Caldwell-Luc procedure for retrieval of displaced root in the maxillary sinus.

OBJECTIVE: The aim of this study was to describe the standard diagnostic procedure and the application of the Caldwell-Luc approach for the retrieval of a displaced root from the maxillary sinus and to share our experience in treating this complication. STUDY DESIGN: Twenty-four patients with a fractured root accidentally displaced into the maxillary sinus were referred by general dentists to our department from 2005 to 2008. All were managed by a standardized diagnostic procedure and a Caldwell-Luc approach. We recorded the age of each patient, the gender, the tooth, the size of root fragmen

Lymphangioma mimicking apical periodontitis.

INTRODUCTION: Lymphangiomas mimicking apical periodontitis, usually found in soft tissue of children and infants younger than 2 years, are extremely rare in aged patients, particularly in bone tissues. METHODS: An 81-year-old woman was seen in a radiological service to undergo imaging studies for the placement of dental implants. A panoramic radiograph showed a radiolucent multilocular structure in the left mandibular molar region, where teeth #17 and #18 had previously been treated endodontically. A fracture in tooth #17 was detected. Because of the patient's clinical history and the charact

Immediate, non-submerged, root-analogue zirconia implants placed into single-rooted extraction sockets: 2-year follow-up of a clinical study.

This study evaluated non-submerged, root-analogue zirconia implants with two different surfaces for immediate single-rooted tooth replacement in 18 patients. After tooth extraction the root was laser scanned and one-piece root analogue zirconia dental implants with one of two different surfaces were manufactured. In group A (n=6) the implant surface was roughened by sandblasting only, in group B (n=12) additional macroretentions limited to the interdental space, to avoid fracture of the thin buccal cortex, were designed prior to laser scanning. Implants were placed in the socket 1-8 days after

Structure and mechanical properties of the soft zone separating bulk dentin and enamel in crowns of human teeth: insight into tooth function.

The 200-300 microm soft zone of dentin, found beneath enamel in crowns of human teeth, is thought to fulfill important roles in tooth function, but little is known about its structure-mechanical relations. Scanning electron microscopy images of fracture surfaces showed that near the dentino-enamel junction (DEJ), a porous reticulate matrix of intertubular-dentin contains tubules with no peritubular lining. Peritubular-dentin however is found at some distance from the DEJ, and it gradually thickens with increasing depth into the bulk dentin. Concurrently, tighter packing of the collagen fibers

Incidence of dental trauma in the Western Australian School Dental Service.

A prospective study was undertaken to determine the annual incidence of trauma to the anterior permanent teeth of children enrolled in the Western Australian School Dental Service. The population comprised 66,500 6-12-yr-old children. 1.66 children and 2.05 teeth per 100 children received trauma. Of the children incurring trauma the incidence per 100 erupted teeth was 11.7. 80.5% of children traumatised one tooth only per incident, but 35% of all teeth that were traumatised involved trauma to two or more teeth. 88% of all traumatised teeth were central incisors. Girls received proportionately

Dentinogenesis imperfecta in the Brandywine isolate (DI type III): clinical, radiologic, and scanning electron microscopic studies of the dentition.

Teeth of seven patients from the Brandywine isolate who had dentinogenesis imperfecta (DI) type III were evaluated by clinical, radiologic, and scanning electron microscopic techniques. The deciduous and permanent teeth were opalescent, and there was marked attrition. Enamel pitting was present on some permanent teeth. Anterior open bites were found in all persons with complete permanent dentitions. Pulps of developing teeth were larger than normal during early development but rapidly became almost completely obliterated. There was increased constriction at the cementoenamel junctions. While r

Comparison of fracture resistance of fiber-reinforced post and core with different cementation techniques: in vitro study.

OBJECTIVE: The current study aimed to inspect the fracture resistance of fiber post to canal dentin using a different technique of cementation. MATERIALS AND METHODS: 60 sound single-rooted central incisors with comparable size and length were stored in normal saline. Each tooth was immersed in 5% sodium hypochlorite (NaOCl). The specimens were randomly divided into 6 groups of 10 specimens each. All included specimens received root canal treatment (RCT). Post-space preparation was done using Gates Glidden drills. Post space was standardized with 10 mm length, keeping 3 to 5 mm as an apical s

Multicenter RCT 6-Year Survival of Zirconia-Reinforced Lithium Silicate Endocrowns: 90° Shoulder Versus Butt-Joint Margins.

OBJECTIVES: Margin design significantly influences endocrown biomechanical performance. Nevertheless, long-term clinical data comparing margin designs within a singular zirconia-reinforced lithium silicate (ZLS) system remain scarce. This study reports the 6-year follow-up results of a multicenter, prospective, randomized, assessor-blinded superiority trial. The trial compares CAD/CAM ZLS endocrowns with 90° shoulder margins and butt-joint margins in endodontically treated molars (ETMs). METHODS: Two hundred ETMs in 200 patients meeting the inclusion criteria were randomly restored with chair

CBCT Use in Endodontic Diagnosis.

