Can anesthesia improve the outcome in high risk obstertric patients?

Obstetric anesthetists should have an infrastructure that allows for referral and assessment of high risk patients. Management plans should be agreed well before delivery. This information must be available to other members of the team. Protocol for common high risk problems should be agreed and introduced. Promoting regional blockade for Cesarean section will reduce maternal anesthetic mortality. Epidural anesthesia preserves fetal biochemistry better than other forms of anesthesia.

Foreign body aspiration in children - being safe and flexible.

PURPOSE OF REVIEW: Anesthesia for foreign body removal in children can be quite challenging. Even though rigid bronchoscopy is considered the gold standard for foreign body removal, there is increasing evidence for successful foreign body removal using flexible bronchoscopy. This review discusses the recent implications for flexible bronchoscopy for the purpose of foreign body removal and will compare these findings to rigid bronchoscopy. RECENT FINDINGS: During the last few years, several observational studies on foreign body removal by flexible bronchoscopy have been published, with promisi

[Respiratory failure from upper airway collapse following anterior cervical spine surgery].

OBJECTIVES: To determine the frequencies of variables that might predispose to upper airway collapse in a series of patients undergoing anterior cervical spine surgery. PATIENTS AND METHODS: Retrospective review of the medical records of 204 patients who underwent anterior cervical spine neurosurgery between 2003 and 2009. We gathered information on perioperative variables that might be related to upper airway collapse, on whether intensive care unit admission was planned or not, and on the moment when obstruction developed. RESULTS: Partial obstruction occurred in 7 cases (3.4%); 4 (1.9%) r

Prospective model-based comparison of different laryngoscopes for difficult intubation in infants.

BACKGROUND: Difficult intubation in infants is uncommon but may be a challenge for the anesthesiologist. Many optical-assisted techniques are available to ease endotracheal placement of tube but have not been systemically evaluated for pediatric practice. AIM: The study was performed to compare conventional pediatric Macintosh - with different optical laryngoscopes in difficult endotracheal intubation in infants. We hypothesized that inexperienced anesthetists would perform more successful with optical devices and that differences between the devices would be found. METHODS/MATERIALS: In thi…

A Rare Event With High Acuity: A Case of Autonomic Dysreflexia.

Autonomic dysreflexia (AD) presents a unique circumstance that many learn about in school but seldomly see in clinical practice. In this case study, AD is identified in a higher-level spinal cord injury (SCI) than what is typically imagined. In this case, physical and pharmacologic techniques are used to mitigate AD. Additionally, the procedure is still performed using various pharmacologic agents and collaborative efforts from the surgical and anesthesia team. A unique discussion on differentials and treatment options for AD is then presented. In conclusion, anesthesia providers must recogniz

Peri-operative cardiac arrest due to suspected anaphylaxis as reported to the 7th National Audit Project of the Royal College of Anaesthetists.

The 7th National Audit Project (NAP7) of the Royal College of Anaesthetists studied peri-operative cardiac arrest. Among 59 cases reported as possible anaphylaxis, 33 (56%) were judged to be so by the review panel with high or moderate confidence. Causes in excluded cases included: isolated severe hypotension; bronchospasm; and oesophageal intubation. Severe bronchospasm leading to cardiac arrest was uncommon, but notably in one case led to a reported flat capnograph. In the baseline survey, anaesthetists estimated anaphylaxis as the cause of 10% of cases of peri-operative cardiac arrests and

Australian and New Zealand Anaesthetic Allergy Group/Australian and New Zealand College of Anaesthetists perioperative anaphylaxis management guideline 2022.

Perioperative anaphylaxis is a potentially life-threatening emergency that requires prompt recognition and institution of life-saving therapy. The Australian and New Zealand College of Anaesthetists and Australian and New Zealand Anaesthetic Allergy Group have partnered to develop the anaphylaxis management guideline along with crisis management cards that are recommended for use in suspected anaphylaxis in the perioperative setting. This is the third version of these guidelines with the second version having been published in 2016. This article contains the revised Australian and New Zealand

Unusual case of a leiomyoma in the space of Retzius mimicking broad ligament fibroid.

This case report will discuss an interesting case of a patient who presented with pain in the lower abdomen of 2-month duration. Clinical examination and imaging findings were suggestive of a broad ligament fibroid. However, intraoperatively, the mass was found to be present in the space of Retzius, which is an extremely rare location for such a pathology. The access to the tumour was very challenging. Successful surgical excision was done with the help of a multidisciplinary team involving a gynaecologist, a urologist and an anaesthetist. Histopathology reported it to be benign leiomyoma. Pos

Advanced diagnostic and therapeutic techniques for anaesthetists in thoracic trauma: an evidence-based review.

