[Antibiotic-prophylaxis in the minimally invasive pain surgical procedures].

The minimally invasive pain surgical procedures are more and more frequently used in the treatment and management of the chronic pain. The patients will often have recourse to a higher infection's risk during the proceedings for acquired general conditions (like enterotomy, skin ulcers, bladder catheter). The analysis literature doesn't produce specific treatment guidelines about antibiotic prophylaxis in pain therapy. This document, drawn up with multidisciplinary approach, correspond to rational, efficacy and functional guide about the choice and management of the antibiotic prophylaxis durin…

Antibiotic prophylaxis for patients undergoing elective endoscopic retrograde cholangiopancreatography.

BACKGROUND: The use of prophylactic antibiotics before endoscopic retrograde cholangiopancreatography (ERCP) is recommended by all major international gastroenterological societies, especially in the presence of an obstructed biliary system. Their use is intended to decrease or eliminate the incidence of complications following the procedure, namely cholangitis, cholecystitis, septicaemia, and pancreatitis. OBJECTIVES: To assess the benefits and harms of antibiotics before elective ERCP in patients without evidence of acute or chronic cholecystitis, or acute or chronic cholangitis, or severe

Adherence to perioperative antibiotic prophylaxis among orthopedic trauma patients.

BACKGROUND: The goal of this study was to assess whether patients receive their antibiotic prophylaxis as prescribed. We also investigated what doses and durations of antibiotics are typically ordered, which patients actually receive antibiotics and factors causing the ordered antibiotic regimen to be altered. METHODS: We performed a retrospective review of 205 patient charts and sent a national survey to all surgeon members of the Canadian Orthopaedic Trauma Society (COTS) about antibiotic prophylaxis in the setting of surgical treatment for closed fractures. RESULTS: In all, 93% (179 of 19…

Antibiotic prophylaxis with meropenem after allogeneic stem cell transplantation.

In the present study, we analyze the efficacy of prophylaxis with meropenem in patients receiving a matched related donor allogeneic transplant. In total, 38 patients were sequentially treated with meropenem starting on the day of the first febrile episode (n=17, group A) vs prophylactic meropenem starting on the first day with <500/mm(3) granulocytes (n=21, group B), and maintained until resolution of fever or after granulocyte count >500/mm(3). Of these, 16 (94%) patients in group A developed fever as compared to 16 (76%) in group B (P=0.02). While only one patient in group A did not require

[Health Economical Evaluation of antibiotic prophylaxis against postoperative endophthalmitis].

BACKGROUND: Two independent epidemiological studies recently suggested the prophylactic relevance of an intraocular antibiosis against endop … dophthalmitis cases by the antibiotic intervention. CONCLUSIONS: From an economical point of view, antibiotic prophylaxis can be suggested; the risk of longitudinally reduced antibiotic effectiveness of the antibiotic agents, however, strongly calls for an overall health political decision rather than for an immediate implementation of the corresponding intervention pr …

Antibiotic prophylaxis for pit viper envenomation: prospective, controlled trial.

The efficacy of prophylactic antibiotics for preventing infectious complications at the site of pit viper envenomation has not been well studied. We undertook a prospective, controlled trial of antibiotic treatment versus no antibiotic treatment among 114 victims of crotalid envenomation in Ecuador's Amazon rain forest. A group of 59 patients received intravenous gentamicin and chloramphenicol, and 55 patients did not. All other aspects of care were identical. There were no statistically significant differences between antibiotic-treated and untreated patients with regard to demographics, dela

[Perioperative antibiotic prophylaxis in breast surgery: cefotiam versus clindamycin].

97 patients undergoing breast surgery received either 2 g cefotiam or 1200 mg clindamycin as i.v. singleshot application in the course of an open randomised comparison of parallel groups to evaluate the efficacy and tolerability of both antibiotic regimens. Both regimens proved to be well to-lerated and equally suitable for the prophylaxis of postoperative infections in breast surgery.

Antibiotic prophylaxis for bacterial meningitis: overuse and uncertain efficacy.

BACKGROUND: There is little evidence supporting the efficacy of prophylactic antibiotics in preventing secondary cases of bacterial meningit … early symptoms of meningitis, as well as an explanation of the rationale behind the prescribing of antibiotic prophylaxis to contacts. This may reduce the likelihood of unnecessary prescribing and subsequent complications.

Prophylaxis for infective endocarditis: antibiotic sensitivity of dental plaque.

The antibiotic sensitivity pattern of bacteria isolated from bacteraemia after dental extraction was compared with that of bacteria isolated from dental plaque samples from the same patient. The results supported the current practice of using penicillin and erythromycin empirically for prophylaxis. The prediction of the most appropriate antibiotic for prophylaxis using dental plaque samples was most accurate when the minimum inhibitory concentration (MIC) of plaque isolates were used. It appears that a predictive test using dental plaque may be of some value in patients with recent or long-ter

Perioperative antibiotic prophylaxis, prostate size and transperineal prostate biopsy.

