Osteoblastoma: clinical and radiologic findings in 98 new cases.

The data on 98 new patients with osteoblastoma were studied. The clinical features of pain, scoliosis, and neurologic deficit were largely consistent with those in previous reports. Osteoblastoma is usually a lytic lesion originating in the medulla of bones with matrix ossification and mild surrounding sclerosis. Osteoblastoma of the spine demonstrated better tumor delineation in the vertebrae, and a bony rim on the soft-tissue side was seen less frequently than previously observed. Osteoblastoma of the talus frequently appears as a blister on the surface of the bone and is accompanied by oste

Cervical Necrotizing Fasciitis, Diagnosis and Treatment of a Rare Life-Threatening Infection.

IMPORTANCE: Necrotizing fasciitis is a relatively uncommon and potentially life-threatening soft tissue infection, with morbidity and mortality approaching 25% to 35%, even with optimal treatment. The challenge of diagnosis for necrotizing soft tissue infections (NSTIs) is their rarity, with the incidence of approximately 1000 cases annually in the United States. Given the rapid progression of disease and its similar presentation to more benign processes, early and definitive diagnosis is imperative. FINDINGS: Signs and symptoms of NSTIs in the early stages are virtually indistinguishable fro

Group B Streptococcus-Associated Necrotizing Soft Tissue Infection Following Minor Ankle Trauma: A Case Report.

Necrotizing soft tissue infections (NSTIs) are rapidly progressive, life- and limb-threatening emergencies requiring immediate surgical source control. Group B Streptococcus (GBS; Streptococcus agalactiae) is a rare cause of NSTIs in adults. We present a 44-year-old woman with poorly controlled diabetes who developed progressive right foot pain and swelling after minor ankle trauma. Examination showed spreading erythema, crepitus, dusky discoloration, malodor, and a deep plantar wound, accompanied by marked leukocytosis and diabetic ketoacidosis. Computed tomography demonstrated infection exte

Chest wall reconstruction using Mersilene mesh and titanium rib plates after extensive tumour resection with immediate postoperative extubation: a case report.

A 36-year-old male presented with a progressively enlarging right anterior chest wall mass over one year. Preoperative biopsy revealed adenocarcinoma, with no definite primary tumor identified on thoracic imaging or evaluation of adjacent organs. Computed tomography demonstrated a large heterogeneous mass with rib destruction and extension into the surrounding soft tissue and pleura. The patient underwent en bloc resection of the involved chest wall, including the third to sixth ribs, followed by reconstruction using Mersilene mesh and titanium rib plates. The combined reconstruction provided

Atypical Presentation of Fournier Gangrene as Syncope in a Patient With Chronic Alcohol Use Disorder: A Case Report.

Fournier gangrene is a rapidly progressive necrotizing soft tissue infection of the perineal and genital regions associated with substantial morbidity and mortality. Chronic alcohol use disorder (AUD) is a recognized predisposing condition because of its effects on immune function, nutritional status, and hepatic function. We report a 63-year-old man with AUD who presented after a syncopal episode with abdominal pain and systemic inflammatory response syndrome criteria but without a documented perineal or genital complaint. Computed tomography (CT) of the abdomen and pelvis demonstrated soft t

Tuberculosis of the Cuboid Bone: An Unfamiliar Manifestation of a Familiar Disease.

This report discusses a patient with chronic ankle pain for 2 years. No respiratory symptoms were present, but her father had tuberculosis (TB) when she was four. Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were elevated. Empirical antibiotic therapy was initiated. The viral panel test was negative, while the purified protein derivative (PPD) test showed >20 mm induration. Chest radiograph and computed tomography (CT) scan were normal. Ankle radiograph revealed hypodense areas, osteolytic changes, and soft tissue swelling in the cuboid bone. Magnetic resonance imaging (MR

An aggressive gastric CIC-DUX4 sarcoma surgically resected with multivisceral organs: a case report.

BACKGROUND: Capicua transcriptional repressor-double homeobox 4 sarcoma (CDS) is a rare and aggressive malignant soft tissue tumor that typically arises within the soft tissues. We report an exceptionally rare case of a gastric CDS successfully resected despite its extensive invasion into surrounding organs. CASE PRESENTATION: A 48-year-old male presented with a progressively enlarging abdominal mass. Upper gastrointestinal endoscopy revealed a large ulcerative tumor on the posterior gastric wall. Biopsy results initially suggested a neuroendocrine cell carcinoma. Contrast-enhanced computed t

Computed Tomography Evaluation of Frozen or Glycerinated Bradypus variegatus Cadavers: A Comprehensive View with Emphasis on Anatomical Aspects.

Bradypus variegatus has unique anatomical characteristics, and many of its vascular and digestive tract aspects have yet to be clearly understood. This lack of information makes clinical diagnoses and surgical procedures difficult. The aim of this study was to evaluate the anatomical aspects of frozen and glycerinated corpses of B. variegatus using computed tomography (CT), emphasizing vascular and digestive contrast studies. Nine corpses that died during routine hospital were examined via CT in the supine position with scanning in the craniocaudal direction. In frozen cadavers, the contrast w

Application of Biodegradable Magnesium Membrane Shield Technique for Immediate Dentoalveolar Bone Regeneration.

