Alterations in resting-state neural networks in subacute non-fluent aphasia after stroke: a fNIRS study.

BACKGROUND: Post-stroke aphasia is a prevalent and often disabling language impairment. Despite its high incidence, the neuropathological mechanisms underlying post-stroke aphasia and the neural substrates of functional recovery remain poorly understood. Understanding these mechanisms is essential for developing targeted rehabilitation strategies. METHODS: In this study, we used fNIRS to collect resting-state data from three participant groups. Pearson correlation was applied to compute whole-brain functional connectivity, followed by graph theory analysis to compare global and regional netwo

Effect of feedback type and self-selected practice schedule on speech fluency during aphasia script treatment.

INTRODUCTION: Aphasia, particularly in persons with nonfluent aphasia (PWNA), disrupts speech fluency. Script treatment-delivered with recursive self-feedback (RSF) or external feedback (EF)-has been shown to improve fluency in PWNA. Evidence from anomia treatment suggests that structured practice schedule (continuous vs. discontinuous) may further influence outcomes. However, it remains unknown how feedback type and PWNA's self-selected practice schedules-reflecting the variability of real-life routines-interact to influence language outcomes. This study introduces a novel self-selected pract

Word Count Matters: Features of Written Language Production in Progressive Apraxia of Speech with and without Agrammatism.

UNLABELLED: <p>Introduction: Apraxia of speech (AOS) is a motor speech disorder characterized by sound distortions, substitutions, deletions, and additions; slow speech rate; abnormal prosody; and/or segmentation between words and syllables. AOS can result from neurodegeneration, in which case it can be accompanied by the primary agrammatic aphasia (PAA), which when presenting together are called AOS+PAA. AOS can also be the sole manifestation of neurodegeneration, termed primary progressive AOS (PPAOS). Together these form the agrammatic-apractic spectrum disorders. Recent work has shown that

Type and frequency of phonetic errors in aphasics: therapeutic aspects.

Fifty aphasics were studied in three separate groups: nonfluent Broca, fluent Wernicke and fluent anomics in order to form a therapeutic guide of their phonetic errors (omission substitution, addition, repeated phonemes, perseveration of phonemes belonging to the preceding word, reversed order, phonematic targetting and misplacing. Frequency of phonetic error was also assessed.

Influence of written form on reading comprehension in aphasia.

Reading comprehension of 20 aphasic patients was systematically assessed using equivalent stimuli written in both print and cursive form. A Single-Word-to-Picture Matching task and a Sentence-to-Picture Matching task were employed. Performances of the aphasics were not uniformly facilitated by one written form over the other. Furthermore, the reading comprehension of the majority of individual patients was not influenced by written form. Four of the subjects, however, did appear to display real performance discrepancies when reading cursive versus print, particularly on the Sentence-to-Picture

[Several methodologic aspects of the problem of aphasia].

Main concepts of aphasia are analysed. It is shown that each of them emphasizes analytically certain aspects of this complicated problem. It is maintained that the one-sided view on this problem is overcome historically in the context of developing dialectico-materialistic understanding of the scientific psychophysiological problem. It is proven that the intergrative methods of investigating speech functions are, of the greatest diagnostic value.

Predicting clinical decline in progressive agrammatic aphasia and apraxia of speech.

OBJECTIVE: To determine whether baseline clinical and MRI features predict rate of clinical decline in patients with progressive apraxia of speech (AOS). METHODS: Thirty-four patients with progressive AOS, with AOS either in isolation or in the presence of agrammatic aphasia, were followed up longitudinally for up to 4 visits, with clinical testing and MRI at each visit. Linear mixed-effects regression models including all visits (n = 94) were used to assess baseline clinical and MRI variables that predict rate of worsening of aphasia, motor speech, parkinsonism, and behavior. Clinical predic

[Current status of aphasia therapy].

Aphasia therapy in adults has been established to a larger extent relatively lately in the history of aphasiology, i.e. after its social medical importance had been realized and one of the cardinal problems of neurology solved more satisfactorily--lesion localization by imaging techniques. In order to evaluate the efficiency of aphasia therapy--which is still not quite uncontradicted--it was necessary to acquire sufficient knowledge of the spontaneous recovery process. It takes place--e.g. after stroke--mainly during the first 3 months, coming, as a rule, to a halt during the first year. Longe

Speech re-education in patient with motor aphasia in collaboration with family.

The presented paper is an example of family education in managing the patient with motor aphasia following the stroke. The neurological diagnosis confirmed a medium hemiparesis on the right side. During the third week following the stroke, the re-education of speech was taken up basing on the new style of working with patient, i.e. biopsychosocial model of illness and treatment. In this aspect patient is the subject and the therapy is addressed to the patient taking into account his system of values with reference to the illness. The aim of the study--speech therapy--was achieving a maximum in

The validity of Barlow's 1877 case of acquired childhood aphasia: case notes versus published reports.

In 1877, Barlow described a ten-year-old boy with right hemiplegia and aphasia, quick recovery of language function, and subsequent left hemiplegia and aphasia, who was shown to have symmetrical left and right Broca's area lesions at autopsy. The report of this case motivated many writers in the second half of the nineteenth century to develop theories on localization, laterality, equipotentiality and development of specialization, recovery of function, and the role of the right hemisphere (see Finger et al., 2003, for review). This paper presents an analysis of the original archived case note

Infectious endocarditis complicated by an ischemic stroke and revealing Marfan syndrome.

