Brown-Sequard syndrome associated with Horner's syndrome following a penetrating drill bit injury to the cervical spine.

We report a 41-year-old male who presented with a partial Brown-Sequard syndrome and Horner's syndrome following a penetrating drill bit injury to his mid cervical spine. As the injury was not a complete hemisection of the spinal cord, the patient presented with ipsilateral motor deficit and hyperesthesia and diminished contralateral fine touch sensation; however, proprioception, vibration and temperature were all initially intact. A cervical CT and MRI scan showed a damaged spinal cord at the C5/6 level with posterior cord compression secondary to haematoma. A decompressive laminectomy and ev…

Role of ankle taping and bracing in the athlete.

Adhesive tape is often used to help athletes recover from ligament sprains of the ankle or to prevent further injury. The choice of taping t … ncerning the effects of various methods of ankle support on swelling, stability, range of movement, proprioception, muscle function, gait, and performance tests. There is still some contradiction in the literature about the effects of taping and braces in both the acute and chronic phases of ligament sprains of the ankle.

Visual destabilisation of posture in normal subjects.

A new and simple method of assessing reliance on vision for postural control was evaluated in 41 normal subjects. Left-right reversal of peripheral vision induced by a head-mounted mirror device caused an initial dramatic instability in approximately half the subjects, when standing on foam to reduce the value of proprioception. Lateral reversal of central vision by means of a prism device evoked similar responses. Sensitivity to vision reversal was significantly correlated with motion sickness susceptibility. Despite some rapid habituation (partially retained over several weeks) sway remained…

Fascial manipulation as an adjunct to physiotherapy management in obstetric brachial plexus palsy: A case report.

The integrity of connective tissue sheaths surrounding the nerves influences both the severity and the potential for recovery of brachial plexus lesions. This study presents an innovative, early onset, multidisciplinary approach to obstetric brachial plexus palsy. This approach is aimed at functional recovery of the nerve lesion and includes mobilization of the fascia using the Fascial Manipulation® method. This case study discusses how, in addition to conventional treatment, interventions aimed at the fascial system can potentially affect tension around the neural sheaths, enhance propriocept

Motor cortex excitability changes following a lesion in the posterior columns of the cervical spinal cord.

We used transcranial magnetic stimulation (TMS) to explore if an impairment of central sensory function produced by an isolated lesion in the cervical posterior white columns would change motor cortex excitability. Cortical silent period duration was prolonged when compared with the control subjects, while central motor conduction and motor thresholds were in the normal limits. We first demonstrate that the involvement of the ascending proprioceptive sensory pathways in spinal cord diseases may have direct consequences on the activity of intracortical inhibitory interneuronal circuits. These f

Movement-related cerebellar activation in the absence of sensory input.

Movement-related cerebellar activation may be due to sensory or motor processing. Ordinarily, sensory and motor processing are obligatorily linked, but in patients who have severe pansensory neuropathies with normal muscle strength, motor activity occurs in isolation. In the present study, positron emission tomography and functional magnetic resonance imaging in such patients showed no cerebellar activation with passive movement, whereas there was prominent movement-related cerebellar activation despite absence of proprioceptive or visual input. The results indicate that motor processing occur

Pseudochoreoathetosis in four patients with hypesthetic ataxic hemiparesis in a thalamic lesion.

Thalamic lesions give rise to a variety of clinical syndromes including choreoathetotic movements and ataxic hemiparesis as well as sensory deficits. We describe four patients exhibiting pseudochoreoathetosis, hypesthesia, and ataxic hemiparesis in the limbs contralateral to a thalamic lesion. Three of the four patients showed the involuntary movements within 10 days of stroke onset; the remaining patient was not seen until 4 years later. Three had infarction and the other one hemorrhage in the posterior and lateral thalamus. All the patients had both cerebellar and sensory ataxia. These cases

[Two stage Syme amputation].

In 1843 James Syme described the amputation which bears his name. It is done in two stages and can be accomplished in the presence of wounds and infections or gangrene of the feet. The operation is of great value especially in the elderly. Patients with a Syme stump can be fitted with a strong, light, plastic prosthesis, they develop an excellent gait and the entire body weight can be borne without pain. The advantages of the Syme amputation are that the knee joint is retained and the long lever allows an almost normal gait with an energy cost only 10% above normal. The stump has a large inter

Multisensory feedback makes swimming circuits robust against spinal transection and enables terrestrial crawling in elongate fish.

