Interatrial septum dissection and atrial wall hematoma following transseptal puncture: A systematic review of the literature.

BACKGROUND: Interatrial septum (IAS) dissection due to transseptal puncture (TSP) is a rare, underreported complication of the procedure. Data on the mechanism, diagnosis, and management of this complication are lacking. METHODS: We conducted a systematic review of all reported cases of IAS dissection with or without associated LA hematoma due to TSP, by thoroughly searching MEDLINE and EMBASE through May 2019. RESULTS: After screening of n = 882 studies, eight studies with a total of 19 patients addressed the complication of IAS dissection and/or LA hematoma secondary to TSP. Median age was…

Percutaneous closure of multiple defects of the atrial septum: procedural results and long-term follow-up.

BACKGROUND: The percutaneous closure of single atrial septal defect (ASD) is a valid alternative to surgery. OBJECTIVES: To assess the feasibility of percutaneous treatment of multiple ASDs. METHODS: Between 1998 and 2007, 165 out of 1280 consecutive patients undergoing ASD percutaneous closure at our institution showed multiple defects that were classified in four categories: double atrial septal defects (d-ASD), multifenestrated atrial septal defects (f-ASD), multifenestrated defects with no signs of right heart overload (f-PFO), and complex cases (c-ASD). The following end points were tak

Rate-dependent slow conduction velocity in the cavo-tricuspid isthmus and septum in patients with atrial flutter.

PURPOSE: To evaluate and compare the effects of heart rate on conduction velocity in the cavotricuspid isthmus (CTI) and septum in patients with and without typical atrial flutter (AF) using electro-anatomic mapping (EAM) of the right atrium (RA). METHODS: Ten patients (age 53+/-10 yrs, 7M/3F) with AF and 13 patients (age 51+/-11 yrs, 5M/8F) with atrioventricular nodal reentrant tachycardia (AVNRT) underwent conventional electrophysiological study, electro-anatomic mapping and radiofrequency ablation. Using EAMs obtained during coronary sinus pacing at pacing cycle length (PCL) 600 ms, 400 ms

[Aneurysm of the interatrial septum with atrial septal defect, disclosed by pulmonary embolism complicated by paradoxical embolism].

The authors report the case of a patient admitted to hospital for bilateral phlebitis of the lower limbs, associated with an atrial septal defect. The presence of significant pulmonary hypertension accounted for the paradoxical embolism by inversion of the intracardiac shunt. No recurrence has been observed with a follow-up of six months on anticoagulant treatment.

Noninvasive tissue characterization: diagnosis of lipomatous hypertrophy of the atrial septum by nuclear magnetic resonance imaging.

An atrial septal mass was identified by echocardiogram in a patient with multiple subcutaneous lipomas. In order to differentiate the benign condition of lipomatous hypertrophy from myxoma, thrombus and other tumors, nuclear magnetic resonance and computed tomographic imaging were performed. Both techniques identified the adipose nature of the tissue noninvasively, consistent with the diagnosis of lipomatous hypertrophy. Pathologically demonstrated lipomatous hypertrophy in a postmortem heart was similarly characterized by nuclear magnetic resonance imaging.

Long-Term Outcomes of Unrepaired Isolated Partial Anomalous Pulmonary Venous Connection With an Intact Atrial Septum.

The natural history of an unrepaired isolated partial anomalous pulmonary venous connection(s) (PAPVC) and the absence of other congenital anomalies remains unclear. This study aimed to expand the understanding of the clinical outcomes in this population. Isolated PAPVC with an intact atrial septum is a relatively uncommon condition. There is the perception that patients with isolated PAPVC are usually asymptomatic, that the lesion generally has a limited hemodynamic impact, and that surgical repair is rarely justified. For this retrospective study, we reviewed our institutional database to id…

[Atrial septum defect and open foramen ovale. Diagnosis, clinical aspects and therapy using a catheter technique].

The purpose of this report is to describe the pathophysiology and clinical symptoms of atrial septal defects with emphasis on right ventricular failure and the risk of paradoxical embolism in the setting of a patent foramen ovale. The diagnosis of both the atrial septal defects and the patent foramen ovale, usually is made by echocardiography. Percutaneous transcatheter occlusion of an atrial septal defect and a patent foramen ovale has gained interest for some years as a treatment alternative to surgical closure. Preliminary results of different devices have been encouraging and the method ap

Reversed shunting across the ductus arteriosus or atrial septum in utero heralds severe congenital heart disease.

OBJECTIVES: This study was performed to define the significance of Doppler color flow mapping in demonstrating reversal of the direction of the normal physiologic flow across the atrial septum and ductus arteriosus in the human fetus. BACKGROUND: Reversal of the physiologic shunting across the ductus arteriosus or atrial septum in utero (i.e., left to right) can be readily identified by Doppler color flow mapping, complemented by pulsed and continuous wave Doppler information. METHODS: We reviewed echocardiograms recorded at our three institutions from 1988 to 1993, which displayed reversal …

[Surgical correction for partial anomalous pulmonary venous connection with intact atrial septum without using extracorporeal circulation].

A 13-year-old boy with partial anomalous pulmonary venous connection (PAPVC) having an intact atrial septum (IAS) is reported. He had open heart surgery 3 years ago at another hospital, but the cardiac septum was intact and PDA was ligated. After his discharge, cardiomegaly and increased right pulmonary vasculature were seen in chest roentgenograms. Our cardiac catheterization data and pulmonary angiograms showed that he had a PAPVC with IAS. Surgical correction was accomplished by utilizing a polytetrafluoroethylene graft with external ring support and interposing it between the right upper p…

Orientation of the Atrial Septum to the Inferior Vena Cava May Contribute to the Persistent Patency of the Foramen Ovale.

PURPOSE: There is growing evidence that paradoxical embolism through patent foramen ovale (PFO) is a cause for cryptogenic stroke. However, it is still unclear why the foramen ovale fails to close after birth. We studied whether the 3D relations between the atrial septum (AS) and the inferior vena cava (IVC) are associated with PFO. METHODS: We recruited 30 patients (18-49 years) with a first-ever cryptogenic stroke and 30 age- and sex-matched stroke-free controls. Using cardiac magnetic resonance, an approach to evaluate the 3D relations between the AS and the IVC was developed. The presence…

Imaging Assessment of the Interatrial Septum for Transcatheter Atrial Septal Defect and Patent Foramen Ovale Closure.

Transcatheter closure of atrial septal defects and patent foramen ovale has become increasingly common with advances in device and imaging t … ardiac echocardiographic modalities used for periprocedural and intraprocedural imaging of the interatrial septum.

Intervention in the critically ill neonate and infant with hypoplastic left heart syndrome and intact atrial septum.

Neonates that present with hypoplastic left heart syndrome (HLHS) and intact atrial septum (IAS) pose a major management problem for the pediatric cardiac team. They are critically ill newborns with profound hypoxemia and acidosis that require immediate attention. Controversy exists as to the most appropriate management strategy. In one series where a primary and emergent surgical-staged reconstructive procedure was performed, the in-house hospital mortality was 65% and the overall survival was 17%. With equal abysmal results, transcatheter creation of an atrial septal defect (ASD) using conve…
