Unusual case of leptomeningeal dissemination of a diffuse pontine high-grade astrocytoma in a child.

A 12-year-old girl was seen in our hospital and diagnosed with diffuse pontine glioma. Treatment with radiotherapy was suggested, but the parents rejected this plan and took the child to another hospital where partial resection of the tumor was performed. The histological diagnosis was anaplastic astrocytoma. Returning to our hospital, the parents agreed to radiotherapy which was given in a total dose of 54 Gy. However, 3 months after completion of radiotherapy, the child developed widespread leptomeningeal dissemination of her disease and died 12 months after the initial diagnosis. Since no e

Microsurgical anatomy of the mesencephalic veins.

OBJECT: The mesencephalic veins drain crucial brainstem areas. Due to the narrowness of the tentorial notch, these veins can become obstructed as a result of herniation or surgery, leading to hemorrhage and severe consequences for the patient. There is little in the literature about the mesencephalic veins. The aim of this study was to perform an exact analysis of their microanatomy. METHODS: Fifty-two cadaveric hemispheres were examined under an operating microscope, and measurements were made with a digital caliper. The authors focused on the basal vein (BV), pontomesencephalic vein (PMV),

Bilateral latero-pontine infarction: a rare syndrome, an unusual imaging.

The authors present one case of bilateral infarction of the brain stem, in the territory of the antero-inferior cerebellar artery. Besides the fact that such bilateral ischaemic lesions are rare, the initial clinical presentation mimicking a vestibular neuronitis indicates that this latero-pontine syndrome may appear as a peripheral disorder. It also reminds clinicians that careful and repeated examinations should always be performed in all patients suffering from vertigo. An unusual iconography is also presented.

The value of the heavily T2-weighted sequence in evaluation of the cisternal and petroclival segment of the abducent nerve.

The purpose of the study was to evaluate the usefulness of magnetic resonance (MR) in imaging of the cisternal and petroclival segments of the abducent nerve. Heavily T2-weighted submillimetric 3D sequence in axial plane, T1-weighted 3D, 1.5 mm slice thickness sequence in axial plane and TOF sequence were performed on 16 volunteers. Additionally the reformatted T2-weighted images in sagittal and in oblique parasagittal plane parallel to the abducent nerve in the pontine cistern were performed. The heavily T2-weighted sequence provides high contrast between fluid and other structures. High sign

MRI characteristics of the MLF in MS patients with chronic internuclear ophthalmoparesis.

OBJECTIVE: The authors imaged the medial longitudinal fasciculus (MLF) in 58 patients with MS and chronic internuclear ophthalmoparesis (INO … in 88% and 48% of patients, respectively. With PDI, dorsomedial tegmentum lesions were seen in the pons in 93% of patients and in the midbrain of 66% of patients. Lesions were observed at both locations in 59% of patients. One patient had an MLF lesion with a corresponding T1 hypointensity. CONCLUSIONS: PDI best shows the MLF lesion in patients with …

[Nightmares and behavior during REM sleep].

This study reports the current neurobiologic data on REM sleep including psychoanalysis, distinguishing between the "instincts of pleasure" and the "instincts of death" (Beyond the principles of pleasure, S. Freud, 1920). The award and punishment systems are also reported and current data related to nightmares and mainly dream behavior during REM sleep without atonia are presented underlining the frequency of parasomnia in ponto-cerebellous atrophy and Parkinson's disease. The author emphasizes the need for establishing discussions among the different neuroscientific fields related to these qu

Decreased diffusion in central pontine myelinolysis.

Two patients with central pontine myelinolysis (CPM) were studied with diffusion-weighted MR imaging 1 week after onset of tetraplegia. In both patients, affected white matter showed hyperintensity on diffusion-weighted images associated with a decrease in apparent diffusion coefficient (ADC) values. In one patient studied serially, ADC values normalized by 3 weeks after tetraplegia. Early in the clinical course, diagnosis of CPM can sometimes be difficult. Hyperintensity on diffusion-weighted images may therefore have diagnostic utility. Decreased lesional ADC values support the notion that C

Cognitive functions in chronic locked-in syndrome: a report of two cases.

We describe a 38-year-old woman and a 38-year-old man who have been living in a "locked-in" condition 3 and 2 years, respectively. Extensive neuropsychological testing, carried out with a communication aid system (writing apparatus) which exploited the patients residual eye movements, showed preserved cognitive abilities in spite of the long standing de-efferentation.

Extra pontine myelinolysis in a tetraplegic patient: case report.

Hyposmolar hyponatremia (serum sodium < 130 mmol/l) is a common phenomenon in the spinal cord injury (SCI) patient population and in most cases, it is of relatively little consequence. However, rapid correction or over correction of hyponatremia (a change in serum sodium > 25 mmol/l within 48 h) has been linked to Central Pontine Myelinolysis (CPM) and Extra Pontine Myelinolysis (EPM), usually along with other recognized predisposing factors. We report the first case of isolated Extra Pontine Myelinolysis in an SCI patient without any of the recognized predisposing factors, following correctio

[A study of eight patients with non-paralytic pontine exotropia].

We reported 8 patients with non-paralytic pontine exotropia (NPPE): 7 men and 1 woman aged from 56 to 79 years (mean: 67.8 years). From 1991 … performed to detect the causative lesion, a small infarct was found in the upper, middle, and lower pons in 1, 3, and 4 patients, respectively. NPPE is generally considered to develop when the MLF and the paramedian pontine reticular formation (PPRF) are completely and incompletely destroyed respectively. In the present study, however, the pathophysiol …

[Pure sensory stroke due to pontine lacunar infarction].

A 72-year-old woman presented a sudden onset isolated sensory deficit of medial lemniscal type in the right half of the body. MRI showed a small lacune in the left paramedian pontine tegmentum corresponding to the location of the medial lemniscus. MRI appeared to be most valuable in the topographic delineation of lacunar infarctions which were responsible for pure sensory stroke.

Fatal basilar vasculopathy complicating bacterial meningitis.

BACKGROUND: Bacterial meningitis complicated by thrombosis, vasculitis, and aneurysm formation affecting both small and distal branches of cerebral vessels has been well described. Involvement of major cerebral vessels is rare and has only been documented late in the course of disease. CASE DESCRIPTION: We describe the clinical and pathological findings in a young man who presented with pontine infarction as an early manifestation of bacterial meningitis. Streptococcus milleri, an unusual organism in this setting, was cultured. Despite improvement with antibiotic therapy, the patient experien

[Unusual pathological ocular movement caused by dysfunction of the saccade generation mechanism].

We describe a patient with abnormal ocular movement, probably due to dysfunction of saccade generation mechanism in the lower pons involving the omnipause neuron and a feed-back circuit. A 26-year-old man had fever, mild headache and sore throat for a few days and also noted unstable gait and diplopia. These symptoms subsided, but he noticed oscillopsia about 2 months later. On admission in our hospital, the neurological examination revealed no abnormalities except for abnormal eye movements, which were induced by voluntary or involuntary ocular movement and lasted for a few minutes. The abnor…

Pontospinal transmitters and their distribution.

The dorsolateral pontine tegmentum of the cat is known to contain a large population of catecholaminergic neurons. Additionally, several studies have also shown the presence of other neurochemicals (acetylcholine, enkephalin, neuropeptide Y, serotonin, somatostatin and substance P). In this study, we have employed retrograde transport of horseradish peroxidase in combination with immunocytochemistry to determine the locations of pontospinal neurons which contain catecholamine, enkephalin, neuropeptide Y, and serotonin. Furthermore, we have combined the retrograde transport of Fast Blue and imm