Marfan syndrome is a systematic genetic disease of the connective tissue. The cardiac affection would predict the prognosis and ischemic stroke might complicate it. The purpose of this work is to discuss the mechanisms of the ischemic stroke in Marfan syndrome which have to be considered in all young patients of ischemic strokes. We report the case of a 17-year-old male patient who presented with right hemiparesis with brachio-facial dominance, hemihypoesthesia and Broca's aphasia; these symptoms were followed by partial right somato-motor epileptic seizure. The cerebral computed tomographic (

An approach to analyzing a single subject's scores obtained in a standardized test with application to the Aachen Aphasia Test (AAT).

Methods for the analysis of a single subject's test profile(s) proposed by Huber (1973) are applied to the Aachen Aphasia Test (AAT). The procedures are based on the classical test theory model (Lord & Novick, 1968) and are suited for any (achievement) test with standard norms from a large standardization sample and satisfactory reliability estimates. Two test profiles of a Wernicke's aphasic, obtained before and after a 3-month period of speech therapy, are analyzed using inferential comparisons between (groups of) subtest scores on one test application and between two test administrations fo

Some controversies about type and nature of aphasic symptomatology in Landau-Kleffner's syndrome: a case study.

The syndrome of acquired aphasia and convulsive disorders in childhood has received ample discussion since the initial description by Landau and Kleffner in 1957. Despite numerous investigations considering the linguistic and epileptic phenomena of the syndrome, the symptomatology still remains of a puzzling nature. Two aspects of the syndrome, the acquired nature of the aphasia and the type of linguistic symptomatology, have not been discussed thoroughly. In our case not only a history of developmental language pathology was present but language symptomatology also revealed a predominantly mo

New patterns of pathological language mixing in bilingual aphasia.

BACKGROUND: A central challenge in bilingual aphasia research is the absence of clear markers that distinguish pathological language mixing (PLM) from mixing used as a compensatory mechanism, leaving ongoing debates unresolved. Also, prior work suggests that PLM occurs primarily during retrieval of open-class words and increases with utterance length during structured tasks, yet these findings, and the hesitation behaviors accompanying PLM, remain largely untested in spontaneous speech. METHOD: This study examines PLM in Patient P3, a 59-year-old female bilingual speaker of Spanish and Englis

Working memory training in post-stroke aphasia: Near and far transfer effects.

PURPOSE: Individuals with aphasia (IWA) show various impairments in speech, language, and cognitive functions. Working memory (WM), a cognitive system that functions to hold and manipulate information in support of complex, goal-directed behaviors, is one of the impaired cognitive domains in aphasia. The present study intended to examine the effects of a WM training program on both memory and language performance in IWA. METHOD: This quasi-experimental study with an active control group was performed on 25 people with mild or moderate Broca's aphasia aged 29-61 years resulting from left hemis

Benzodiazepine receptor imaging with iomazenil SPECT in aphasic patients with cerebral infarction.

To investigate the relationship between prognosis of aphasia and neuronal damage in the cerebral cortex, we evaluated the distribution of central-type benzodiazepine receptor (BZR) binding in post-stroke aphasics with [123I]iomazenil and SPECT. We performed iomazenil SPECT in six aphasic patients (aged from 45 to 75 years; all right-handed) with unilateral left cerebral infarction. Three patients showed signs of Broca's aphasia and the other three Wernicke's aphasia. Cerebral blood flow (CBF) imaging was performed with [123I]iodoamphetamine (IMP). The regions of interest (ROIs) on both images

rTMS treatments combined with speech training for a conduction aphasia patient: A case report with MRI study.

RATIONALE: To date, little is known regarding the neural mechanisms of the functional recovery of language after repetitive transcranial magnetic stimulation (rTMS) in aphasia. Our aim was to investigate the mechanism that underlies rTMS and speech training in a case report. PATIENT CONCERNS AND DIAGNOSES: We report the case of a 39-year-old woman who was initially diagnosed with conduction aphasia following a left hemisphere stroke. INTERVENTIONS: The rTMS location comprised the left Broca area, and a frequency of 5 Hz for 20 min/d for 10 days during a 2-week period was used. She had receiv

Variation in the pattern of omissions and substitutions of grammatical morphemes in the spontaneous speech of so-called agrammatic patients.

We describe the patterns of omissions (and substitutions) of freestanding grammatical morphemes and the patterns of substitutions of bound grammatical morphemes in 20 so-called agrammatic patients. Extreme variation was observed in the patterns of omissions and substitutions of grammatical morphemes, both in terms of the distribution of errors for different grammatical morphemes as well as in terms of the distribution of omissions versus substitutions. Results are discussed in the context of current debates concerning the possibility of a theoretically motivated distinction between the clinica

Stroke caused by cerebral amyloid angiopathy: a case of difficult diagnosis.

Cerebral amyloid angiopathy is a common cause of intracerebral hemorrhage, most often occurring in older adults and in patients with dementia. One of its main differential diagnoses is hypertensive intracerebral hemorrhage. This is a case report of a hypertensive male patient who presented to the emergency department with short-term amnesia and Broca's aphasia. The initial diagnosis was hemorrhagic stroke of hypertensive etiology. However, subsequent diagnostic tests revealed that the bleeding was due to amyloid angiopathy. This case highlights the similarities and differences in the diagnosis
