Fibromyalgia syndrome (FMS) presents a complex and challenging disorder in both the diagnosis and treatment, with emerging evidence suggesting a role of small fibre pathology (SFP) in its pathophysiology. The significance of the role of SFP in FMS remains unclear; however, recent evidence suggests degeneration and dysfunction of the peripheral nervous system, particularly small unmyelinated fibres, which may influence pathophysiology and underlying phenotype. Both skin biopsy and corneal confocal microscopy (CCM) have consistently demonstrated that ~ 50% of people with FMS have SFP. CCM, a non…

Medullary thyroid carcinoma (MTC) is the most common neuroendocrine carcinoma within the thyroid gland, characterized by strong invasiveness, high metastasis and recurrence rates. It is relatively rare among thyroid malignancies. The cytological and histological features of MTC are diverse and disperse, presenting as papillary, follicular, solid, trabecular, and spindle cell patterns. Immunohistochemical staining shows variable expression of calcitonin, carcinoembryonic antigen, and neuroendocrine markers. MTC can be classified into hereditary and sporadic types, with most cases caused by germ

The accumulation of intramuscular adipose tissue (IMAT) is a nearly ubiquitous feature of skeletal muscle pathology, strongly correlating with impaired contractility and metabolic dysfunction across a wide spectrum of clinical conditions, from aging and obesity to genetic myopathies and orthopedic injuries. For decades, a critical question has persisted: is IMAT a passive biomarker of disease progression or an active pathogenic agent? This review synthesizes emerging evidence to address this question by exploring several key areas. We first evaluate the mechanisms by which IMAT impairs muscle …

Hair and nail complaints are common in the primary care clinic. Although not a comprehensive assessment of all conditions, this article addresses the most common hair and nail pathologies seen in primary care clinics. It discusses physiologic explanations for pathology to provide clinicians with a framework for narrowing their differential diagnosis. It also provides guidance for identifying significant pathology that would best be served through referrals to specialist providers.

Clostridium colinum causes ulcerative enteritis in several avian species. The disease is particularly prevalent in quail, and it is therefore colloquially known as quail disease. The pathogenesis of the infection is poorly understood. A retrospective study of C. colinum infection in quail submitted for necropsy and diagnostic work up to the California Animal Health and Food Safety Laboratory System between 1992 and 2022 was performed. The necropsy reports were reviewed, and polymerase chain reaction (PCR) for C. colinum (16S rRNA) was performed on formalin-fixed, paraffin-embedded tissues. C.

Vestibular schwannomas (VS) are WHO Grade 1 tumors arising from the vestibule-cochlear nerve and account for 8% of all intracranial neoplasms. In recent years, there has been a dramatic evolution in how the pathology of these tumors is regarded, and there is emerging interest in the role of the tumor microenvironment. In this chapter, we provide a comprehensive description of the pathology and microenvironment of VS, with a particular focus on the interlinked processes of immune cell infiltration and angiogenesis. Improved understanding of the pathophysiology of these tumors has already yielde…

Presbycusis, also known as age-related hearing loss (ARHL), is a progressive auditory impairment and ranks among the most prevalent sensory disorders in the elderly population. It is primarily caused by damage to hair cells, degeneration of spiral ganglion neurons, and atrophy of the stria vascularis, which are integral components of the cochlea. While extensive research has been devoted to hair cells and spiral ganglion neurons, this review focuses on the critical role of the stria vascularis in ARHL. The primary function of the stria vascularis is to maintain the endocochlear potential, whic

Palatine tonsils are part of the Waldeyer's ring and consist of lymphoid tissue. As a first-line immune system mechanism, they act against entering pathogens. They recognize and initiate an immune response to exogenous antigens. Tonsillar disease in children, particularly inflammatory conditions, is well known in the medical community. Inflammatory diseases include conditions such as tonsillitis and peritonsillar cellulitis and other more complicated diseases such as intratonsillar abscess, peritonsillar abscess, and parapharyngeal abscess. Their treatment varies, with more severe cases often

Root canal treatment has been shown to be a predictable procedure with a favourable outcome in the case of pulpitis and apical periodontitis. The most important outcome measure is retention of an asymptomatic and functional tooth. When teeth are lost after root canal treatment, this is often the result of fracture due to weakening. By preventing or postponing root canal treatment, the long-term prognosis of the tooth may be better. Minimal interventions in endodontics are vital pulp therapy (indirect pulp therapy and pulpotomy), postponing and monitoring, and partial revision.

Among the cardiac outpouchings, left ventricular aneurysms or pseudo-aneurysms that develop secondary to myocardial infarctions are the most common. On the other hand, atrial appendageal and valvular aneurysms are uncommon occurrences. The appendageal aneurysms develop possibly due to congenital dysplasia of atrial pectinate muscles, while valvular aneurysms result from infective endocarditis, mechanical injury or degenerative changes. Despite their unusual locations, they are prone to life-threatening complications. We present our experience of unusual cardiac aneurysms in surgical cardiovasc

Pediatric foot development throughout childhood and adolescence can present a diagnostic dilemma for radiologists because imaging appearances may be confused with pathology. Understanding pediatric foot development and anatomical variants, such as accessory ossification centers, is essential to interpret musculoskeletal imaging in children correctly, particularly because many of these variants are incidental but others can be symptomatic. We first briefly review foot embryology. After describing common accessory ossification centers of the foot, we explain the different patterns of foot matura…

Cubital tunnel syndrome (CuTS) is the second most common peripheral neuropathy in the upper limb. It occurs due to ulnar nerve compression within the fibro-osseous cubital tunnel at the elbow joint. Although CuTS is typically diagnosed clinically and with electrodiagnostic studies, the importance of imaging in evaluating the condition is growing. Knowing the typical imaging findings of ulnar nerve entrapment is necessary for precise diagnosis and proper treatment. In this article, we focus on the clinical features, workup and complex imaging of the "anatomic" cubital tunnel and relevant pathol

Normal lower limb alignment is with the tibia in varus and the femur in valgus, forming an oblique joint line in bipedal stance and a horizontal line in unipedal stance. Alignment may be valgus or varus in case of femoral metaphyseal or tibial-femoral deformity, respectively. Bone correction must be performed at the site of the deformity. If a femoral deformity is corrected at the tibia, this results in an oblique joint line and malunion, with poor functional outcome. In genu valgum, distal femoral osteotomy (either medial closing or lateral opening wedge) may be indicated in case of lateral f

Subacute sclerosing panencephalitis (SSPE) is a fatal, slowly progressive brain disorder caused by a mutated measles virus. Both subacute inflammatory and neurodegenerative mechanisms appear to play significant roles in the pathogenesis. TAR DNA-binding protein 43 (TDP-43) inclusions are a common co-pathology in several neurodegenerative disorders with diverse pathogenesis. In the present study, we examined brains of 16 autopsied SSPE patients for the presence of TDP-43 pathology and possible associations with tau pathology. Immunohistochemical staining identified TDP-43 inclusions in 31% of S…

Acute cholangitis is a well-known biliary tree pathology most often encountered in patients with gallstone disease. When left untreated, acute cholangitis can lead to severe complications, including death. Therefore, identifying and properly treating acute cholangitis is crucial to avoiding such complications. This paper describes an 84-year-old female patient with acute cholangitis who presented with atypical symptoms of chest pain and cough. The patient was successfully treated with endoscopic retrograde cholangiopancreatography (ERCP), antibiotics, and ursodeoxycholic acid. We focus on this…
