Videofluoroscopy of swallowing in symptomatic patients who have undergone long-term intubation.

OBJECTIVE: The aim of this study was to evaluate whether specific patterns of swallowing dysfunction occur in symptomatic patients after long-term intubation. SUBJECTS AND METHODS: Twenty-one patients (16 men, five women; mean age, 66 years) who presented with clinical signs of aspiration after long-term intubation (mean duration, 24.6 days) underwent videofluoroscopy. They were analyzed for functional abnormalities of the tongue, soft palate, epiglottis, hyoid and larynx, pharynx, and the upper esophageal sphincter. We assessed the presence or absence of aspiration, the type of aspiration (p

Dysphagia in children with a 22q11.2 deletion: unusual pattern found on modified barium swallow.

OBJECTIVES: To delineate feeding dysfunction in a population of children with a 22q11.2 deletion and report the associated findings noted during the modified barium swallow (MBS). STUDY DESIGN: Seventy-five children with a chromosome 22q11.2 deletion and history of persistent feeding difficulty received a feeding evaluation, including an MBS for those children for whom there was concern about airway penetration. RESULTS: A consistent pattern of feeding difficulty, independent of palatal or cardiac involvement, emerged from the evaluations. This group typically has trouble coordinating the su

Kinetic magnetic resonance imaging analysis of swallowing: a new approach to pharyngeal function.

The emergence of turbo-FLASH MR sequences allows us to acquire five 10-mm sections each second and thus to catch images of the soft tissues during function. One can trace the pathway of a liquid between the tongue, the soft palate, the epiglottis and the pharyngeal apparatus and analysis the role of the anatomic structures during swallowing. Restricted to the sagittal plane for the purpose of this preliminary study, this technique can be extended to the other planes to provide a three-dimensional analysis of oropharyngeal function or dysfunction.

A preliminary comparison of videofluoroscopy of swallow and pulse oximetry in the identification of aspiration in dysphagic patients.

Pulse oximetry has recently received attention in the dysphagia literature because of its possible contribution to the management of neurogenic dysphagia. The present study was devised to examine whether pulse oximetry could be exploited to determine episodes of aspiration in patients with known dysphagia of neurologic origin. To this end, pulse oximetry was undertaken in six patients undergoing videofluoroscopic study of swallow. Normal controls also underwent pulse oximetry during feeding. The results indicate that there is no clear-cut relationship between changes in arterial oxygenation an

[Temporal and spatial pattern analysis of pharyngeal swallowing in patients with abnormal sensation in the throat].

There are many patients who complain of abnormal sensations, such as an obstructive sensation, foreign body sensation, difficulty in swallowing, etc., in the throat, which do not have an obvious cause. The causes of such unpleasant symptoms have not been adequately investigated. As one of the potential factors in volved in abnormal sensations in the throat, we considered the existence of subclinical dysphagia. We then performed videofluoroscopic analysis in patients with an abnormal sensation in the throat to examine whether or not there were functional disorders in swallowing. Videofluoroscop

Death, after swallowing and aspiration of a high number of foreign bodies, in a schizophrenic woman.

A 46-year-old woman who had had a long-term schizoid psychosis collapsed on the street. Upon admission to the hospital, she was determined to have an acute abdomen. The chest radiograph showed metallic foreign bodies in both main bronchi; foreign bodies in the stomach were not observed clinically. The woman died from repeated cardiac arrest shortly after hospital admission. At the autopsy a screw and a nail were found in both main bronchi. The abdominal cavity contained 2 L of greenish purulent fluid and a massive fibrinoid peritonitis was observed. Two perforations of the stomach, each 1 cm i

Swallow syncope associated with complete atrioventricular block: a case report and review of the literature.

Swallow or deglutition syncope is a very unusual but treatable disorder. A 44-year-old male presented with multiple, recurrent episodes of complete atrioventricular block associated with swallowing. Over a 24-hour period, 92 episodes of complete atrioventricular block occurring with meals were recorded by Holter monitoring. Evaluation did not demonstrate any underlying esophageal or cardiac disease. A trial of anticholinergic medications was unsuccessful. The patient's symptoms resolved following placement of a demand cardiac pacemaker. This is a rare but potentially lethal disorder that can b

Congenital difficulties with swallowing and breathing associated with maternal polyhydramnios: neurocristopathy or medullary infarction?

