OSTEOSYNTHESIS OF A SCAPHOID NECK FRACTURE WITH A CANNULATED COMPRESSION SCREW: EVALUATION OF 52 PATIENTS.

OBJECTIVE: To evaluate the effectiveness of the dorsal fixation technique with a cannulated compression screw (CCS) for transverse scaphoid neck fractures. METHODS: A case series study was carried out with patients treated with a CSS between April 2014 and May 2021. The main outcome was the healing of the fracture, verified by radiographic evaluation that used images of the wrist in anteroposterior, lateral, radial deviation, ulnar deviation and oblique views, obtained in the postoperative period. RESULTS: Fifty-two patients aged between 15 and 65 years were analyzed, of which 43 (83%) were

Hypoxia in Bone and Oxygen Releasing Biomaterials in Fracture Treatments Using Mesenchymal Stem Cell Therapy: A Review.

Bone fractures have a high degree of severity. This is usually a result of the physical trauma of diseases that affect bone tissues, such as osteoporosis. Due to its highly vascular nature, the bone is in a constant state of remodeling. Although those of younger ages possess bones with high regenerative potential, the impact of a disrupted vasculature can severely affect the recovery process and cause osteonecrosis. This is commonly seen in the neck of femur, scaphoid, and talus bone. In recent years, mesenchymal stem cell (MSC) therapy has been used to aid in the regeneration of afflicted bon

[Fate of the calcium-dense proximal fragment in scaphoid fracture].

A cohort study with retrolective and prolective collection of data was carried out 49 of 99 patients with scaphoid fractures, initially treated by plaster cast immobilization and having developed a proximal fragment with increased bone density were studied. More than half of these patients had a good or excellent result according to the modified score of Green and O'Brien 29 of 49 proximal fragments with increased bone density returned to normal density: 45 of 49 showed bony union, and 35 of 49 showed no signs of posttraumatic arthrosis. The appearance of stripes with increased bone density in

A comparative study of image quality and diagnostic confidence in diagnosis and follow-up of scaphoid fractures using photon-counting detector CT and energy-integrating detector CT.

PURPOSE: Scaphoid fractures in patients and assessment of healing using PCD-CT have, as far as we know, not yet been studied. Therefore, the aim was to compare photon counting detector CT (PCD-CT) with energy integrating detector CT (EID-CT) in terms of fracture visibility and evaluation of fracture healing. METHOD: Eight patients with scaphoid fracture were examined with EID-CT and PCD-CT within the first week post-trauma, and with additional scans at 4, 6 and 8 weeks. Our clinical protocol for wrist examination with EID-CT was used (CTDIvol 3.1 ± 0.1 mGy, UHR kernel Ur77). For PCD-CT matche

Medium-term outcome of closed radial wedge osteotomy of the distal radius for Preiser disease with concomitant Kienböck disease: Two case reports and a literature review.

RATIONALE: Idiopathic avascular necrosis of the scaphoid or lunate bone are known as Preiser disease and Kienböck disease, respectively. Although there are reports of avascular necrosis involving more than one carpal bone, concurrent idiopathic avascular necrosis of the scaphoid and lunate bones is rare, with only five cases reported in the English literature (including the two herein). Although the optimum treatment for Preiser disease with concomitant Kienböck disease has not been established, our cases underwent closed radial wedge osteotomy based on the evidence of satisfactory outcomes fo

Periscaphoid and Lunate Dislocation With an Intact Rotated, Scapholunate Interosseous Ligament: A Case Report.

Perilunate disassociation injuries are rare and may be missed in the acute setting. Prompt identification and management are critical to prevent late complications such as arthritis and instability. These injuries can present several patterns with soft tissue disruption, fracture, or a combination of both. In this report, we present a unique pattern of a periscaphoid, perilunte injury with an intact scapholunate interosseous ligament and describe our treatment strategy and outcome.

Comparing Conservative and Surgical Treatment of Acute Nondisplaced Fractures of the Scaphoid Based on Fracture Location: A Systematic Review and Meta-Analysis.

INTRODUCTION: Treatment of nondisplaced scaphoid fractures with either nonsurgical or surgical methods ideally involves shared decision making. To date, no meta-analysis has explicitly reported differences in healing rates between fracture locations and treatment choices, making this conversation challenging. We aimed to report the nonunion risk in nondisplaced scaphoid fractures when stratified by both treatment type and fracture location. METHODS: We searched PubMed, SCOPUS, CINAHL, SportsDiscus, HAND , Journal of Hand Surgery , and Plastic and Reconstructive Surgery for outcome studies of

The clinical application of individualized three-dimensional printing guiding template for assisting scaphoid fracture fixation through dorsal approach.

The purpose is to introduce a novel scaphoid screw placement surgery based on a novel three-dimensional (3D) printing guiding template through dorsal approach, and to evaluate the clinical feasibility and accuracy. The diagnose of scaphoid fracture was confirmed by Computed Tomography (CT) scanning, and subsequently the CT scanning data was input into a three-dimension imaging system(Hongsong software, China). An individualized 3D skin surface template with a guiding hole was printed. We put the template to the correct position on patient's wrist. Fluoroscopy was used to confirm the accurate p

Traumatic Nondissociative Carpal Instability: A Case Series.