Anaesthetists play an important role in the evaluation and treatment of patients with signs of thoracic trauma. Anaesthesia involvement can provide valuable input using both advanced diagnostic and therapeutic interventions. Commonly performed interventions may be complicated in this setting including airway management, damage control resuscitation, and acute pain management. Anaesthetists must consider additional factors including airway injuries, vascular injuries, and coagulopathy when treating this population. This evidence-based review discusses traumatic thoracic injuries with a focus on

Prevention of Nausea and Vomiting After Laparoscopic Sleeve Gastrectomy: Are We Doing Enough?

The worldwide obesity epidemic has led to an increase in the number of bariatric procedures for treatment. Currently, the most commonly performed bariatric surgical operation in the United States is the laparoscopic sleeve gastrectomy (LSG). The risk of postoperative nausea and vomiting (PONV) is exceptionally high following the LSG: as much as 65%. Anesthesia providers should be mindful of this elevated risk and stay up to date on evidence-based techniques that help to mitigate the incidence of PONV related to this specific procedure. The purposes of this clinical review are to increase provi

Prevention and Treatment of Laryngospasm in the Pediatric Patient: A Literature Review.

Laryngospasm is an exaggeration of a protective reflex that prevents aspiration of foreign objects into the lower airway (eg, during swallowing). This results in complete or partial closure of the glottis, and impedance or total obstruction of airflow to the trachea and lungs. Often, the resulting hypoxia will by itself break a laryngospasm; however, if the spasm continues without relief, it can lead to pulmonary edema, cardiac dysrhythmias, cardiac arrest, and ultimately death. This evidence-based literature review explores the pathophysiology of laryngospasm and covers mechanical and pharmac

Anesthetic implications of necrotizing fasciitis.

Necrotizing fasciitis, also known as necrotizing acute soft tissue injury (NASTI), commonly occurs as a consequence of group A streptococcal disease. An estimated 9,400 cases of invasive group A streptococcal disease occur annually in the United States, with 600 cases classified as NASTI. Mortality associated with NASTI is estimated at 20% to 50%. Research indicates that early diagnosis and surgical excision of necrotic tissue is the key to minimalization of morbidity and mortality associated with NASTI. Repeated surgeries typically are required in patients with NASTI, posing unusual anestheti

A case report of tracheal tenting to the skin: An option to tracheostomy for post-thyroidectomy tracheomalacia.

Operating on a huge and long-standing goiter is challenging to the surgeon and anesthetist because of the possibility of tracheomalacia and collapse of the tracheal rings after extubation. We report our innovation of tenting the trachea to the skin using sutures (passed through the strap and sternocleidomastoid muscles) to prevent post-thyroidectomy tracheomalacia.

Anesthesia and the porphyrias.

The porphyrias are a result of disordered heme metabolism. The sequelae are related to the accumulation of precursors and/or their metabolism. Induced drug metabolism can lead to acute attacks and may result in fatalities. Careful selection of intravenous agents, absolute avoidance of barbiturates, and use of inhalation agents can enable the anesthetist to administer safe anesthesia to these endangered patients.

Case Report: Hemodynamic challenges during the anesthetic management of a patient who presented with a cervical vagal schwannoma in southern Ethiopia: a rare clinical case.

BACKGROUND: Vagal schwannoma is a benign tumor formed from the Schwann cells of the vagus nerve. Therefore, surgery is a viable treatment option for patients requiring tumor excision. However, anesthetic management of patients presenting with vagal schwannoma may pose a significant challenge to anesthetists because of an increased risk of intraoperative hemodynamic instability, cardiac arrhythmias, and postoperative functional impairment of the vocal cords, resulting in a life-threatening airway compromise. CASE DESCRIPTION: A 68-year-old female patient was referred to the otorhinolaryngology…

[Training IADE students in managing preoperative anxiety].

Preoperative anxiety in patients is a field of knowledge and practice that state-certified nurse anesthetist students begin to learn about in the early stages of their training. Positive communication and conversational hypnosis are considered as part of a holistic approach, in the interests of quality of care, the caregiver-patient relationship, and the well-being of the caregiver themselves.

When Regional Anesthesia Met Pharmacokinetics.

Pharmacokinetics of Local Anaesthetic Agents. By Tucker GT, Mather LE. Br J Anaesth 1975; 47(suppl 1):213-24 Information derived from measurements of blood concentrations of local anaesthetics can be extended by the application of pharmacokinetic analysis. A better understanding of quantitative aspects of the disposition and absorption of these drugs should assist the anaesthetist in deciding the optimal agent and dosage for regional block techniques.
