[Right accessory pathways with slow and decremential anterograde conduction. Electrophysiological changes during ablation of the proximal atrial poles].

Right sided accessory pathways with slow and decremential anterograde conduction differ from the anatomical substrates of the nodo-ventricular connections described by Mahaim. The connection may also be atrio-ventricular or atrio-fascicular. The authors report a series of 6 patients (2 men, average age 31 +/- 9 years) with antidromic reciprocating tachycardia implicating accessory pathways with decremential conduction (wide QRS complexes with left bundle branch block) the radiofrequency ablation of which was centered on the proximal atrial pole. The increment of pre-excitation was 75 +/- 18 ms

Classical response of antidromic atriofascicular tachycardia to premature atrial extrastimulus delivered during septal refractoriness.

A 23-year-old gentleman presented with a history of palpitations. The 12-lead electrocardiogram showed no manifest ventricular pre-excitation. Echocardiogram was within normal limits. A retrograde study showed concentric activation of the atrium with decremental conduction. Atrial pacing from right atrial free wall showed progressive pre-excitation. No anterograde nodal duality was documented.

[Total or partial electrical interruption of the atrioventricular pathway. Evaluation of clinical results and indications].

After checking on the efficacy and innocuity of catheter ablation of the His bundle in an experimental study in the sheep, 22 patients with supraventricular tachycardia resistant to an average of 5.5 +/- 1.7 antiarrhythmic drugs per patient underwent this procedure. The most common arrhythmias were atrial fibrillation or flutter (poorly tolerated in 14 cases), and reciprocating tachycardia (8 cases, including 3 with accessory atrioventricular pathways). In 3 of the 5 patients with intranodal tachycardia, an initial attempt was made to modify one of the two AV nodal conduction pathways. Althoug

Common Supraventricular and Ventricular Arrhythmias in Children.

The most common pediatric arrhythmias are tachycardias, and the most common type is supraventricular tachycardia, originating from or above the atrioventricular node and HIS bundle. Ventricular tachycardias are less common but more dangerous. Supraventricular tachycardias usually cause a narrow complex tachycardia unless there is a basal bundle branch block or rate-dependent aberration. A wide QRS tachycardia should be treated as ventricular tachycardias unless proven to be an supraventricular tachycardia with aberration. Diagnosis of both tachyarrhythmia types depends mainly on 12-lead electr

[Use of P-synchronized electrical stimulation of the heart for preventing supraventricular tachycardia paroxysms due to accessory conduction pathways].

The paper deals with a possibility to apply P-synchronized ventricular stimulation for the prevention of frequent, drugirresponsive paroxysms of supraventricular tachycardia (SVTC) due to the accessory conduction pathways. During the electrophysiologic study of 5 patients with SVTC paroxysms, the presence of Kent's bundle conducting the impulses in a retrograde way has been revealed. The observed mechanisms of SVTC occurrence (delayed His-Purkinye system conduction in 2 patients and atrioventricular nodal conduction in 3 patients) enabled its prevention to be achieved by a method of ventricula

Right Side Accessory Pathway Mediated Cardiomyopathy Treated with Amiodarone: First Adult Case Report.

BACKGROUND: Right-sided accessory pathways can cause ventricular dyssynchrony through early right ventricular preexcitation, mimicking left bundle branch block and leading to reversible cardiomyopathy even without sustained tachycardia. Although catheter ablation is standard, pharmacologic reversal in adults has not been reported. DATA SOURCES: Single-patient case from a US community hospital. Literature review (PubMed/Embase) confirmed no previous adult cases of complete pharmacologic reversal. MAIN FINDINGS: A 44-year-old man with known Wolff-Parkinson-White presented with palpitations, dy

Successful elimination of concealed accessory pathway-mediated tachycardia by ablation of AV nodal slow pathway.

We report a case of atrioventricular reentrant tachycardia (AVRT) using a concealed para-Hisian accessory pathway for retrograde conduction, which also required anterograde conduction over the AV nodal slow pathway to maintain the tachycardia. The shortest VA interval during AVRT (70 ms) was noted at a site with His bundle electrogram amplitude of 0.25 mV. The AVRT was cured by radiofrequency ablation of the AV nodal slow pathway without affecting accessory pathway conduction. The patient has not reported any sustained palpitations at 2 years after ablation while receiving no medications. The

Concealed retrograde bypass tracts and enhanced atrioventricular nodal conduction. An unusual subset of patients with refractory paroxysmal supraventricular tachycardia.

The clinical and electrophysiologic features of eight patients with unusually rapid, medically refractory paroxysmal supraventricular tachycardia are described. Exercise induction of tachycardia and functional bundle branch block patterns during tachycardia were common. Tachycardia resulted from anterograde enhanced atrioventricular nodal conduction combined with retrograde conduction by a concealed left atrial-left ventricular accessory pathway producing rates ranging from 200 to 300 beats/min. Management and late follow-up study were characterized by generally unsuccessful electrophysiologic

Oncocytic cardiomyopathy of infancy with Wolff-Parkinson-White syndrome and ectopic foci causing tachydysrhythmias in children.

