Reactive arthritis.

During the past year, new information has been obtained regarding the pathogenetic process in reactive arthritis. Characterization of the triggering microbes as well as of their interaction with T cells and other host components has brought us closer to achieving a comprehensive picture of the events leading from infection to reactive arthritis. Efforts are ongoing to define clinically applicable diagnostic criteria. It is becoming apparent that chronic forms of the disease are not uncommon.

Viral arthritis.

Viral infections can present with different patterns of joint and soft tissue involvement, and the etiologic role of viruses in various rheumatic diseases is a subject of continued great interest. Recently, new immunoenzymatic assays have brought a better understanding of the relationship between hepatitis C virus serotypes and their immunologic manifestations. Our knowledge of the consequences of parvovirus B19 infection has broadened to include the variable clinical spectrum the role of inflammatory cytokine production in parvovirus-induced arthritis, a postulated causative role for B19 in r…

Bacterial arthritis.

A number of experimental models of bacterial arthritis demonstrated that immune factors, especially directed cytokines, play an important role in cartilage destruction. The most important studies in bacterial arthritis were a review of the clinical manifestations of gonococcal arthritis and two reports of the use of polymerase chain reaction to detect Neisseria gonorrhoeae DNA in synovial fluid. Polymerase chain reaction may be an important diagnostic test in culture-negative cases and may be very helpful in understanding the pathophysiology of gonococcal arthritis.

Crystal arthritis.

Diagnostic and classification criteria have the purpose of separating patients with a certain disease from those without the condition and from normal subjects. Specific identification of the different microcrystals establishes the definite diagnosis of crystal arthritides. The diagnostic criteria for gouty arthritis set up by the American Rheumatism Association in 1975 function well; both their sensitivity and specificity are satisfactory. Unfortunately, we have had no data on the real value of the diagnostic criteria used in CPPD or in hydroxyapatite deposition disease. Status or ranking cri…

[Gonorrheal arthritis].

A 43-year-old female patient presented with migratory arthralgias, generalized exanthema and arthritis of the left elbow. She had a history of fever and chills three days earlier. Beside to a maculopapular and pustular rash and a joint involvement we found a swelling of the dorsal surface of her right hand without wrist effusion or synovitis. The clinical presentation was typical for the diagnosis of disseminated gonococcal infection (arthritis-dermatitis syndrome). Arthrocentesis was performed, and the cultures of the synovial fluid on chocolate agar were positive for Neisseria gonorrhoeae. T…

Infectious arthritis.

Infectious arthritis should be suspected in any patient with a swollen joint but especially in children, debilitated patients, immunocompromised persons, those with infection elsewhere (even if on antibiotics), and those with other types of arthritis or a prosthetic joint. Diagnosis depends on obtaining joint fluid for culture and Gram stain. Initial appropriate broad spectrum antibiotics are best narrowed to suit the individual organism, which 2 to 6 weeks of therapy can eradicate. Repeated needle aspiration or surgical drainage is usually helpful in lessening joint damage and achieving a mor…

[Bacterial arthritis].

During the years 1980-89, 13 patients were hospitalized because of bacterial arthritis of the peripheral joints. There were five juvenile patients (under 15 years of age). The remaining eight patients were adults, of whom seven were older than 50 years. The mean delay in establishing the diagnosis was four days (0-10 days). Monarthritis was found in 12 patients, and in one patient two joints were involved. The most commonly affected joints were the shoulder (four cases), knee (four cases), ankle (three cases), hip (two cases) and toe (one case). A predisposing condition was found in five patie…

Bacterial arthritis.

In this review of the 1990 septic arthritis literature, we revisit synovial fluid leukocytosis, examine the utility of synovial fluid glucose and protein measurements, and look at the levels of two cytokines, tumor necrosis factor and interleukin-1, in infected joint fluids. We see the many faces of gonococcal arthritis and the ravages of septic arthritis when the host has rheumatoid arthritis. Should we recommend antibiotic prophylaxis for the rheumatoid patient with a prosthetic joint who is undergoing a procedure that leads to transient bacteremia? What are some of the salient features of s…

Jaccoud's arthritis.

A 16 year old girl with multivalvular heart disease and recurrent episodes of polyarthritis with correctable deformities of hands and feet fitting into that of Jaccoud's arthritis is reported. The condition is rare and often difficult to differentiate from rheumatoid arthritis.

Rheumatoid arthritis.

RA is a common disease with a worldwide prevalence of about 1% with an annual incidence of about 3/10,000 adults. It is two to three times more common in women. There is some evidence that over the last few decades the disease is declining in incidence or severity. Patients with RA have a reduced life expectancy and a one in three chance of becoming disabled, depending on the severity of the disease at onset. The cause of the disease is unknown, although genetic factors account for up to 30% of disease susceptibility. The most important genetic factors are related to HLA-DR4 and DR1, which may

[Purulent arthritis].

At all ages, Staphylococcus aureus is the most common microorganism responsible for septic arthritis, but in children below five years of age Haemophilus influenzae is frequently found. In about 1/3 of the cases cultures are negative, primarily because of initiation of antibiotic therapy prior to sampling. A distended joint should be treated as infection until otherwise proved. Today repeated aspirations of the infected joint are recommended as initial treatment, if possible with additional joint lavage, together with systemic antibiotic therapy. In case with no clinical response within 2-4 da…

[Septic arthritis].

Septic arthritis is a serious medical problem that should be promptly recognized and requires appropriate treatment to avoid permanent joint damage. The disease is caused by several different microorganisms but the most frequent in children and in adults is Staphylococcus aureus. Pathogenetic mechanism, general (antiblastic therapy, diabetes, rheumatoid arthritis) and local (intra-articular injections) promoting factors are discussed. Usefulness of laboratory and radiological investigations is debated. Finally indications for needle aspiration, adequate drainage as well as guide lines on gener…

Infectious arthritis.

Any patient who presents with an acute monarticular arthritis, especially a new asymmetric effusion with underlying joint disease, should be suspected of having a bacterial process. Because synovial fluid findings (leukocyte counts and glucose) may not be predictive of infection, bacteriologic analysis by smear and culture is necessary in the evaluation of any new synovial effusion. A chronic monarticular process is highly likely to be infectious also, but mycobacterial or fungal etiologies frequently require appropriate culture of synovial tissue in addition to processing fluid. Acute polyart…

[Reactive arthritis].

Reactive arthritis is an abacterial form of arthritis that develops after infection at a site distant from the affected joint(s). The condition has long been recognised, often under the name of Reiter's syndrome. During recent years a number of research teams have published new findings concerning the pathogenesis of reactive arthritis which may also prove to be of importance for other rheumatic diseases. The clinical picture, treatment and prognosis of reactive arthritis are also discussed in the article.

Lyme arthritis.

Evanescent arthralgias and myalgias are common early symptoms of Lyme disease. Transient, intermittent episodes of monoarticular, oligoarticular, or sometimes migratory polyarticular arthritis, lasting weeks to months, with disease-free intervals, are frequently observed in untreated patients with erythema chronicum migrans. A minority of patients develop chronic joint involvement, most commonly affecting the knee. Antibiotic therapy given during erythema chronicum migrans is often protective with regard to late joint manifestations. In chronic Lyme arthritis, however, the response to antibiot…
