Posttraumatic Malunion Following Comminuted Scapular Fracture.

A 42-year-old male police officer sustained multiple injuries in a motorcycle accident, including a comminuted fracture of the scapula and several rib fractures. Four months after his injury, the patient was prescribed outpatient physical therapy, after 2 months of which the patient's cervical and lumbar spine function were restored but his shoulder mobility deficits persisted. A second physical therapy opinion was sought 6 months post injury for the left shoulder. Given his functional deficits, including an inability to return to work, he was referred to an orthopaedist for additional imaging

Anterior Capsule Augmentation and Posterior Glenohumeral Capsular Reconstruction With Human Dermal Allograft for Multidirectional Shoulder Instability.

Recurrent multidirectional shoulder instability is a common clinical presentation in certain demographics and sporting groups. One such demographic is patients with connective tissue disorders (CTD) such as Ehlers-Danlos syndrome (EDS), in whom shoulder pathologies are exacerbated owing to ligamentous laxity. Features of this laxity can present as both anterior and posterior shoulder instability, which are problematic sources of shoulder pain. Many patients with these injuries require surgical anterior and/or posterior glenohumeral reconstruction. Surgical reconstruction for posterior capsular

Electromyographic analysis of the shoulder muscles during depression transfers in subjects with low-level paraplegia.

OBJECTIVE: To document and compare the intensity of selected shoulder muscle activity during depression transfers. DESIGN: Intramuscular electrodes were used to record the activity of 12 shoulder muscles while transferring to and from a wheelchair. PARTICIPANTS: Twelve adult men with spinal cord injuries resulting in low paraplegia. OUTCOME MEASURES: Three phases of the transfer were analyzed: preparation, lift, and descent. Median intensities were compared between muscles, transfer phase, and direction of transfer using Freidman's test. RESULTS: Trunk elevation was accomplished mainly by

Musculoskeletal system injuries in the Polish Territorial Defence Forces.

BACKGROUND: Musculoskeletal system injuries (MSIs) are the major health problem of soldiers. The aim of this study was to assess the types of MSIs that occurred most frequently among soldiers and which parts of the body they generally affected. METHODS: The study included 140 Territorial Army soldiers, both men and women, serving in the Polish Territorial Defence Forces (TDF). Times and frequency of MSIs were assessed using an original questionnaire available via a web-based platform, the TDF Information Portal and the Yammer network. RESULTS: Among the participants, 42% reported MSIs, most

Re: Zyluk A. and Szlosser Z. The results of carpal tunnel release for carpal tunnel syndrome diagnosed on clinical grounds, with or without electrophysiological investigations: a randomized study. J Hand Surg Eur. 2013, 38: 44-9.

OBJECTIVES: The aim of this study was to describe, using video analysis, the mechanisms of head injuries and of incidents with a high risk of head injury in elite football. METHODS: Videotapes and injury information were collected prospectively for 313 of the 409 matches played in the Norwegian (2000 season) and Icelandic (1999 and 2000 season) professional leagues. Video recordings of incidents where a player appeared to be hit in the head and the match was consequently interrupted by the referee were analysed and cross referenced with reports of acute time loss injuries from the team medica

Upper brachial plexus injury in adults: comparative effectiveness of different repair techniques.

OBJECT: Adult upper trunk brachial plexus injuries result in significant disability. Several surgical treatment strategies exist, including nerve grafting, nerve transfers, and a combination of both approaches. However, no existing data clearly indicate the most successful strategy for restoring elbow flexion and shoulder abduction in these patients. The authors reviewed the literature to compare outcomes of the three surgical repair techniques listed above to determine the optimal approach to traumatic injury to the upper brachial plexus in adults. METHODS: Both PubMed and EMBASE databases w

Biologics in shoulder and elbow pathology.

In recent years, orthobiologics have been of increasing clinical interest in the treatment of shoulder and elbow pathology. In some conditions, such as rotator cuff injury and lateral epicondylitis, there have been high-quality trials that support the use of platelet-rich plasma in reducing pain, restoring functionality, and improving clinical outcomes. However, as the numbers of both cellular-based biologics and the conditions being augmented by biologics continue to expand, there is a substantial need for high-quality investigations to support their routine use in most shoulder and elbow con

Self-inflicted injuries: Three case reports.

The means by which people self-harm are diverse and so are the motives that drive them. They are intended for personal gain which may be psychological, legal or material. Fabricated injuries may be self-inflicted or permitted by others. We present three cases of fabricated injuries over the shoulder and upper limbs both alleging assault with a firearm. In all the cases meticulous examination elicited suspicion on how these were caused. Meticulous examination is essential to differentiate true from false accusations so that no innocent person is punished.