Two female infants, ages 6 months and 13 months, were first seen in the newborn period with supraventricular tachycardia associated with Wolff-Parkinson-White syndrome. One infant had echocardiographic and angiographic evidence of diffuse cardiomyopathy and died suddenly at home. The other infant was seen initially at 13 months of age with refractory ventricular tachycardia and died following surgical resection of arrhythmogenic foci on the left and right ventricles. Autopsy showed diffuse patchy oncocytic cardiomyopathy in both instances. Serial histologic sections of the cardiac conduction s

Utilization of 3D mapping systems in interventional electrophysiology and its impact on procedure time and fluoroscopy-Insights from the "Go for Zero Fluoroscopy" project.

AIM: The implementation of 3D mapping systems plays an important role in interventional electrophysiology (EP) in recent years. The aim of the present study was to evaluate use of 3D mapping systems regarding fluoroscopy and procedure duration. METHOD: In the "Go for Zero Fluoroscopy" project 25 European centers provided data of consecutive EP procedures. Data on use of 3D mapping systems as well as utilization of contact force catheters and multipolar mapping catheters were associated with fluoroscopy time, dose area product (DAP), and procedure duration. RESULT: A 3D mapping system was use

Multiple Ablation Targets in Children: Multiple Accessory Pathways and Coexistent Arrhythmia.

The coexistence of different mechanisms of arrhythmia and multiple accessory pathways (MAPs) leading to multiple ablation targets is rarely seen in children, and data regarding these patients in the literature are limited. Herein, we aimed to evaluate patients who required multiple ablation applications, focusing on different targets during the procedures in children, and evaluating the characteristics of coexistent arrhythmia and MAPs, and the results of these procedures in children. Ablation procedures conducted between March 2009 and December 2018 were evaluated retrospectively, and patient

Radiofrequency ablation of a concealed left-sided accessory pathway in the mid-coronary sinus after failed conventional ablation attempts.

The case of a 71-year-old female patient with recurrent supraventricular tachycardia is reported. During the initial electrophysiological study, an orthodromic atrioventricular reentrant tachycardia utilizing a concealed left-sided accessory pathway (AP) was induced. Radiofrequency (RF) ablation via a retrograde aortic approach was unsuccessful. A second ablation attempt 6 weeks later via a transseptal approach using the EnSite Precision three-dimensional (3-D) cardiac mapping system and an irrigated-tip RF ablation catheter also failed to ablate the AP. Therefore, mapping in the coronary sinu

[Supernormal and alternating conduction in intermittent bundle branch block and intermittent or concealed ventricular preexcitation. Electrophysiological study, mechanisms and clinical considerations].

BACKGROUND: Supernormal and alternating conduction are not rare in clinical arrhythmology, detectable at various levels of the conduction system in different pathophysiological conditions, and often associated, so as to justify the search for a possible link between them. METHODS: In order to define a possible relationship between supernormal and alternating conduction, the electrophysiological data of two patient groups were analyzed. Group 1 included 9 patients with intermittent bundle branch block in the presence of supernormal conduction through the pathological branch. Group 2 included 1

Radiofrequency catheter ablation of septal accessory pathways within the triangle of Koch: importance of energy titration testing other than the local electrogram characteristics for identifying the successful target site.

Radiofrequency (RF) catheter ablation of accessory atrioventricular (AV) connections in the proximity of His bundle or AV node is at high risk of developing complete heart block. A safe and effective protocol has not been well established. Nineteen consecutive patients with 19 septal accessory pathways within the triangle of Koch underwent a protocol with power-titrated RF energy testing to identify the target site for successful catheter ablation. At every potential target site preselected by local electrogram characteristics, RF energy was started at 5 W for 10 seconds, with an increment of

[Atrial fibrillation and regular tachycardia in two young patients--are both treated with atrial fibrillation ablation?].

Two young patients with documented episodes of symptomatic paroxysmal atrial fibrillation have been referred for pulmonary vein isolation. Both patients had a history of concomitant regular tachycardia. in both cases pulmonaryvein isolation has not been performed but in each patient a supraventricular tachycardia (right focal atrial tachycardia/atrioventricular reentry tachycardia) could be induced and ablated instead. Concomitant supraventricular tachycardia acting as a trigger arrhythmia is frequent in young patients with atrial fibrillation without underlying cardiacdisease. In these patien

Successful Epicardial Ablation of a Right Infero-paraseptal Accessory Pathway Presenting as Ventricular Fibrillation.

A 22-year-old man presented with aborted sudden cardiac arrest due to ventricular fibrillation and recurrent tachyarrhythmia related to a right inferoparaseptal accessory pathway. A previous electrophysiology study with both endocardial and epicardial ablation had failed. Repeat electrophysiology study demonstrated diffuse early activation on endocardial mapping but a focal earliest site during epicardial mapping. The successful ablation site showed a sharp QS in the unipolar electrogram with a contact force vector directed toward the visceral pericardium/myocardium and required careful cathet
