Incidence of appendiceal neoplasms in appendectomy patients.

BACKGROUND: Non-operative management has been suggested as a therapy for uncomplicated appendicitis. Notwithstanding, the risk of missing an appendiceal tumor must be considered, being the surgical piece crucial to rule out neoplasms. Therefore, we aim to determine the incidence of appendiceal neoplasms in patients with acute appendicitis, tumor types and the importance of the anatomopathological study of the surgical piece. STUDY DESIGN: Retrospective study in which we described patients who underwent emergent appendectomy with histopathological findings of appendiceal neoplasms from January…

Appendectomy in Management of Malrotation of Gut is it Necessary?

INTRODUCTION: Malrotation is a congenital anatomical anomaly that affects the normal positioning of the intestines. Traditional management of malrotation, as described by Ladd, consists of detorsion of the volvulus if present, division of Ladd's bands, widening of the mesenteric root, proper positioning of the small and large bowels, and a prophylactic appendectomy. This study was done to determine whether appendectomy should be an integral part of the Ladds procedure or if it can be avoided. MATERIALS AND METHODS: This retrospective observational study was conducted in one pediatric surgical…

Magnetic Appendix: An Uncommon Indication for Appendectomy.

Foreign object ingestions are a common occurrence in pediatrics, often necessitating endoscopic or surgical intervention. The ingestion of multiple magnets poses an increased risk for serious complications. Our article presents a case of a five-year-old boy who swallowed two pennies and four magnets. The latter failed to pass spontaneously and were lodged in the appendiceal orifice resulting in a challenging and unsuccessful endoscopic retrieval and hence required laparoscopic exploration, appendectomy, and partial cecal resection.

Amyand hernia repair with mesh and appendectomy.

BACKGROUND: Amyand hernias have been described in case reports in the literature and are a rare occurrence in the career of a surgeon. Their management is even less well described and often poses problems in the management of repair due to concerns with contamination associated with appendectomy. Here, we describe a patient with an Amyand hernia seen on CT imaging preoperatively, who underwent appendectomy along with a lightweight mesh plug repair of his hernia. CASE PRESENTATION: An 88-year-old male presented with a right groin bulge and underwent preoperative imaging which indicated the pre…

[Laparoscopic appendectomy for mucinous cystadenoma of the appendix].

Treatment of acute and chronic appendicitis is still an actual problem. There are some rare courses of the disease besides well-known complications of appendicitis. Mucocele is one of the rarest forms of chronic appendicitis. Mucocele is not accompanied by clinical symptoms and diagnosed accidentally in more than 25% of cases. The most serious complication of mucocele of the appendix is malignization observed in up to 36% of cases. Ultrasound, CT and colonoscopy are the most effective methods of perioperative diagnosis. Unclear symptoms, delayed diagnosis and surgical treatment increase the ri

Predictive factors and outcomes of negative appendectomy.

BACKGROUND: The aim of this study was to assess predictive factors for negative appendectomy and to evaluate the outcomes of negative appendectomy. METHODS: A retrospective chart review of 4,878 patients who underwent appendectomy at our institution from January 2008 to December 2014 was performed. RESULTS: Younger age (≤15 years), normal white blood cell count, appendix diameter of less than 6 mm on computed tomography (CT), and CT grade less than 3 were found to be independent predictive factors for negative appendectomy. When complications were investigated according to the results of pat…

Laparoscopic appendectomy in complicated appendicitis: Is it safe?

BACKGROUND: Because of lack of good evidence supporting laparoscopic approach for complicated appendicitis, we carried out this study to evaluate efficacy of laparoscopic appendectomy (LA) in management of patients with complicated appendicitis. MATERIALS AND METHODS: This study was carried out in Surgical Department, Minia University, Egypt involving 214 patients underwent appendectomy for complicated appendicitis over three years. 132 patients underwent LA and remaining 82 patients underwent OA. Parameters studied included operating time, return to oral feeding, postoperative pain, wound in…

Laparoscopic appendectomy in the treatment of acute appendicitis.

BACKGROUND: Laparoscopic appendectomy (LA) has many advantages over the classic appendectomy (CA), but this method has not been accepted yet in Bosnia and Herzegovina. Therefore, we attempted controlled randomized study in order to compare classic appendectomy with laparoscopic appendectomy and confirm eventual advantages of caring base of appendix with hem-o-lok clips regarding ordinary accepted endoloop method during laparoscopic appendectomy. METHODS: In this prospective study 120 patients were involved which are divided into two groups. In group 1, 60 patients were operated with classic m…

[Surgical techniques of appendectomy for acute appendicitis].

Over the last 20 years, the treatment of acute appendicitis has been transformed by the development of the laparoscopic approach. And yet the net value of this approach continues to be debated. Laparoscopic appendectomy is associated with a lower complication rate and a shorter period of disability in the general population. While operative costs are higher, the global cost of laparoscopic appendectomy is lower than for open appendectomy. There is a somewhat higher rate of abdominal abscess for the laparoscopic route. Laparoscopic appendectomy shows clear advantages in obese patients and in th…

Failure of appendectomy to resolve appendiceal intussusception.