Two babies with congenital difficulties in swallowing and breathing are presented and contrasted. Both were associated with maternal polyhydramnios, but one was due to medullary infarction and the other to a malformation ("neurocristopathy"). In the former case, isolated tenth and twelfth cranial nerve palsies provided the clue as to the correct etiology, whereas the latter baby had dyscoordination of swallowing and breathing with sleep apnea ("Ondine's curse"). It is somewhat ironic that the case without a difficult delivery had the infarct, probably prenatal in onset, whereas the one with a

Perforation of the sigmoid colon by swallowed chicken bone: case reports and review of literature.

Two hundred and ninety-eight cases of intestinal perforation due to foreign bodies have been reported so far. The majority of perforations occur at narrowings and angulations. In the cases reported by McPherson, 83% of the perforation occurred in the ileum. It is more common in the elderly who wear dentures, in the mentally infirm and in chronic alcoholics. We present three cases of chicken bone perforation of the sigmoid colon with three different clinical manifestations: 1) Sigmoidovesical fistula: 2) Acute surgical abdomen; 3) Inflammatory mass. All the patients were elderly and wore dentur

Prospective evaluation of patient reported swallow function with the Functional Assessment of Cancer Therapy (FACT), MD Anderson Dysphagia Inventory (MDADI) and the Sydney Swallow Questionnaire (SSQ) in head and neck cancer patients.

OBJECTIVES: The Functional Assessment of Cancer Therapy (FACT) instrument is comprised of a group of related and overlapping quality of life (QoL) questionnaires including a core general form, head and neck cancer (HNC)-specific items, and an expert-selected index (FACT-HNSI). Understanding how these relate to more HNC-specific instruments such as the MD Anderson Dysphagia Inventory (MDADI) and Sydney Swallow Questionnaire (SSQ) is vital for guiding their use in clinical trials. MATERIALS AND METHODS: HNC patients concurrently completed MDADI, SSQ, and FACT questionnaires at radiation oncolog

Technology to remotely capture key elements of swallowing and speech: State of the science in Parkinson's disease.

Swallowing and speech impairments are common in Parkinson's disease (PD) and can significantly affect health and quality of life. Traditional methods for assessing and managing these functions rely on in-person visits and large, expensive equipment, which are inaccessible to many patients. Recent advances in health technologies offer opportunities for remote monitoring and assessment, enabling early detection, personalized intervention, and longitudinal tracking of swallowing and speech deficits in PD. This review synthesizes evidence on current and emerging technologies designed to remotely c

Engineering a Built-in Piezoelectric Electron Sponge in Swallow Nanozyme for Inflammatory Bowel Disease Therapy.

Inflammatory bowel disease (IBD) is characterized by chronic inflammation and excessive reactive oxygen species (ROS) production, leading to mucosal damage and impaired immune homeostasis. Here, we report the development of a core-shell structured barium titanate @ cerium oxide nanozyme (BTO@CeOx, x = 1.9, 1.8, and 1.85), functioning as an orally administered "barium meal" for targeted IBD imaging and treatment. Leveraging the inherent piezoelectric properties of BaTiO3, we imbue this nanozyme with a built-in piezoelectric electron sponge effect, enhancing its catalytic efficiency. Upon ultras

Prognosis of feeding and swallowing function in children and adolescents/young adults after corpus callosotomy.

BACKGROUND: Acute disconnection syndromes frequently occur after corpus callosotomy (CC), with poor oral intake being among the symptoms. Most patients achieve normal oral intake by 2 weeks postoperatively. However, in some cases, feeding and swallowing functions fail to recover, and risk factors for delayed improvement in oral intake remain unclear. This study aimed to investigate the frequency of delayed improvement in oral intake at 2 weeks after CC, to descriptively examine feeding and swallowing function outcomes at 6 months postoperatively, and to identify associated risk factors. Post-stroke dysphagia is associated with abnormal gamma power and phase synchronisation during swallowing.

Post-stroke dysphagia (PSD) has a series of complications that are associated with poor recovery and even mortality. Uncovering abnormal neural activities during swallowing bears critical implications for optimising treatment strategies for PSD patients. The current study was designed to identify the abnormal neural representations of swallowing in PSD. PSD individuals, stroke individuals without dysphagia, and healthy controls underwent a swallowing task while ongoing EEG was recorded. Compared to healthy controls, PSD patients demonstrated predominant gamma-band hypoactivity and hypoconnecti
