalized neonates. METHODS: This cross-sectional study included 149 mothers at a university hospital NICU. Cross-cultural adaptation followed Beaton et al.'s five-stage framework. Psychometric evaluation included internal consistency (Cronbach's α and McDonald's ω), confirmatory factor analysis (CFA) using both maximum likelihood on Pearson correlations and unweighted least squares on polychoric correlations, convergent and discriminant validity, and criterion-rela

Investigating milk-derived extracellular vesicles as mediators of maternal stress and environmental intervention.

Parental communication signals are transmitted through nursing and critically shape neurodevelopmental trajectories. Mirroring some well characterized effects of gestational challenges in rodents, maternal immune activation (MIA) during the lactational period disrupts maternal physiology, decreases lipid content in milk, and is associated with adverse neurobehavioral outcomes in offspring. This occurs without MIA significantly affecting maternal care. While gestational MIA models are responsive to environmental interventions, which beneficially alter maternal milk composition and associated of

[Holistic approach of the care of the infant with hypoxic-ischaemic encephalopathy in Spain].

INTRODUCTION: There is not much information about the care of infants with hypoxic-ischaemic encephalopathy (HIE) treated with therapeutic hypothermia (TH) in Spain. This includes whether protocols are routinely used, the type of neuro-monitoring performed, and how information on the neurological prognosis is presented to families. The answers to these would allow to detect and implement areas of improvement. METHOD: A cross-sectional analysis was performed on the responses to structured questionnaires sent to all the Spanish neonatal units that were performing TH in June 2015. Questions were

Timing and Outcomes of an Indication-Only Use of Intravenous Cannulation During Spontaneous Labor.

INTRODUCTION: In the United States, most women presenting in spontaneous labor undergo intravenous (IV) cannulation on admission to hospital labor and birth units. There is limited evidence for this routine practice in pregnant women at low risk for adverse outcomes during labor or birth. METHODS: A retrospective, exploratory, descriptive study of an indication-only practice of IV cannulation on admission for women presenting in spontaneous labor and cared for by a nurse-midwife service was performed. Descriptive data included the timing of IV cannula placement (admission, during labor or pos

Detection of hyperbilirubinaemia in jaundiced full-term neonates by eye or by bilirubinometer?

UNLABELLED: The aim of this study was to compare predictions of hyperbilirubinaemia by eye, performed by trained physicians and nurses, with predictions obtained using two commercial bilirubinometers. Jaundice was assessed in 92 white and 48 non-white healthy full-term neonates using three non-invasive methods and by total serum bilirubin as the reference method. Clinical assessment of cephalocaudal progression of jaundice was carried out independently by a physician and by nurses. Simultaneously, the Minolta Airshields JM-102 was applied on the sternum, the BiliCheck on both the forehead and

Expression of lysostaphin in milk of transgenic mice affects the growth of neonates.

As an initial step towards enhancing mastitis resistance in dairy animals, we generated BLG-Lys transgenic mice that secrete lysostaphin, a potent antistaphylococcal protein, in their milk. In the current study, we continue our assessment of lysostaphin as a suitable antimicrobial protein for mastitis resistance and have investigated mammary gland development and function in three lines of transgenic mice. As the lines were propagated, there was a tendency for fewer BLG-Lys litters to survive to weaning (51% as compared to 90% for nontransgenic lines, p = 0.080). Nontransgenic pups fostered on

Care of the COVID-19 exposed complex newborn infant.

As we confront COVID-19, the global public health emergency of our times, new knowledge is emerging that, combined with information from prior epidemics, can provide insights on how to manage this threat in specific patient populations. Severe Acute Respiratory Syndrome (SARS) and Middle East Respiratory Syndrome (MERS), both caused by coronaviruses, caused serious respiratory illness in pregnant women that resulted in adverse perinatal outcomes. Thus far, COVID-19 appears to follow a mild course in the vast majority of pregnant women. A significant proportion of pregnant women appear to be as