Appendiceal intussusception is a rare entity that primarily affects children. The optimal surgical management is not known. We describe a child with appendiceal intussusception treated by simple appendectomy who suffered recurrent intussusception of the cecum at the base of the appendiceal stump. He was cured by resection of a rim of cecum around the appendiceal staple line. Partial cecectomy with appendectomy may be the most effective operation for appendiceal intussusception.

Results of emergency appendectomy for appendicular mass.

Emergency appendectomy was done on 1142 patients during the period of July 1990 to January 2003 to evaluate the results. Of which 656(57.44%) were male and 496(42.56%) were female. The mean age was 22.21+/-3.93 years (04-85 years). The duration of pain before admission was 3.05+/-0.94 days (01-17 days) and 708(62%) patients presented with palpable mass; ultrasonogram revealed additional lump in 114(9.98%) patients, rest of the lump (28.02%) was detected during operation. 342(30%) patients had appendix abscess and 228(19.96%) had loculated collection. Eight patients had tuberculosis and four ha…

Role of appendectomy in training for laparoscopic surgery.

BACKGROUND: This study aimed to evaluate a program of training in laparoscopic surgery based on clinical practice in the emergency room, in which laparoscopic appendectomy is the first technique that residents perform as surgeons. METHODS: A prospective nonrandomized study was conducted involving all the laparoscopies performed in emergencies with a diagnosis of acute abdomen, appendicular in origin, during the period between June 1991 and December 1997. RESULTS: There were no statistically significant differences between residents and assistants in terms of conversion rates (22/242 vs 15/15…

Current status of laparoscopic appendectomy in children.

Acute appendicitis is the most common surgical emergency in children. Open appendectomy has been the standard treatment for decades, with excellent results. Owing to advances in pediatric laparoscopic instrumentation and increased experience with the technique of laparoscopy, however, many centers are routinely performing laparoscopic appendectomy in children. Most studies show a marginal advantage of laparoscopic appendectomy over open appendectomy with regard to length of hospitalization, postoperative analgesia use, length of time until return to full activity, and wound infection rate. The…

Laparoscopic appendectomy (LA) with a new technique.

UNLABELLED: The technique of LA has evolved during the past decade. We describe a simplification using a scope with work channel. TECHNIQUE: A 1.2 cm paraumbilical trocar enters the CO2(-)-inflated peritoneum. The 1 cm scope with work channel is introduced. If the view confirms the diagnosis of appendicitis we place a 1-cm trocar in the midline 2 cm above the symphysis pubis. The scope is switched to the 1 cm trocar. The appendix is immobilized through the work channel. Through the paraumbilical site the appendiceal artery is clipped or coagulated as necessary. The stump is closed by Roeder l

[Initial experience with laparoscopic appendectomy in children].

The article generalizes the first experience in successful laparoscopic appendectomies in children carried out by the method suggested by F. Götz et al. which we modified and adapted to pediatric practice. In all cases of acute appendicitis the laparoscopic manipulation began with the diagnostic stageo by performing two abdominal punctures with small-diameter trocars. Laparoscopic appendectomy was undertaken only in recognized destructive appendicitis. It was conducted 205 times in children 2 to 14 years old in various localization of the vermiform process, including subhepatic and retroperine…

Is there a role for laparoscopic appendectomy in pediatric surgery?

Although laparoscopic procedures are currently in vogue in general surgery, the role of this approach in children has not been prospectively evaluated in the United States using the new instrumentation now available to us. To assess the value of laparoscopic appendectomy (LA) in childhood, we prospectively compared 14 LAs with 50 open appendectomies (OA) over 6 months in a single children's hospital. Antibiotic usage was at the discretion of the surgeon regardless of the procedure performed and was not different between groups. LA was performed under the direction of a single laparoscopy-train…

[Is concomitant preventive appendectomy in childhood justified?].

Appendicitis is an entity without specific symptoms. Multiple false diagnoses result from this, especially in childhood, in pregnancy, and in old age. We discuss whether incidental protective appendectomy is recommended in childhood in small bowel obstructions due to intussusception, comparing the data obtained from 100 children who were appendectomized with those of 32 children who were not.

Appendiceal calculi and fecaliths as indications for appendectomy.

This study was done to determine the association between appendiceal fecaliths or appendiceal calculi and the presence of acute appendicitis. We retrospectively reviewed the medical records of all patients from 1977 through 1986 who underwent appendectomy for a clinical diagnosis of acute appendicitis (group 1, 2,331 patients) or who underwent appendectomy either incidentally or during colectomy for nonappendiceal pathologic factors (group 2, 1,066 patients). Fecaliths were six times more common than calculi, but calculi were more often associated with perforated appendicitis or periappendicea…

Guidelines for therapeutic decision in incidental appendectomy.

Incidental appendectomy is contraindicated in patients whose conditions are unstable, patients previously diagnosed with Crohn's disease, patients with an inaccessible appendix, patients undergoing radiation treatment, patients who are pathologically or iatrogenically immunosuppressed and patients with vascular grafts or other foreign material. In patients ten to 30 years of age--the age group associated with a higher incidence of acute appendicitis--who are otherwise healthy, incidental appendectomy is effective in preventing morbidity and death associated with acute appendicitis. In patients…