Endodontic diagnosis and treatment planning has taken a giant leap forward due to introduction of CBCT in dentistry. While conventional 2-D radiographs remain the most cost-effective and routine method to evaluate a patient’s dentition, their diagnostic potential is limited. The 3-D manipulation of images that CBCT offers provides better insight into diagnostic dilemmas and complicate treatment decisions. Despite the advantages of CBCT imaging, it should be used complimentary to 2-D radiography, not as a replacement. The principle of ALARA (in which patients should be exposed to radiation “as

Complicated late mandibular fracture following third molar removal.

Mandibular fractures are a rare complication after third molar removal. Symptoms show a wide variance. Treatment options range from prescription of a soft diet to surgical treatment by open reduction and internal fixation. This article describes a patient who presented a late mandibular fracture following third molar removal. The fracture was not detectable in radiographs at the time of fracture. Six weeks after the reported cracking noise, the patient presented a mandibular fracture with associated osteomyelitis. Treatment by open reduction and internal fixation and autologous iliac crest gra

[Problems of iatrogenic jaw fractures].

Iatrogenic fractures of the jaw were analyzed epicritically, and the attempt was made to determine whether fractures can be avoided and, if so, what type. Preoperative bases for complications in the course of the intervention are present in most cases so that the danger of an iatrogenic fracture of the jaw can be avoided by precise diagnostic radiology.

Review of Maxillofacial Hardware Complications and Indications for Salvage.

From 2002 to 2006, more than 117,000 facial fractures were recorded in the U.S. National Trauma Database. These fractures are commonly treated with open reduction and internal fixation. While in place, the hardware facilitates successful bony union. However, when postoperative complications occur, the plates may require removal before bony union. Indications for salvage versus removal of the maxillofacial hardware are not well defined. A literature review was performed to identify instances when hardware may be salvaged. Articles considered for inclusion were found in the PubMed and Web of Sci

[Sports and maxillofacial injuries: etiological and clinical aspects apropos of 46 cases. Preventive measures].

Maxillofacial injuries due to sporting activities represent approximately 10% of all cases of injury admitted to the University Hospital Center, Lyon, France, and more than half of these occur in football or rugby players. The causes of these accidents (blows against another player, the ground, the field equipment or the game accessories) are discussed. Lesions are generally simple in young, fit adults and results of surgical treatment satisfactory, apart from the rare cases of facial disruption. Preventive measures include examination for aptitude for sporting activities, adequate referring o

Residual root preparation
for socket-shield procedures:
a facial window approach.

Two common complications that occur following immediate implant placement and provisionalization procedures in the anterior maxilla are changes to the facial periimplant soft tissue levels and the facial contours. To counteract these changes, different techniques and treatments have been recommended. The recently introduced socket-shield technique (SST) appears to be a viable treatment option for stabilizing the facial osseous and gingival architecture; however, preparation of the root fragment can present challenges. This article describes a surgical approach designed to facilitate preparatio

Complications of third molar surgery and their management.

The frequency and severity of untoward events associated with surgical procedures are influenced by multiple factors that may be related to the procedure, patient, and/or surgeon. Not every third molar needs to be removed. Full bony impacted lower third molars well below the cervical margin of the second molar crowns should be considered for retention. Certain deviations from normal healing should be considered to be complications. Risk factors associated with third molar removal should be carefully established and explained to the patient. Third molar surgery has a predictable postsoperative

Cuspal reinforcement in endodontically treated molars.

UNLABELLED: This in vitro study compared the ability of horizontal pins and a dental adhesive to reinforce the facial cusps of endodontically treated mandibular molars. Seventy-two mandibular molars were divided into six groups and mounted in acrylic blocks (n = 12). In Groups 1-5 standardized endodontic access and instrumentation in the coronal one-third of each root canal were completed. In Groups 1-4 the lingual cusps were reduced, leaving the buccal cusps intact. The facial cusps of the teeth in each group received one of the following modes of reinforcement: Group 1--no reinforcement; Gro

A six-year follow-up study of sports-related dental injuries in children and adolescents.

The records were analyzed of 106 patients with sports-related dental traumas treated in 1983 at the public oral surgery unit in Helsinki, Finland; 51 were examined six years after injury. The mean age was 11.8 years (range 7-24 years). The woman/man ratio was 1:3. In 39% of cases, the injuries had arisen from ice hockey or skating; 30% happened during school hours; 80% were uncomplicated crown fractures, concussions or subluxations. During the six-year follow-up, of 80 teeth in 51 patients, root resorption was found in 6 teeth (7.5%), periapical lesions were noted in 2 teeth (2.5%), and oblite