Vertebrate locomotion is due to the interplay of neural oscillators and sensory feedback loops in the spinal cord that interact with the body and the environment. Here, we study these circuits with a focus on undulatory locomotion as produced by elongated fish such as eels and lampreys. We address three questions: i) How do proprioception (stretch feedback) and exteroception (pressure on skin) interact with local oscillators to generate stable swimming patterns? ii) Can these feedback loops also contribute to dry ground locomotion? iii) Can they explain the remarkable robustness of eels agains…

No significant links between somatognosia, stereognosia, and hypermobility: sensory processing unlikely to drive common complaints in hypermobile population.

BACKGROUND: Previous research has demonstrated impaired proprioception and poorer responses to tactile deep pressure, visual-tactile integration, and vestibular stimuli in individuals with generalized hypermobility, potentially leading to sensory processing issues. Therefore, we aimed to explore the influence of hypermobility on somatognosia and stereognosia. METHODS: Forty-six participants were assessed using the Beighton score and categorized into three groups: non-hypermobile (n = 20), symptomatic hypermobile (n = 13), and asymptomatic hypermobile (n = 13). Somatognosia was evaluated using…

Impact of lumbar delayed-onset muscle soreness on postural stability in standing postures.

BACKGROUND: Similar impact on proprioception has been observed in participants with lumbar delayed-onset muscle soreness (DOMS) and chronic low back pain (LBP), raising questions about the relevance of lumbar DOMS as a suitable pain model for LBP when assessing back pain-related postural stability changes. RESEARCH QUESTION: Does lumbar DOMS impact postural stability? METHODS: Twenty healthy adults participated in this experimental study and underwent a posturographic examination before and 24 to 36 h after a protocol designed to induce lumbar DOMS. Posturographic examination was assessed du…

Distinct neural signatures of multimodal resizing illusions.

Illusory body resizing typically uses multisensory integration to change the perceived size of a body part. Previous studies associate these multisensory body illusions with frontal theta oscillations and parietal gamma oscillations for dis-integration and integration of multisensory signals, respectively. However, recent studies also support illusory changes of embodiment from unimodal visual stimuli. This preregistered study (N = 48) investigated differences between multisensory visuo-tactile and unimodal visual resizing illusions using EEG, to gain a more comprehensive understanding of the

Interoceptive Accuracy in Youth with Tic Disorders: Exploring Links with Premonitory Urge, Anxiety and Quality of Life.

Aberrant interoceptive accuracy could contribute to the co-occurrence of anxiety and premonitory urge in chronic tic disorders (CTD). If it can be manipulated through intervention, it would offer a transdiagnostic treatment target for tics and anxiety. Interoceptive accuracy was first assessed consistent with previous protocols and then re-assessed following an instruction attempting to experimentally enhance awareness. The CTD group demonstrated lower interoceptive accuracy than controls but, importantly, this group difference was no longer significant following instruction. In the CTD group,

A unified internal model theory to resolve the paradox of active versus passive self-motion sensation.

Magnesium, an important cofactor of more than 300 enzymes, has previously been found to modulate blood lipid levels, atherogenesis and atherosclerosis in rabbits, when added to their diet. The aim of this study was to examine whether magnesium fortification of drinking water, without a change in diet content, can affect atherogenesis. The study included six groups of LDL-receptor-deficient mice. The mice received either distilled water or water containing 50 g of magnesium sulfate per liter. In the first (12 weeks) and second (6 weeks) stages of the experiment, the mice received low- and high-

Ultrasound-guided administration of lidocaine into the sciatic nerve in a porcine model: correlation between the ultrasonographic evolution of the lesions, locomotor function and histological findings.

Intraneural puncture of local anaesthetics has been associated with permanent or transitory nerve injury. The use of ultrasound (US)-guided techniques for the blockade of peripheral nerves has revealed that intraneural puncture is a relatively common complication, which is not frequently associated with neurological deficits. In this study, 2.5 mL of lidocaine were administered using US-guidance into the sciatic nerve (ScN) of 12 piglets. The punctured nerves were sequentially evaluated by US (cross sectional area and relative echogenicity) before and immediately after the injections, and then

Soccer-specific fatigue decreases reactive postural control with implications for ankle sprain injury.

To quantify the influence of soccer-specific fatigue on reactive dynamic balance, ten male professional soccer players (age 24.7 ± 4.4 yr, body mass 77.1 ± 8.3 kg, VO2max 63.0 ± 4.8 ml·kg·min(-1)) completed an exercise protocol replicating the activity profile of match-play. Pre-exercise, and at 15 min intervals, players completed three balance tasks requiring response to a system perturbed to induce either plantar flexion of the ankle or inversion of the ankle (by rotation or translation). ANOVA revealed a significant main effect for exercise duration in each task, with both reaction time and
