Impaired Smooth Pursuit During Transient Global Amnesia.

BACKGROUND AND PURPOSE: During transient global amnesia (TGA), selective impairment of episodic memory is assumed to occur due to alteration in the neuronal network between the hippocampus and parietooccipital cortices that also include a hub for smooth pursuit (SP) eye movements. This study aimed to determine whether SP is impaired during TGA, and to identify any anatomical and functional linkage present between the oculomotor and memory systems. METHODS: Within a median of 1.0 day of TGA, horizontal SP was evaluated in 145 patients with a target moving at peak velocities of 10°/s and 20°/s.

Transient global amnesia and its aeromedical implications.

INTRODUCTION: Transient global amnesia (TGA) is an episode of severe anterograde amnesia of sudden onset, characteristically lasting 4 to 6 h. Patients become disorientated in time and place, but not in person. Automated motor tasks are preserved; however, the retention of new information is impaired, with clear implications to a pilot's fitness to fly. This study examines the United Kingdom Civil Aviation Authority (UK CAA) experience of pilots with TGA and compares it to the medical literature. METHODS: The UK CAA medical records database was searched for subjects who had reported a history

'Les ictus amnésiques' and transient global amnesia.

In March 1909, R. Benon was probably the first to report a typical case of what we now call transient global amnesia. In 1956, Bender, and independently, Guyotat and Courjon described clinical and epidemiological features of transient amnesic attacks. The condition achieved general recognition after the term transient global amnesia (TGA) was introduced by Fisher and Adams in 1958. Their historic work is the main focus of this review. They reported 17 patients, with an abrupt anterograde amnesia of short duration. Classification and criteria are outlined. Various aetiologies have been postulat

Recurrent transient global amnesia with intrathecal baclofen.

A middle-aged woman began experiencing spells of profound anterograde amnesia several months after beginning intrathecal baclofen treatment for generalized dystonia. Her spells met criteria for transient global amnesia, but were unusual because of their frequent recurrence and because their frequency was somewhat dose-dependent on baclofen. Fludrocortisone decreased the frequency of these episodes, and sublingual nitroglycerin both prevented and terminated them. Baclofen-induced amnesia in rodents is a reliable model of memory impairment. In contrast, baclofen-induced memory impairment in huma

Motor cortex excitability in transient global amnesia.

OBJECTIVE: To investigate the physiology of motor cortical areas in patients with transient global amnesia (TGA). MATERIALS AND METHODS: We performed transcranial magnetic stimulation (TMS) and single photon emission computed tomography (SPECT) in 13 patients during and after the acute phase of a typical episode of TGA. Measures of cortical excitability included motor threshold (MT) to magnetic stimulation, cortical silent period (SP) duration and intracortical inhibition (ICI) using a paired-pulse TMS technique. RESULTS: We found thalamic hypoperfusion and an ipsilateral significantly decre

Transient global amnesia: a quantified electroencephalographic study.

In 13 patients who experienced transient global amnesia (TGA), quantified electroencephalographic (qEEG) records were carried out within 1 week of the episodes, and compared with those in 13 cases with cerebral transient ischemic attacks (TIA), and 13 normal controls, matched for age and sex. A statistical comparison of the absolute power showed few differences between TGA and TIA patients, and many statistical differences between TGA patients and controls. The qEEG pattern reveals an involvement of the temporo-parietal structures in TGA patients. These findings suggest the presence of subclin

Transient global amnesia. A case control study.

A case control study of transient global amnesia (TGA), transient ischaemic attacks (TIA) and normal controls is described. Each of the 51 TGA patients, selected between January 1985 and March 1990, was compared with four controls (two TIAs and two normals) for the presence of vascular risk factors (hypertension, diabetes, smoking habits, cholesterol, triglycerides and haematocrit levels, heart disease, previous stroke), previous TGA, migraine, psychiatric illness and recent head trauma. Patients with TGA had less diabetes, hypercholesterolaemia and hypertriglyceridaemia than TIA. TGA subjects

[Transient global amnesia syndrome of traumatic origin].

The authors describe the condition of a 52-year-old female patient who during a car crash contracted an injury manifested by typical symptoms of transient global amnesia for a period of nine hours. There was no previous disorder of consciousness or other traumatic changes. Other similar nine cases from the literature are quoted.

Ethmoidal meningioma revealed by transient global amnesia.

A patient in whom transient global amnesia (TGA) led to the diagnosis of an ethmoidal meningioma is described. One year after neurosurgery, the patient showed an impairment of long-term memory, without any clinical or neuroradiological sign of relapse. We suggest that TGA may express a preexisting subclinical impairment of memory neuronal systems.

Transient global amnesia and left frontal haemorrhage.

A patient developed spontaneous, acute, dominant frontal lobe haemorrhage neighbouring on a zone of pre-existing post-traumatic encephalomalacia manifesting clinically as transient global amnesia. Amnesia can be secondary to disease of the frontal lobe, affecting pathways interconnecting the basal forebrain and hippocampus of the temporal lobe.

Transient global amnesia: clinical characteristics and prognosis.

We studied the clinical characteristics of transient global amnesia (TGA) in 277 patients with an average follow-up of 80 months. The syndrome occurred most frequently after age 50. There was a history of migraine in 14.1% and cerebrovascular diseases in 11.2% of patients, but these conditions were usually not temporally linked to TGA. Characteristic antecedent events and activity such as exertion existed in 33.4%. The incidence of TGA was 5.2 per 100,000 per year in Rochester, MN. Although 23.8% of the patients had recurrent episodes, they were not at increased risk for subsequent stroke.