For the first time, the clinical application of the first CE registered magnesium membrane is reported. Due to the material characteristics of magnesium metal, new treatment methodologies become possible. This has led to the development of a new technique: the magnesium membrane shield technique, used to rebuild the buccal or palatal walls of compromised extraction sockets. Four clinical cases are reported, demonstrating the handling options of this new technique for providing a successful regenerative outcome. Using the technique, immediate implant placement is possible with a provisional imp

A proximal type epithelioid sarcoma of the vulva with multiple distant metastases: A case report and review of the literature.

Epithelioid sarcomas (ESs) of the vulva are extremely rare soft tissue tumors characterized by an aggressive clinical course and a poor prognosis. The proximal type of ES occurs in the trunk and external genital area and has higher recurrence and distant metastasis rates than the distal type, which is found in the upper and lower extremities. We describe a case of a vulvar ES in a 24-year-old patient who was referred from the department of plastic surgery with protruding mushroom-like lesions in multiple areas, including the lower abdomen, whole vulva, anus, and both inguinal lesions. A biopsy

Identification of tumor nodule in soft tissue: An inverse finite-element framework based on mechanical characterization.

Identification and characterization of nodules in soft tissue, including their size, shape, and location, provide a basis for tumor identification. This study proposes an inverse finite-element (FE) based computational framework, for characterizing the size of examined tissue sample and detecting the presence of embedded tumor nodules using instrumented palpation, without a priori anatomical knowledge. The inverse analysis was applied to a model system, the human prostate, and was based on the reaction forces which can be obtained by trans-rectal mechanical probing and those from an equivalent

[Diagnostics and treatment of posterior shoulder instability].

Posterior shoulder instability has a markedly lower incidence than anterior shoulder instability. It has a wide spectrum of clinical symptom manifestations and the overwhelming number of patients lack a traumatic primary dislocation. In addition to a detailed medical history, a specific clinical examination with the help of standardized provocation tests is essential for the diagnostics. For the detection of structural posterior capsule and labral lesions in cases of chronic courses, magnetic resonance imaging (MRI) should be used with an intra-articular contrast agent. Malignant Metastasis Misdiagnosed as Osteoporotic Compression Fracture: A Case Report.

In cases of vertebral collapse after a trivial injury in elderly patients with severe osteoporosis, it can be a diagnostic challenge to determine whether the cause is a benign compression fracture or malignant metastasis. A 78-year-old male patient was referred to the emergency department for the evaluation of weakness of the left lower limb. He had undergone percutaneous vertebroplasty four months earlier after being diagnosed with L3 osteoporotic compression fracture. He was treated with foraminotomy at the L3-4 level after being diagnosed with foraminal stenosis two months earlier at a spin

The application of rapid prototyping to improve bone reconstruction in immediate dentoalveolar restoration: a case report.

PURPOSE: This article describes the use of rapid prototyping (RP) for diagnosis, planning, and execution of the reconstruction of hard and soft tissue in socket defects using immediate dentoalveolar restoration (IDR). SUMMARY: In cases of tissue loss in anterior dental areas, esthetic rehabilitation poses a major challenge with respect to treatment planning with the goal of long-term tissue maintenance. The IDR technique consists of immediate reconstruction in a single procedure of bone and soft tissue around implants placed immediately after extraction, and prosthetic rehabilitation. As this

Long standing lumbosacral dermoid tumor and intracranial fat droplet dissemination: A case report.

BACKGROUND: Dermoid tumors are slow growing, benign CNS lesions. CASE DESCRIPTION: This case study concerns a 29-year-old female with a 6-year history of lower extremity paresthesias attributed to magnetic resonance (MR)/computed tomography (CT) documented intradural dermoid tumor that extended from L1 to S1. On MR, it was hypointense on T1, hyperintense on T2, and did not enhance with gadolinium. CT showed hyperdensity at the L1-L2 levels. The craniocervical MR imaging showed small hyperintense foci in the cisternal space favoring "fat dissemination." L1-S2 laminectomy revealed an intradural

Pharyngeal airway changes after mono- and bimaxillary surgery in skeletal class III patients: Cone-beam computed tomography evaluation.

The aim of this study was to evaluate the two-dimensional (2D) and three-dimensional (3D) changes in the pharyngeal airway space (PAS) in 20 class III patients who underwent mono- or bimaxillary surgery using cone-beam computed tomography (CBCT). CBCT examination was obtained before surgery (T1) and at least 3 months after surgery (T2). The pharyngeal airway of each patient was studied at three levels: the level of the posterior nasal spine, the level of the most inferior point of the soft palate, and the level of the top of the epiglottis. At each of these levels, the anteroposterior and late

A systematic approach to CT evaluation of orbital trauma.

Computed tomography (CT) is widely used in the initial evaluation of patients with craniofacial trauma. Due to anatomical proximity, craniofacial trauma often involves concomitant injury to the eye and orbit. These injuries may have devastating consequences to vision, ocular motility, and cosmesis. CT imaging provides a rapid and detailed evaluation of bony structures and soft tissues of the orbit, is sensitive in detection of orbital foreign bodies, and often guides clinical and surgical management decisions in orbital trauma. For this reason, radiologists should be prepared to rapidly recogn

Muscle shape as a predictor of traumatic enophthalmos.

The literature on enophthalmos is reviewed to understand its etiology and its prevention following orbital fractures. Specifically, the importance of muscle shape changes in predicting enophthalmos is discussed. The indications for surgical repair of orbital blowout fractures are well established. However, 7 to 10% of patients still develop enophthalmos despite these criteria. Because late repair of enophthalmos is associated with poor esthetic and functional results, the sensitivity and specificity of the current indications need to be further improved. Increased orbital volumes after fractur
