We reviewed 13 patients with end stage jumper's knee, 10 with patellar tendon ruptures, and 3 with ruptures of the quadriceps tendon to evaluate our long-term results in treating these tendon ruptures in an athletic population. The focus was on the natural history, the time until return, and the level of return, to athletic activity. Jumper's knee affected all patients to a varying degree prior to rupture. Specific reparative processes often dictate the function of a tendon upon completion of healing. At times the surgeon must improvise when the results of a previous traumatic or iatrogenic insult to a tendon produced a less than desirable result. The following is a case in which the authors utilized Silastic sheeting as a pseudosheath in the repair of an iatrogenic laceration of the extensor hallucis longus tendon in a previous surgery.

In patients with tennis elbow, the common extensor tendon showed a pronounced reactive change consisting of mesenchymal cell proliferation along with aggregates of newly formed vascular channels. When studied ultrastructurally, many of the mesenchymal cells adjacent to the vascular channels were surrounded by a basal lamina, which is not normally seen around tenocytes. Sixty-four children with tendon injuries about the foot and ankle have been reviewed in order to present a plan of management when such injuries occur. One-third involved the tendo Achillis; the others were equally distributed between the dorsum and sole of the foot, and in only 12% was more than one tendon involved. Sensory nerves were frequently severed, but nearly all recovered completely after early repair. The results of immediate repair of tendons were good. Two patients with patellar tendon rupture and underlying systemic disease are described. One had systemic lupus erythematosus and the other had chronic renal failure and secondary hyperparathyroidism. Experimental studies on the long extensor tendon of the thumb have shown that experimental rupture of the tendon occurs in the same region as in clinical cases. The application of a new plastic imprint method has demonstrated an area of smallest diameter in the tendon. This part is nearly identical with the usual site of the rupture, which can, therefore, be simulated in experiments.

Posterior tendon transfer to the dorsum of the foot through the interosseous membrans is an effective procedure for spastic equinovarus deformity. The complications of excessive calcaneal or excessive valgus deformity can be avoided if the transfer is not inserted too tightly with the foot in dorsiflexion and if it is not transferred to a cuboid insertion.
