Three subjects from 2 unrelated families with partial duplication of 17q, derived from a reciprocal parental translocation between chromosomes 11 and 17 with different breakpoints, are described. A female patient from one family with a 46,XX,-11,+der(11),t(11;17)(q24;q23.2)pat chromosome complement had died at 2 months of age. In the second family, a male propositus and a subsequent fetus, identified by cytogenetic prenatal diagnosis, showed a 46,XY,-11,+der(11),t(11;17)(q2505,q24.3) mat chromosome complement. The repeated failure of forearm hemodialysis access grafts in patients with end-stage renal failure often requires the use of an upper arm graft fistula. During a 7-year period, 20 upper arm graft fistulas were placed in 15 patients. The 5-year patency rate by life-table analysis, including graft fistula salvage by thrombectomy, is 53%. The mean survival from graft insertion until revision or thrombectomy is 36 months. Twenty of the twenty-two thrombectomies performed (91%) were successful in reestablishing graft fistula patency. Of 40 stroke patients attending a rehabilitation department, 32 (80%) had associated reactions affecting the hemiplegic arm. These involuntary movements nearly always occurred in association with yawning and less frequently with stretching, coughing, sneezing and laughing. This report is concerned with a male motor bike rider who was injured in a collision with a semi-trailer in June 1974. As a result of his injuries he had his right arm amputated, his right knee restricted, and his right foot partially amputated and pinned. This article presents the subsequent management of the patient which was aimed at achieving an acceptable foot and preventing further breakdown in other body linkages. The design of appliances and treatment were carried out by the physiotherapist, the chiropodist, and the bioengineer.

Fractures of the humeral shaft in various throwing sports is a well recognized clinical entity. The specific etiology remains uncertain. Competitors engaged in Indian and/or wrist wrestling, a sport receiving increased interest, also are known to sustain humeral fractures. It is thought that these particular fractures are the result of bending moment, axial compression, and torsional forces applied to the humerus. The position of the arm during the match appears to be critical in determining the fracture pattern and location. Closed treatment was successful in all reported cases.

Three cases of the cat-cry syndrome are reported and the literature on Bp- syndromes is reviewed. Although the 4p- and 5p- syndromes have many overlapping features, the cleft lip-palate, seizures, iris coloboma, and other features of the 4p- syndrome, as well as absent cat-cry, are frequently sufficient to distinguish between the 4p- and 5p- syndromes.

A 28-year-old male was referred to our emergency department with a swollen left arm after intensive exercise. Physical examination revealed distension of the superficial veins of his left arm and chest. Duplex ultrasound confirmed the diagnosis of Paget-Schroetter syndrome. He was successfully treated with thrombolysis and first rib resection.

Pilomatrixoma is a benign tumor of hair follicles that usually occurs as a single tumor and appears in the head and neck region, extremities, and trunk. The authors present a case of pilomatrixoma with multifocal localization. Simple excision of lesions is the recommended procedure, and recurrence usually is rare.