INTRODUCTION: Transperineal prostate biopsy (TPPB) is increasingly used for prostate cancer diagnosis due to low postoperative infection rates. However, the necessity of perioperative antibiotics remains debated. Although infectious complications such as urinary tract infections (UTIs) are uncommon, acute urinary retention (AUR) is a recognized complication. This study compared infection rates between patients undergoing TPPB with versus without perioperative antibiotics and evaluated rates of AUR and its association with prostate size. METHODS: We conducted a multi-institutional retrospectiv

Absorbable antibiotic bead prophylaxis for implant-based breast reconstruction.

INTRODUCTION: Periprosthetic infection is a serious complication in implant-based breast reconstruction (IBR). Local antibiotic delivery using biodegradable beads is a potential strategy to reduce infection risk. This study describes a single institution's preliminary experience with absorbable antibiotic beads for preventing infection in IBR. METHODS: A retrospective cohort study was performed of patients who underwent IBR between January 2021 and October 2023. Patients who met high-risk criteria for infection (BMI >30 kg/m2, prior smoking history, diabetes, and history of radiation) receive

Antibiotic Prophylaxis Against Infective Endocarditis Before Invasive Dental Procedures.

BACKGROUND: Antibiotic prophylaxis (AP) before invasive dental procedures (IDPs) is recommended to prevent infective endocarditis (IE) in those at high IE risk, but there are sparse data supporting a link between IDPs and IE or AP efficacy in IE prevention. OBJECTIVES: The purpose of this study was to investigate any association between IDPs and IE, and the effectiveness of AP in reducing this. METHODS: We performed a case-crossover analysis and cohort study of the association between IDPs and IE, and AP efficacy, in 7,951,972 U.S. subjects with employer-provided Commercial/Medicare-Suppleme…

Antibiotic prophylaxis for preventing bacterial endocarditis following dental procedures.

BACKGROUND: Infective endocarditis is a severe infection arising in the lining of the chambers of the heart. It can be caused by fungi, but … ce by the National Institute for Health and Care Excellence (NICE) in England and Wales states that antibiotic prophylaxis against infective endocarditis is not recommended routinely for people undergoing dental procedures. This is an update of a review that we first conducted in 2004 and last updated in 2013. OBJECTIVES: Primary objective To determine whether prophyla …

Is antibiotic prophylaxis necessary in mid-urethral sling surgery?

INTRODUCTION AND HYPOTHESIS: Antibiotic prophylaxis is commonly used when implanting a mid-urethral sling (MUS) for female stress urinary incontinence. Use of antibiotics may lead to adverse events and the development of antibiotic resistance. This study compared a variety of outcomes after MUS surgery with and without antibiotic prophylaxis using data from the national Norwegian Female Incontinence Registry. METHODS: Preoperative and 6-12 months postoperative subjective and objective data from 28,687 patients who received MUS surgery from 1998 through 2017 were extracted from the registry. C…

Bacterobilia in pancreatic surgery-conclusions for perioperative antibiotic prophylaxis.

BACKGROUND: Jaundice or preoperative cholestasis (PC) are typical symptoms of pancreatic masses. Approximately 50% of patients undergo preoperative biliary drainage (PBD) placement. PBD is a common cause of bacterobilia (BB) and is a known surgical site infection risk factor. An adjustment of preoperative antibiotic prophylaxis (PAP) may be reasonable according to the profile of BB. For this, we examined the microbiological findings in routine series of patients. AIM: To investigate the incidence and profile of biliary bacterial colonization in patients undergoing pancreatic head resections. …

Avoiding Inadequate Intrapartum Antibiotic Prophylaxis for Group B Streptococci.

OBJECTIVE: To estimate the frequency and reasons for inadequate group B streptococci (GBS) prophylaxis at our institution and to estimate what proportion of cases can be avoided with perfect protocol adherence. METHODS: This was a retrospective cohort study of neonates born to GBS-colonized women who received inadequate prophylaxis between April 30, 2013, and May 1, 2014. The maternal chart was analyzed to categorize each case as avoidable (adequate time on labor and delivery to receive antibiotics 4 hours before birth and β-lactam antibiotic-eligible) or unavoidable and to determine whether

Compliance with surgical antibiotic prophylaxis guidelines in pediatric surgery.

INTRODUCTION: Surgical antibiotic prophylaxis (AP) guidelines balance the need to prevent infection with the risks of adverse drug effects. Our aim was to assess compliance with AP guidelines. METHODS: A retrospective study was performed in a pediatric medical center. Included were patients aged 0 to 18 years that underwent clean-contaminated surgery during a 1-year period (2008-2009) and required AP. Compliance with four AP bundle guidelines was evaluated. Risk factors for noncompliance were identified using univariate and multivariate analyses. RESULTS: AP was given to 239 of 247 (96.8%) o…

Antibiotic prophylaxis for preventing post solid organ transplant tuberculosis.

BACKGROUND: Organ transplant recipients are at increased risk of infection as a result of immunosuppression caused inadvertently by medical treatment. Tuberculosis (TB) is a challenging infection to manage among organ transplant recipients that can be transmitted from infected people or triggered from latent infection. Organ transplant recipients have been reported to be up to 300 times more likely to develop TB than the general population. Consensus about the use of antibiotic prophylaxis to prevent post solid organ transplant TB has not been achieved. OBJECTIVES: This review assessed the be…
