Dysmyelopoiesis in the cat: a hematological disorder resembling refractory anemia with excess blasts in man.

Clinical and pathological findings in three cats affected with a myelodysplastic disorder are presented. This hematological disorder resembles that of refractory anemia with excess of blasts as seen in man. The hematological profile in man is one of peripheral cytopenia in one or all of the marrow cell lines which occurs despite a normal to hypercellular bone marrow. Quantitatively, the marrow has a preponderance of blasts (up to 20%). Qualitative abnormalities consist of dysthrombopoiesis and/or dyserythropoiesis and/or dysgranulopoiesis. Myelodysplastic disorders in the cat are a form of mar

Deficiency of a major myeloid antigen in neutrophils of patients with refractory anemia and excess myeloblasts.

The expression of a major granulocyte-related antigen was assessed on the peripheral blood neutrophils of patients with the syndrome of refractory anemia with excess blasts. Of thirteen patients studied six showed a marked decrease in the number of cells with detectable staining. These results indicate that, although the neutrophils may appear normal by standard morphological criteria in this disease, they may be defective in a major component associated with myeloid maturation.

Neonatal-Onset Refractory Thrombocytopenia and Anemia Associated With a Homozygous In-Frame NFE2 Duplication.

BACKGROUND NFE2 encodes p45/NF-E2, a transcriptional regulator of megakaryocyte maturation and platelet production. However, germline NFE2-associated cytopenias are rare; consequently, the clinical spectrum is incompletely defined. CASE REPORT We report the case of a preterm neonate born to consanguineous parents who developed day-1-onset bicytopenia with transfusion-dependent thrombocytopenia and intermittent normocytic anemia. Initial evaluation did not support immune-mediated or consumptive causes; moreover, counts were refractory to intravenous immunoglobulin and romiplostim (a thrombopoie

5q- in a child with refractory anemia with excess blasts: similarities to 5q- syndrome in adults.

A 19-month-old boy was referred to our institution because of chronic macrocytic anemia and severe thrombocytopenia. At age 17 months, he had developed petechiae. He had a leukocyte count of 4.4 x 10(9)/L, hemoglobin concentration of 7.9 g/dL, packed cell volume of 21%, mean corpuscular volume of 101 fL, and platelet count of 19 x 10(9)/L. At the time of referral, a bone marrow aspirate and biopsy revealed myelodysplastic changes that included megakaryocytic hyperplasia with hypolobated megakaryocytes, megaloblastoid erythropoiesis, 12% blast cells, and bone marrow fibrosis; the diagnosis was

[Non-specific esterase positive neutrophils in a case of refractory anemia with excess of blasts].

A 61-year-old woman was admitted with complaint of fever. The peripheral blood showed pancytopenia and bone marrow aspirate showed dysplasia in trilineage blood cells with increased blasts (18.2%). Bone marrow chromosome study revealed a karyotype of 46XX, -6, 3q-, +mar in 19 cells of 20 analyzed. She was diagnosed as refractory anemia with excess of blasts (RAEB). 95% of neutrophils in the bone marrow and 84% of that in the peripheral blood were stained with non-specific esterase using alpha-naphthyl acetate as substrate. On the other hand, the positivity of neutrophils for peroxidase. Sudan

The bone marrow karyotype in seventeen cases of refractory anemia with excess of blasts (RAEB).

The bone marrow karyotype was studied in 17 patients affected by refractory anemia with excess of blasts (RAEB). A cytogenetic finding common to nearly all the cases is hypodiploidy, present in even quite high percentages. In 3 patients were identified the same interstitial deletion of the long arm of a chromosome 5, present in a percentage varying between 30% and 65%. The presence of this chromosomal anomaly appears to correspond to a slower development of the illness and transformation into acute leukemia seems rare or delayed.

Identification of genes underlying different methylation profiles in refractory anemia with excess blast and refractory cytopenia with multilineage dysplasia in myelodysplastic syndrome.