PURPOSE: We report 8 cases of acute or subacute unilateral nondissociative carpal instability (CIND) in the context of nondisplaced scaphoid fractures. METHODS: Eight patients from 3 centers developed abnormal volar intercalated segment instability (VISI) or dorsal intercalated segment instability (DISI) following the diagnosis of a nondisplaced scaphoid fracture. An operative inspection in each patient confirmed intact scapholunate and lunotriquetral interosseous ligaments. We outline the demographic characteristics of our patient cohort, radiologic and operative findings of CIND-DISI and CI

Diagnostic Performance and Advanced Imaging Reduction With Digital Tomosynthesis in Scaphoid Fracture Management.

BACKGROUND: Management of scaphoid fractures often requires advanced imaging to achieve accurate diagnoses and appropriate treatment. Digital tomosynthesis (DTS) is a cross-sectional imaging modality that may be used to substitute magnetic resonance imaging or computed tomographic scans. The purpose of this study is to: (1) determine the diagnostic accuracy of DTS in occult scaphoid fractures; and (2) report on the reduction of other advanced imaging when using DTS. METHODS: From May 2014 to October 2017, the charts of all patients who underwent scaphoid tomogram were retrospectively reviewed

Multiphasic scaffold for scapholunate interosseous ligament reconstruction: A study in the rabbit knee.

Scapholunate interosseous ligament tears are a common wrist injury in young and active patients that can lead to suboptimal outcomes after repair. This research aims to assess a multiphasic scaffold using 3D-printing for reconstruction of the dorsal scapholunate interosseous ligament. The scaffold was surgically implanted in vivo in the position of the native rabbit medial collateral ligament. Two branches of treatment were implemented in the study. In the first group, the rabbits (n = 8) had the knee joint fixed in flexion for 4 weeks using 1.4 mm K-wires prior to sample harvesting. The secon

Management of scapholunate advanced collapse and scaphoid nonunion advanced collapse without proximal row carpectomy or four corner fusion.

Four corner arthrodesis and proximal row carpectomy are the most common techniques for the management of advanced radiocarpal arthritis due to longstanding scapholunate instability and scaphoid nonunion. The advantages and short comings of each technique have been well defined in the literature. Advancements in joint replacement and arthroscopic surgery have resulted in new operations to manage radiocarpal and midcarpal arthritis. Most of these new procedures are modifications of the two classical operations, but some use modern implants and newer materials. New individualized options, like os

[Rupture of the scapholunate intercarpal ligament].

Complete rupture of the scapholunate (SL) ligament can cause a dissociative carpal instability (CID). These ligamentous lesions are caused by a fall from a limited height of approximately 1 m or a fall, e. g. when playing handball or soccer. For a freshly injured wrist joint, the X‑ray signs of a static instability (after excluding a fracture) are a SL distance ≥3 mm, a SL angle >60°, and a dorsal displacement of the proximal scaphoid pole. Dynamic instabilities are best seen in kinematography of the wrist joint. Early ligament refixation is mandatory, ideally done in the first week after the

The Effect of Computed Tomography Scans Oriented Along the Longitudinal Scaphoid Axis on Measurements of Deformity and Displacement in Scaphoid Fractures.

PURPOSE: Reformatting computed tomography (CT) scans along the scaphoid longitudinal axis improves the ability to detect scaphoid fractures compared with reformats along the wrist axis. However, it remains unclear whether scaphoid axis reformats affect measurements of displacement or deformity, which are factors that drive the clinical decision to perform open reduction internal fixation. Our null hypothesis was that reformatting CT scans along the scaphoid axis does not affect measurements of fracture displacement and deformity. METHODS: Thirty patients with CT scans demonstrating scaphoid f

Treatment of Scaphoid Fractures: European Approaches.

Scaphoid fractures account for 2% of all fractures. In Europe, the incidence is 12.4/100,000/y. This article focuses on the European perspective on understanding and management of these injuries. These fractures occur in young, active patients. The aim of treatment is union. Osteoarthritis is almost inevitable if the fracture does not unite. Cast immobilization is the treatment of choice in occult or stable fractures with 90% to 95% healing. Acute/primary surgery may be considered in some patients. The European literature stresses the importance of taking the patient's wishes into consideratio

[Scaphoid fracture in a seven-year-old girl].

We report a case of a seven-year-old girl, who had a scaphoid fracture, which occurred when she fell off her bicycle. She was examined in the emergency room. X-ray did not show any sign of fracture of the scaphoid, but the fracture was later verified with a magnetic resonance imaging scan. She wore a shin cast for eight weeks and recovered completely. Very few cases of children aged under eight years and diagnosed with scaphoid fracture have been reported in the literature. Scaphoid fracture should be suspected and examined for in all children with relevant trauma and symptoms, regardless of a