Humeral shaft fractures.

Fractures of the humeral shaft are common injuries with multiple management strategies. Many still regard nonoperative management as the standard of care; however, as the understanding of these injuries increases, treatment recommendations are also evolving. Fracture pattern, fracture location, and identifiable patient risk factors may predict poor outcome with nonoperative management, and earlier operative intervention may be recommended. Operative management includes open reduction and internal fixation through a variety of exposures, intramedullary nail fixation, and external fixation. With

The utility of musculoskeletal ultrasound in rehabilitation settings.

Aside from its prompt use in musculoskeletal injuries (sports lesions, degenerative/inflammatory joint disorders, and peripheral nerve pathologies), ultrasonographic imaging can be used quite conveniently in various types of rehabilitation conditions as well. If used in a rehabilitation setting, ultrasound can significantly contribute to the diagnostic/therapeutic algorithm of rehabilitation patients. Accordingly, in this article, the authors focus on the diagnostic/interventional utility of ultrasound particularly for shoulder problems, overuse injuries of wheelchair users, heterotopic ossifi

Transfer of the supinator muscle to the extensor pollicis brevis for thumb extension reconstruction in C7-T1 brachial plexus palsy.

With C7-T1 brachial plexus injuries, finger motion is absent while shoulder, elbow and wrist function are largely preserved. Previously, we have reconstructed finger flexion by transferring the brachialis muscle to the flexor digitorum profundus and flexor pollicis longus; and we have restored extension of thumb and finger by transferring the motor nerve to the supinator to the posterior interosseous nerve, which is only feasible in fresh injuries. We describe the transfer of the supinator muscle to the extensor pollicis brevis to reanimate thumb extension in patients with long standing C7-T1

[Trendy sport: golf].

Golf is currently the most frequently played sport in the world and it will continue to grow in importance. Nevertheless, there is a striking deficit in our knowledge of the physical and psychological profile of golfers. In the foreground are not the acute but the chronic sport specific injuries, predominantly of the lumbar spine, the upper extremities (golfer's elbow and golfer's shoulder), and less commonly of the lower limbs (minor injuries to the ligaments). In addition to therapy, prophylaxis should also be used in order to better recognise and influence the sport specific burdens.

Bilateral vocal cord hematomas associated with shoulder harness use.

A case of bilateral vocal cord hematomas caused by a shoulder harness injury is presented. The patient was restrained by a three-point belt system and was involved in a front-end collision. She presented with mild facial and chest injuries and a contusion of the neck. One hour after injury she began to complain of hoarseness without airway compromise. Fiberoptic laryngoscopy showed bilateral true vocal cord hematomas. The patient had an uneventful hospital course and a full recovery. The importance of the mechanism of injury and associated injuries is discussed.

Repair of isolated axillary nerve lesions after infraclavicular brachial plexus injuries: case reports.

Stretch injuries of the infraclavicular brachial plexus have a much better prognosis for spontaneous recovery than do their supraclavicular counterparts. We present three patients with stretch injuries of the infraclavicular brachial plexus who had spontaneous restoration of function in all muscles except the deltoid. Decreased shoulder abduction was a serious handicap to these individuals. At surgical exploration, each patient had an isolated, complete axillary nerve disruption at the quadrilateral space. Deltoid muscle function was restored in all three patients by repair of the axillary ner

Injuries of distal clavicular physis in children.

We report ten patients (all boys) with distal clavicular physeal injuries. Eighty percent of the injuries occurred on the right side. Nine boys were treated conservatively, but one required operation. Kirschner wire osteosynthesis was performed. Regardless of the type of treatment, all patients healed without functional sequelae. In seven of the nine treated conservatively, however, a deformity of the injured shoulder resulted. I suggest that in cases of great displacement of the central metaphyseal fragment resulting in shortening of the clavicle an open reduction with internal fixation is in

Association Between Shoulder Morphological Changes and Glenohumeral Maximum External Rotation During Late Cocking Phase.

BACKGROUND: Excessive external rotation of the glenohumeral joint during the late cocking phase of throwing is a factor in internal impingement; however, the relationship between maximum external rotation (MER) of the glenohumeral joint and morphological changes in the shoulder joint is unclear. PURPOSE: To clarify whether glenohumeral MER is associated with quantified assessment of morphological changes in the throwing shoulder joint. STUDY DESIGN: Descriptive laboratory study. METHODS: This study included 15 male university and adult baseball players from a competitive team. The posterior
