Does Anterior Disc Displacement with Reduction Affect Postoperative Pain Perception After Root Canal Therapy? A Prospective Comparative Clinical Study.

Background/Objectives: Temporomandibular disorders may influence the perception of odontogenic pain through shared trigeminal pathways and referred pain mechanisms. This study compared postoperative pain following root canal treatment (RCT) between patients with anterior disc displacement with reduction (ADDwR) and individuals without temporomandibular disorders. Methods: The study was registered at ClinicalTrials.gov (NCT07329413; 8 January 2026). Individuals with irreversible pulpitis and symptomatic apical periodontitis in a vital mandibular molar, with or without ADDwR according to the Dia…

Identification of clusters of individuals relevant to temporomandibular disorders and other chronic pain conditions: the OPPERA study.

The classification of most chronic pain disorders gives emphasis to anatomical location of the pain to distinguish one disorder from the other (eg, back pain vs temporomandibular disorder [TMD]) or to define subtypes (eg, TMD myalgia vs arthralgia). However, anatomical criteria overlook etiology, potentially hampering treatment decisions. This study identified clusters of individuals using a comprehensive array of biopsychosocial measures. Data were collected from a case-control study of 1031 chronic TMD cases and 3247 TMD-free controls. Three subgroups were identified using supervised cluster

The Semantics of 'Hip Pain' and its Impact on Clinical Practice in Patient-Reported Outcome Measures (PROMs) of Disease: Results from a Clinical and Radiological Evaluation Cohort.

INTRODUCTION: The item 'hip pain' is widely used in questionnaires related to Spondyloarthritis and/or Ankylosing spondylitis (AS), either in clinics with patients being physically present or remotely, as the hip joint is known to affect AS in particular. Patients in clinics often claim to have hip pain. However, by stating "hip" they are referring to variable structures located in the hip region not necessarily related to hip joint itself. OBJECTIVE: To assess which structure(s) patients mean when referring to hip pain. METHODS: A diagram used as a proforma for patients to indicate the site

Abdominal Cutaneous Nerve Entrapment Syndrome: The Cause of Localized Abdominal Pain in a Young Pregnant Woman.

BACKGROUND: Despite the broad differential diagnosis in any patient referring with symptoms involving the chest or abdomen, a small number of conditions overshadow the rest by their probability. Chest and abdominal wall pain continues to constitute a common and expensive overlooked source of pain of unknown cause. In particular, cutaneous nerve entrapment syndrome is commonly encountered but not easily diagnosed unless its specific symptoms are sought and the precise physical examination undertaken. CASE REPORT: A primigravida woman with unbearable abdominal pain was referred repeatedly seeki

Do arthralgias occur after bilio-intestinal bypass for morbid obesity.

BACKGROUND: Intestinal bypass operations are used occasionally for treating morbid obesity if other methods are unsuitable. Jejuno-ileal bypass (JIB) operations are considered to be a cause of occasional joint pain related disorders. However, bilio-intestinal bypass (BIB) is intended to avoid the blind loop syndrome. This study compared joint pain disorders in an operated BIB group, an obese nonoperated control group, and a lean reference group. METHODS: Using a questionnaire in a cross-sectional study, pain disorders were investigated in age-, sex- and weight-matched groups of operated, obes

Spinal Cord Stimulation for Painful Neuropathic Cauda Equina Syndrome Following Ballistic Penetrating Lumbar Spine Injury: Proof-of-Concept Case.

Modern warfare, as well as the rise of terrorist attacks on the mainland France, make military personnel deployed overseas but also civilians more prone to sustain wartime penetrating injury, the spine being involved in 6%-8% of the cases. In the long-term, some of these casualties may suffer from neuropathic pain of the lower limbs refractory to medical treatment. Thoracic spinal cord stimulation is a possible surgical treatment option for chronic neuropathic pain of the limbs caused by diabetic neuropathy, type 2 persistent spinal pain syndrome after back surgery, and complex regional pain s

Prevalence of subtypes of low back pain in a defined population.

BACKGROUND: For two generations, some back care specialists have emphasized that clinical low back pain is composed of a number of specific syndromes, such as sacroiliitis or trigger points, but the prevalence of these syndromes outside of specialized clinics remains unknown. The purpose of this study was to describe the prevalence of subtypes of low back pain in a defined population. METHODS: The setting was a group model HMO with a population of 54,000. We used a formal group process involving clinicians from many disciplines, long interviews, critical appraisal of the literature, case disc

Topical capsaicin selectively attenuates heat pain and A delta fiber-mediated laser-evoked potentials.

Cutaneous stimulation with CO2 laser pulses activates A delta of nociceptive afferents and evokes late cerebral potentials (LEPs), the amplitude of which correlates parametrically with the perceived magnitude estimation of laser pulses. Capsaicin is known to desensitize the nociceptive terminals of C fibers. In this double-blind, vehicle-controlled experiment, we tested the hypothesis that topical capsaicin would inactivate A delta afferents and lead to an attenuation of the LEPs. Subjects applied capsaicin cream to the dorsum of one hand and vehicle cream to the other 3 times daily for a peri
