Comprehensive overview of management and risk assessment of breast cancer-related lymphedema: a multidisciplinary approach.

Breast cancer is one of the most commonly diagnosed cancers worldwide, with a particularly high incidence in women. Although many are cured of their malignancy, treatment options can present with significant long-term complications for the patient. Lymphedema is the accumulation of protein-rich fluid that occurs following a disturbance to the lymphatic system. Secondary lymphedema to the arm often results following breast surgery or axillary lymph node dissection, although risk and exacerbating factors include radiation and medical chemotherapy, both in the short- and long-term intervals. Post-dural Puncture Headache: Pathophysiology, Risk Stratification, Prevention, and Evidence-Based Management for Practicing Anesthesiologists.

Post-dural puncture headache (PDPH) is a well-recognized secondary headache disorder resulting from persistent cerebrospinal fluid (CSF) leakage following iatrogenic dural disruption during neuraxial procedures, including spinal anesthesia, diagnostic lumbar puncture, dural puncture epidural (DPE), and unintentional dural puncture (UDP). The syndrome reflects a complex interaction between CSF hydrodynamics, craniospinal compliance, vascular compensation, and nociceptive sensitization. Reduction in CSF volume produces intracranial hypotension and compensatory venous dilation in accordance with

AI-enabled comprehensive patient safety management in acupuncture: from risk identification to continuous quality improvement.

Acupuncture, as an important component of traditional medicine, has been widely integrated into the management of various clinical conditions. Although acupuncture is generally considered safe and well tolerated, adverse events, including bleeding, pneumothorax, neurovascular injury, infection, and retained needles, may still occur. Current safety management approaches largely rely on operator experience and manual monitoring, which may be insufficient to address individual patient variability and dynamic procedural risks. In recent years, Artificial Intelligence (AI) has emerged as a promisin

The Role of Routine Surveillance Cultures in Optimising Sepsis Management in High-Risk Patient Groups.

BACKGROUND: Sepsis remains a leading cause of morbidity and mortality, particularly when caused by multidrug-resistant organisms (MDROs). Early identification of colonising or infecting pathogens may inform initial antimicrobial selection. Surveillance cultures, providing microbiological data prior to infection onset, could guide timely and targeted therapy. This retrospective study analysed routine surveillance culture results from patients with bloodstream infection (BSI) episodes, assessing pathogen prevalence, resistance phenotypes, and concordance with specimen type in haemato-oncology (H

Guidance for clinical management of pathogenic variant carriers at elevated genetic risk for ALS/FTD.

There is a growing understanding of the presymptomatic stages of amyotrophic lateral sclerosis (ALS) and frontotemporal dementia (FTD) and nascent efforts aiming to prevent these devastating neurodegenerative diseases have emerged. This progress is attributable, in no small part, to the altruism of people living with pathogenic variants at elevated genetic risk for ALS/FTD via their willingness to participate in natural history studies and disease prevention trials. Increasingly, this community has also highlighted the urgent need to develop paradigms for providing appropriate clinical care fo

Laparoscopic Management of Suspected Gallbladder Cancer: Risk Stratified Approach in Thick-Walled Gallbladder and Polyps.

INTRODUCTION: Thick-walled gallbladder (TWGB) and gallbladder polyps (GBP) can harbour cancer. Differentiating benign from malignant is challenging owing to the limitations of diagnostic studies and unreliability of predictive models. Simple or radical cholecystectomy pose risks of either inadequate oncological clearance or overtreatment. While anticipatory extended and cystic plate cholecystectomy with frozen section is useful but has limitations and laparoscopic approach with risk stratification is not well documented. METHODS: In this retrospective study, 39 patients with TWGB or GBP suspe

Risk Factors and Management Strategies for Antipsychotic-Induced Weight Gain: A Prescriptive Review for Clinicians.

Antipsychotic-induced weight gain (AIWG) is a pervasive and concerning side effect of treatment with antipsychotic medications (APs) that has grave implications for morbidity and mortality rates in individuals with serious mental illness. As such, acknowledging, identifying, and addressing the metabolic side effects of these medications are critical to improve the overall health and quality of life of individuals treated with APs. This review summarizes the key risk factors and predictors of AIWG and provides a comprehensive and prescriptive overview of the best-researched and evidenced nonpha

Incorporating Community Case Management in Risk-Based Surveillance for Malaria Elimination in the Dominican Republic.

As countries strive for malaria elimination, it is crucial to gather sufficient evidence to confirm the absence of transmission. Routine surveillance data often lack the sensitivity to detect community transmission at low levels. In the Dominican Republic, community health workers (CHWs) have been deployed in malaria foci to perform active case detection. This study aimed to assess the added value of CHWs in enhancing the health system's malaria detection capabilities. Freedom from infection (FFI) is a statistical framework designed to demonstrate the absence of malaria by using routinely coll

Immediate Cervical Muscle Response to Optimal Occlusal Positioning: A Crucial Part of Concussion Risk Management.

Objectives: Strong cervical musculature is recognized as a protective factor against sports-related concussions. Evidence suggests that jaw clenching may activate cervical muscles, potentially reducing head acceleration during impact. Methods: This observational cohort study examined the immediate effects of a customized interocclusal orthotic (CIO) on cervical muscle performance. Forty-two healthy adults (≥18 years) underwent strength and endurance testing with and without a CIO using a digital pressure gauge and six directional isometric contractions. Descriptive statistics and two-way repea

Statins, LDL-C and Beyond: A Contemporary Review of Atherosclerotic Cardiovascular Risk Assessment and Management.

Cardiovascular disease has been the leading cause of death in the United States since 1921 and remains a substantial cause of morbidity and mortality worldwide. Since the discovery of statins, significant improvements in outcomes have been achieved as these drugs have direct atherogenic lipid lowering properties as well as pleiotropic effects on atherosclerotic cardiovascular disease (ASCVD). In recent years, the American Heart Association and the American College of Cardiology published a 10-year ASCVD event risk calculator to standardized risk assessment and guide statin therapy. Incontinence-Associated Dermatitis in Older Adults: A Critical Review of Risk Factors, Prevention and Management.

Prolonged and repeated exposure of the skin to urine and/or faeces may lead to incontinence-associated dermatitis (IAD). IAD is an irritant contact dermatitis characterised by pain, erythema, maceration, erosion, scaling and very often associated with secondary infection. Older adults who are incontinent are at high IAD risk. Several differential diagnoses must be separated from IAD, with allergic contact dermatitis being the most common in older people. The main prevention and treatment principles are to reduce or to avoid the exposure of the skin to urine and stool. The type of incontinence
