Breathing and balance: Clinical insights and management strategies of respiratory acid-base disorders.

Respiratory acidosis and alkalosis are two of the four main acid-base processes encountered in clinical practice. These two processes are tightly balanced by breathing. Identification and management of respiratory acid-base disorders are important for clinicians, particularly when caring for patients with underlying pulmonary and neurological conditions. Basic knowledge of respiratory function, alveolar gas exchange, and innate compensation concepts can provide clinicians with the tools to design safe and effective nutrition care plans. These concepts will also aid in monitoring response to me

A Case Study of Intractable Diarrhea Due to Neonatal Microvillous Inclusion Disease.

BACKGROUND: Microvillous inclusion disease (MVID) is one of the most severe congenital diarrhea disorders, caused by a genetic defect in enterocyte differentiation and polarization. CASE REPORT: We describe a neonate who presented with severe weight loss, hypernatremic dehydration and metabolic acidosis due to intractable diarrhea due to MVID, confirmed by electron microscopy. CONCLUSION: MVID can present with severe weight loss, hypernatremic dehydration and metabolic acidosis that is life threatening. The diagnosis is made by typical findings on light microscopy and electron microscope of

Home Parenteral Nutrition and Intravenous Fluid Errors Discovered Through Novel Clinical Practice of Reconciling Compounding Records: A Case Series.

Home parenteral nutrition (HPN) and home intravenous (IV) hydration are complex, high-risk life-sustaining therapies for children and adults with severe digestive disorders. HPN compounding errors have the potential to cause serious patient harm. Here we present a retrospective case series at an interdisciplinary pediatric HPN program that includes specialized pharmacists reconciling prescriptions against commercial compounding records. Seven HPN or IV hydration patients were affected by significant errors in anion balance, copper, potassium, sodium, and infusion volume. Outpatient pharmacists

Effects of twin gestation on maternal morbidity.

As the incidence of twin gestation increases, it is important to consider the maternal risks associated with carrying multiples. Compared with singleton gestation, there are increased risks to the mother during the antepartum, intrapartum, and postpartum periods. Certain pregnancy complications are more likely to occur during a twin gestation, including preeclampsia and other hypertensive disorders, antepartum hospitalization for preterm labor or abnormal bleeding, nutritional deficiencies, cesarean delivery, and postpartum hemorrhage. Women carrying twins may benefit from early education rega

Accidental pneumothorax from a nasogastric tube in a patient with severe hemineglect: a case report.

Nasogastric tubes are frequently used for nutrition of patients with neurologic diseases. We report an instance of inadvertant placement of a standard nasogastric tube into the left pleural space in a patient with right parietotemporal intracerebral hemorrhage and severe hemineglect on the left side. The 2 confirmatory maneuvers-aspiration of fluid and auscultating the abdomen on insufflating air-were false-positive. We conclude that only radiologic confirmation of the position of nasogastric tubes and the awareness of the associated dangers will help minimize the occurrence of such events in

Intravenous immunoglobulins as treatment of life threatening esophageal involvement in polymyositis and dermatomyositis.

Esophageal involvement is considered a major cause of morbidity and an indicator of poor prognosis in polymyositis (PM) and dermatomyositis (DM). We describe 3 patients with steroid resistant PM/DM with life threatening esophageal involvement, resulting in impossible oral feeding and enteral nutrition with a gastric tube. All patients had both dramatic and rapid improvement of all clinical manifestations after initiation of intravenous immunoglobulin (IVIG) therapy. Swallowing disorders completely disappeared after the second infusion of IVIG, which permitted normal oral feeding and ablation o

The adult respiratory distress syndrome.

The adult respiratory distress syndrome (ARDS) is an extreme form of noncardiogenic pulmonary edema associated with alveolar-capillary damage. Clinical features include acute respiratory distress, dyspnea and tachypnea, severe hypoxemia refractory to oxygen therapy, and diffuse bilateral pulmonary infiltrates. Any number of serious disorders can cause ARDS, but the processes leading to the alveolar permeability defect are not understood. Therefore, therapy remains nonspecific and supportive. Treatment includes positive end-expiratory pressure, careful fluid management, steroid therapy, and ade

Pathogenetic mechanisms of hereditary diabetes mellitus.

Novel laboratory techniques such as the determination of proinsulin, insulin, C-peptide and insulin receptors have allowed further subclassification of diabetes mellitus today as representing a symptom rather than a disease. Numerous pathogenetic mechanisms of diabetes have been identified, many of these being genetically determined and this may extend possibilities for genetic counselling. As a geneticist's view, the pathogenetic concept of diabetes presented here is mostly confined to genetic mechanisms, leaving aside other influences on the development of diabetes such as hormones, viruses,

Some biochemical aspects of chronic alcohol intoxication.

Pathways of ethanol transformation and its respective metabolic disorders in the cell have been considered. One should note the role of acetaldehyde in the ethanol toxic effect. Acetaldehyde produces a significant effect upon the metabolism of biogenic amines in brain tissue. Chronic alcohol intoxication at a certain stage is connected with a considerable reduction in the ratio of alcohol dehydrogenase:acetaldehyde dehydrogenase (this ratio determines the severity of clinical signs in intoxication). The inhibition of acetaldehyde dehydrogenase activity is an enzymological base of hallucination

[Nutritional psychiatry and psychosomatics in the DACH region : Clinical treatment concepts and research at the university hospitals of Graz, Berlin and Zurich].