BACKGROUND: Myelodysplastic syndrome (MDS) is a preleukemic condition that transforms into acute myeloid leukemia. However, the genetic events underlying this transformation remain poorly understood. Aberrant DNA methylation may play a causative role in the disease and its prognosis. Thus, we compared the DNA methylation profiles in refractory anemia with excess blast (RAEB) to those in refractory cytopenia with multilineage dysplasia (RCMD). METHODS: Bone marrow samples were collected from 20 patients with primary MDS (9 with RAEB and 11 with RCMD), and peripheral blood samples were collecte

A diagnostic pitfall in iron-refractory microcytic hypochromic anemia with acquired ring sideroblasts initially treated as iron deficiency anemia-a case report.

Microcytic hypochromic anemia is commonly presumed to represent iron deficiency anemia (IDA) at initial presentation. However, failure to respond to iron supplementation and iron indices inconsistent with IDA should prompt early diagnostic reassessment. We report a 67-year-old woman with severe microcytic hypochromic anemia after ineffective iron therapy. Iron studies indicated iron loading with impaired iron utilization rather than iron deficiency. Bone marrow Perls staining revealed ring sideroblasts in 14% of erythroblasts. Further stratified evaluation supported an acquired ring sideroblas

Anifrolumab for refractory autoimmune hemolytic anemia in SPENCD-associated lupus: a case-based review.

Spondyloenchondrodysplasia (SPENCD) is a rare monogenic interferonopathy caused by biallelic ACP5 mutations, characterized by skeletal dysplasia, neurological involvement and immune dysregulation frequently manifesting as lupus-like disease. Autoimmune cytopenias, particularly autoimmune hemolytic anemia (AIHA), are among the most severe and treatment-refractory manifestations. Anifrolumab, a monoclonal antibody targeting interferon alpha receptor subunit 1 (IFNAR1), blocks signaling by all type I interferons and is approved for moderate-to-severe systemic lupus erythematosus. We report a 50-y

Sirolimus for refractory/relapsed warm autoimmune hemolytic anemia and Evans syndrome: a prospective study.

Effective therapeutic options remain limited for adults with relapsed or refractory (R/R) warm autoimmune hemolytic anemia (wAIHA) and Evans syndrome (ES). We conducted a prospective, single-center, open-label clinical trial to evaluate the efficacy and safety of sirolimus. Adult patients with R/R wAIHA or ES and hemoglobin levels <100 g/L received oral sirolimus (1-3 mg/day; target trough concentration, 4-15 ng/mL) for ≥6 months. The coprimary end points were overall response rate (ORR) and complete response rate (CRR) at 6 months; secondary end points included 12-month efficacy, safety, and

Long-Term Outcomes in Patients with Relapsed or Refractory Severe Aplastic Anemia after Immunosuppression.

BACKGROUND: Sickle cell disease (SCD) predisposes patients to gallstone formation due to chronic hemolysis, which may result in complications such as cholecystitis and pancreatitis. Despite its clinical importance, there is limited data on the prevalence and risk factors of gallstones in children and adolescents with SCD in Uganda. This study aimed to determine the prevalence and associated factors of gallstones in children and adolescents with SCD in Eastern Uganda. METHODS: A hospital-based cross-sectional study was conducted at Mbale Regional Referral Hospital from March to June 2024. We e

Overcoming transfusion-refractory anemia: partial splenic embolization to facilitate orthopedic surgery in β-thalassemia.

β-thalassemia is a hereditary hemolytic disorder. Non-transfusion-dependent patients typically present with mild anemic symptoms under normal conditions, but may develop severe anemia during stress states. Moreover, treatment is often challenging due to concurrent hypersplenism. This article reports a case of a young male patient with non-transfusion-dependent β-thalassemia who was admitted to the hospital due to a closed fracture of the right femur. After admission, the patient developed severe anemia and showed a poor response to blood transfusion therapy. Considering the association with hyp