BACKGROUND: Individuals with mental disorders have an increased risk for metabolic disturbances, inflammation and nutrient deficiencies. Nevertheless, nutritional aspects have so far received little attention in psychiatric care. Nutritional psychiatry and nutritional psychosomatics (NP) combine psychiatry with nutritional and lifestyle medicine to systematically integrate the interaction of these domains into the diagnostics and therapy. OBJECTIVE: Illustration of specialized structures and description of three outpatient clinics in the DACH region (Germany, Austria and Switzerland). METHOD

Maternal Nutrition Status Associated with Pregnancy-Related Adverse Outcomes.

Although maternal nutrition has an impact on fetal development and gestational outcome, tracking maternal nutrition in outpatient practice is still complex and involves proper technical capacitation in this area. Nevertheless, the association between nutritional variables may broaden the ability to predict the occurrence of gestational disorders and prevention management. We aimed to identify factors that could indicate the probability of adverse outcomes in mid-pregnancy. From a cohort of 1165 nulliparous pregnant women without any previous disease, the nutritional status was assessed by body

Exploring the Interrelationships between Diabetes, Nutrition, Anxiety, and Depression: Implications for Treatment and Prevention Strategies.

Across the world, diabetes, depression, and anxiety symptoms have gained widespread recognition as significant public health issues. Recent research has unveiled a mutually influential relationship between diabetes and these two mental health conditions, where each disorder impacts the course and outcomes of the others. The role of nutrition emerges as pivotal in preventing and treating depression, anxiety, and diabetes. A thorough literature review was undertaken to investigate the reciprocal effects between anxiety, depression, and diabetes, including their impact on the development and seve

The relationship between thyroid disorders and vitamin A.: A narrative minireview.

The terms "vitamin A" and "retinoids" encompass a group of fat-soluble compounds essential for human nutrition. Some of them (retinol, retinal, 9-cis-retinoic acid, tretinoin, and 13-cis-retinoic acid) are fully natural, while others are synthetic compounds used mostly for therapeutic purposes. Some evidence indicates that the nutritional status of these retinoids (i.e., the presence or absence of deficiency) is able to modulate thyroid gland metabolism. Vitamin A deficiency is tightly correlated with structural and functional impairment of the thyroid gland and is often associated with iodine

Early experience with a nutrition and survivorship clinic in esophageal cancer.

Improved cure rates in esophageal cancer care have increased focus on health-related quality of life (HRQL) in survivorship. To optimize recovery after esophagectomy, particularly nutritional well-being, a personalized multidisciplinary survivorship clinic was established at this center. Assessments at 6 and 12 months postoperatively include validated European Organization for the Research and Treatment of Cancer (EORTC) symptom and health-related quality of life (HRQL) questionnaires, functional status review, anthropometry, and biochemical screening for micronutrient deficiencies. 75 patient

Malnutrition screening in head and neck cancer patients with oropharyngeal dysphagia.

BACKGROUND & AIMS: Malnutrition in head and neck cancer (HNC) patients is associated with increased morbidity and mortality. The purpose of this study is two-fold: to identify the risk of malnutrition in patients with oropharyngeal dysphagia (OD) secondary to HNC, and to determine the relationship between the risk of malnutrition versus tumor characteristics, treatment modality, time interval (between the end of oncological treatment and swallowing assessment date), level of oral intake, body mass index (BMI), aspiration, pharyngeal pooling, and OD-related quality of life (QoL). METHODS: The

Proposal and Actions to Decrease Malnutrition in Latin America and the Caribbean.

While some of our countries are still facing nutrition deficiencies (stunting/short stature and micronutrient deficiencies), overweight and obesity are increasing in an alarming way. In addition, a very limited research network and lack of funding make it extremely challenging to foster collaborative efforts to address the complex nutritional problems that Latin America is facing. Therefore, as the future leaders in nutrition, we reinforce our commitment to improve our research capacity to identify opportunities for action to end malnutrition in all its forms in our region to address health di

[Nutritional problems of female adolescents].

Feeding in infancy is necessary to allow proper growth and development. Health of these early stages of life may influence the development of many diseases in the future (atherosclerosis, osteoporosis, hypertension, obesity ...). Furthermore habits set in childhood will endure throughout life. Therefore, getting adequate dietary and health patterns in childhood is vital. In adolescence occur a number of changes: rapid growth, development of secondary sexual characteristics, changes in body composition, ... that will be a challenge when getting or keeping that adequate feeding and habits. Maternal food insecurity is associated with increased risk of certain birth defects.

Food insecurity represents a lack of access to enough food to meet basic needs. We hypothesized that food insecurity may increase birth defect risks, because it is an indicator of increased stress or compromised nutrition, which are both implicated in birth defect etiologies. This study used population-based case-control data. Included in the analysis were 1,189 case mothers and 695 control mothers who were interviewed by telephone. We calculated a food insecurity score as the number of affirmative responses to 5 questions from a shortened instrument designed to measure food insecurity. OR for
